BACKGROUND: Individuals with co-occurring posttraumatic stress disorder (PTSD) and HIV are at high-risk for negative HIV-related outcomes, including low adherence to antiretroviral therapy, faster disease progression, more hospitalizations, and almost twice the rate of death. Despite high rates of PTSD in persons with HIV (PWH) and poor HIV-related health outcomes associated with PTSD, an effective evidence-based treatment for PTSD symptoms in PWH does not exist.

BACKGROUND: The postpartum period is a time for multiple clinical interventions. To date, no critical review of these interventions exists. This systematic review examined evidence for the effectiveness of postpartum interventions that have been reported in the literature.
METHODS: MEDLINE, Cinahl, PsycINFO, and the Cochrane Library were searched for randomized controlled trials of interventions initiated from immediately after birth to 1 year in postnatal women that were conducted in North America, Europe, Australia, or New Zealand. The initial literature search was done in 1999, using postp

OBJECTIVES: Randomized controlled trials are considered the fundamental basis of evidence-based medi-cine, especially in the field of organ transplantation. Given the increasing demand for organs and the distinct challenges associated with this topic, our aim was to systematically analyze and characterize the randomized controlled trial landscape in solid-organ transplantation and to identify dominant research trajectories and influential thematic areas.
MATERIALS AND METHODS: Using a bibliometric approach, we extracted randomized controlled trials specific to solid-organ transplantation from

BACKGROUND: In the hierarchy of research designs, the results of randomized, controlled trials are considered to be evidence of the highest grade, whereas observational studies are viewed as having less validity because they reportedly overestimate treatment effects. We used published meta-analyses to identify randomized clinical trials and observational studies that examined the same clinical topics. We then compared the results of the original reports according to the type of research design.
METHODS: A search of the Medline data base for articles published in five major medical journals fr

BACKGROUND: Several studies have demonstrated that children with sensorineural hearing loss (SNHL) exhibit postural instabilities, as well as balance and gait disorders, due to the vestibular dysfunction that they are prone to display as a consequence of inner ear injury. Thus, some experiments have proposed vestibular rehabilitation exercises programs as a treatment to improve these motor skills in children with SNHL.
OBJECTIVE: Assess the evidence quality of the trials that used vestibular rehabilitation exercises programs to improve the postural control, balance and gait of children with S

In order to propose a more precise definition and explore how to reduce ethical losses in randomized controlled clinical trials (RCTs), we set out to identify trial participants who do not contribute to demonstrating that the treatment in the experimental arm is superior to that in the control arm. RCTs emerged mid-last century as the gold standard for assessing efficacy, becoming the cornerstone of the value of new therapies, yet their ethical grounds are a matter of debate. We introduce the concept of unnecessary participants in RCTs, the sum of non-informative participants and non-responder

BACKGROUND AND OBJECTIVES: Cholinesterase inhibitors (ChEIs) have cardiovascular effects in addition to their neurologic activity and might alter mortality. We wanted to know whether treatment with ChEIs modifies mortality in patients with dementia.
METHODS: We searched PubMed, Embase, Cochrane CENTRAL, ClinicalTrials.gov, and ICRTP, from their inception to November 2021, and screened bibliographies of reviews, guidelines, and included studies. We included randomized controlled trials (RCTs) and nonrandomized controlled studies at lower risk of bias comparing ChEI treatment with placebo or us

BACKGROUND: Systemic sclerosis (SSc) is a clinically complex and challenging disease, that leads to skin fibrosis. Its most frequent complication is interstitial lung disease (ILD), which leads to a worse prognosis. In this situation, cyclophosphamide is considered the gold standard for its treatment, despite the controversies regarding its efficacy and toxicity. However, studies using rituximab (RTX) have shown that this drug may be a promising therapeutic option.
OBJECTIVES: This paper objective was to analyze the scientific evidence on the RTX effects on SSc.
METHODS: A systematic review

BACKGROUND: Balance and gait disorders have been observed in children and adolescents with sensorineural hearing loss (SNHL), justified by vestibular dysfunctions that these children may present, due to the injury to the inner ear. Therefore, some investigations have suggested that the practice of sports or recreational activities can improve the balance and gait of this population.
OBJECTIVE: Assess the evidence quality from randomized or quasi-randomized controlled trials that used sports or recreational activities as an intervention to improve the balance and /or gait of children and/or ad

BACKGROUND: Spontaneous supratentorial intracerebral hemorrhage (ICH) is a critical condition with high morbidity and mortality rates warranting urgent surgical evacuation. This systematic review and meta-analysis compare the safety and efficacy of neuro-endoscopy (NE) versus traditional craniotomy (CR) for managing ICH.
METHODS: From inception until July 2024, a comprehensive literature search was undertaken on PubMed, Cochrane Central, ScienceDirect, and Clinicaltrials.gov. Risk ratios (RR) and weighted mean differences (WMD) were pooled for categorical and continuous outcomes under the ran

BACKGROUND: Despite its prevalent and impactful nature, dry mouth remains an underexposed and undertreated symptom in patients with a life-limiting condition or frailty. The main contributing factors are a lack of awareness and knowledge amongst both healthcare professionals and patients, and a scarcity of effective, evidence-based interventions. In the DRy mOuth Project (DROP), we address these factors by investigating both a non-pharmacological and a pharmacological intervention: a nurse-led patient education program and locally applied pilocarpine.
METHODS: This intervention-based research

Platform trials are adaptive randomised controlled trials that address multiple questions using a single protocol, which reduces the time taken to reach a meaningful trial endpoint. This mini review provides a description of how to conceive, design, and carry out a platform trial in urology, using experience gained in the STAMPEDE trial. PATIENT SUMMARY: Clinical trials to test how well a new drug or treatment works can take a long time before meaningful results can be assessed. Trials with a platform design can test multiple treatments using just one protocol and control, which reduces the ti
