BACKGROUND: During surgical treatment of lesions involving the skull base, conductive hearing is often sacrificed due to tympanum destruction. This study aimed to develop a method for tympanum reconstruction using a sternocleidomastoid (SCM) flap to preserve conductive hearing during lateral skull base surgery.
METHODS: This study included five patients with lateral skull base lesions who underwent surgery including tympanum reconstruction with an SCM flap between July 2015 and November 2017.
RESULTS: Three patients seen with facial nerve schwannoma, and two had paraganglioma of the head and

BACKGROUND: The main risk factors for postoperative failure in tympanoplasties are large perforations that are difficult to repair, annular perforations, and a tympanic membrane (TM) with extensive granular myringitis that require middle ear exploration and mastoidectomy. The aim of this study was to investigate a novel technique of perichondrium/cartilage composite graft for repairing the large TM perforation in the patient of otitis media.
METHODS: Retrospective chart reviews were conducted for 102 patients with large tympanic membrane perforations, who had undergone tympanoplasty from Augu

OBJECTIVES: Our objective was to analyze the results of tympanosclerosis surgery using over-underlay tympanoplasty and to find out the effect of single-stage surgery on hearing results.
MATERIALS AND METHODS: Forty-two patients who were operated on for tympanosclerosis between July 1998 and February 2002 were included in the study. These were one-stage tympanoplasties, because second-stage operations and revisions were not included. Operative records and audiograms of the patients were obtained. Of the patients, 48% had bilateral tympanosclerosis. In three (7.1%) of the patients, tympanoscler

UNLABELLED: The measurement of the piston is always described as a significant stage of the surgery but its length is never clearly defined.
OBJECTIVE: The goal of this work is to determine the length of the prosthesis.
MATERIALS AND METHODS: From May 2003 to May 2005, 128 patients with otosclerosis, diagnosed on the basis of their clinical history and their audiogram, were included in this prospective study. Some of them were revision surgeries. The prosthesis generally used was the Portmann piston clip (Medtronic Xomed Inc Portmann Piston Clip Stainless Steel/Fluoroplastic). In stapedectom

The malleus grip prosthesis is designed for ears with conductive hearing loss caused by pathologic conditions of the stapes in association with pathologic conditions of the malleus or incus. The malleus grip procedure was performed on 220 ears, and follow-up studies are available on 203. Mild to profound sensorineural hearing losses occurred in 16 (8.37%). The remaining 187, on the first postoperative test, had an average air-bone gap of 14 dB, and subsequent examinations on 100 of them showed no significant change with the passage of time. The procedure is technically more difficult than stap

The management of a unilateral congenital atresia of the external auditory canal is nonuniform and debated. Various surgical approaches, timing, coordination with microtia repair, and variable hearing improvements all contribute to the debate regarding management of this entity. This paper outlines our craniofacial team approach to the congenital unilateral atresia and microtia in children. Selection criteria, timing of repair, coordination with microtia repair, surgical results, and pitfalls will be discussed. The results of surgery in 16 patients with unilateral congenital atresia of the ext

PURPOSE: Studies have assessed the trauma and change in hearing function from the use of otological drills on the ossicular chain, but not the effects of partial laser ablation of the incus. A study of the effectiveness of a novel middle-ear microphone for a cochlear implant, which required an incus recess for the microphone balltip, provided an opportunity to compare methods and inform a feasibility study of the microphone with patients.
METHODS: We used laser Doppler vibrometry with an insert earphone and probe microphone in 23 ears from 14 fresh-frozen cadavers to measure the equivalent no

OBJECTIVE: To assess hearing rehabilitation in patients with congenital aural atresia using an active middle-ear implant (Vibrant Soundbridge).
METHODS: Of a cohort of 70 microtia and atresia patients, 10 underwent Vibrant Soundbridge implantation between 2008 and 2021. Two of the 10 patients had binaural implantation, resulting in 12 ears for analysis. Pre- and post-operative audiometry data were analysed, and patient satisfaction was evaluated. Surgical issues regarding coupling sites and outcomes were analysed.
RESULTS: Pure tone average (0.5, 1, 2 and 4 kHz) improved from a pre-operative

A case of traumatic incudostapedial joint dislocation with stapes footplate into the vestibular window and pneumolabyrinth patient history. Patients mainly presented with vertigo, hearing loss in the left ear, audible sound when the head was shifted to the left, and subsequent vertigo, manifested as a spiral rotation, accompanied by gait instability, a feeling of stepping on cotton, and decreased attention. Pure tone audiometry showed mixed hearing loss in the left ear and temporal bone CT demonstrated air density in the left vestibule. Then, the patient underwent exploratory tympanotomy, whic

Stimulating the round window membrane via an active actuator of the middle ear implant, named the reverse stimulation, has become an option to help patients with ossicular chain deformity (OCD) to restore hearing. However, there is still no concise description of how OCD affects reverse stimulation considering the overflow characteristics of third windows. In the present study, an impedance model considering the vestibular and cochlear aqueducts was used to investigate the dynamic response of the cochlea to reverse stimulation under OCD. First, a finite-element (FE) model of the middle ear and

Patients with otosclerosis can suffer from different grades of combined hearing loss. In addition to surgery (stapedectomy), conventional hearing aids can be used in the treatment of otosclerosis. In cases of severe conductive components in addition to sensorineural hearing loss, treatment with normal hearing aids can be difficult or impossible. In these patients, implantable hearing systems such as cochlear implants represent a possible alternative. The totally implantable Carina middle ear system can be used in patients with even high-grade sensorineural hearing loss. Based on two exemplary

HYPOTHESIS: The prosthesis length in malleostapedotomy for otosclerosis revision surgery can be calculated if the prosthesis length of previous conventional incus stapedotomy is known.
BACKGROUND: Malleostapedotomy is used in revision surgery for otosclerosis in case of severe incus erosion and malleus or incus fixation.
METHODS: Fifteen cadaveric human temporal bone specimens were investigated by means of micro-CT followed by 3D analysis. The distance between the incus and the stapes footplate as well as distances between the malleus and the footplate were measured and compared.
RESULTS: T

BACKGROUND: In continuation of our previously published report on initial experience with titanium implants in the middle ear [13], we now present the results of routine use over a three year period.
METHODS: From November 1994 to November 1997, 661 titanium implants (Düsseldorf model) were implanted in the authors' clinics (355 PORP, 306 TORP). There was no preselection of patients. Five hundred eighty-two follow up examinations were performed with a medium follow up time of 11.6 months.
RESULTS: The good initial results of the previous study with respect to biocompatibility and functional

To accomplish two objectives, morphological and functional improvements, in plastic operation of major ear anomalies, the authors presented a surgical method utilizing exclusively auto materials and several pedicles of skin, fascia and periostium. A partially implantable middle ear implantable hearing aid (Partial-MEI) is also an indication to those anomalies. Incidentally, we implanted the first Partial-MEI to a patient with unilateral atresia-microtia. The case was presented and demonstrated Partial-MEI implantation as one of the approaches for major ear anomalies to improve hearing.
