[Primary bone tumors of the spine].

BACKGROUND: Primary bone tumors of the spine are rare causes of spinal symptoms and often present with nonspecific clinical findings. Reliable differentiation from degenerative or inflammatory conditions is usually only possible by imaging. Precise anatomical compartment allocation as well as the combination of magnetic resonance imaging (MRI) and computed tomography (CT) are essential for diagnosis. OBJECTIVE: The aim of this review is to provide an overview of primary benign and malignant bone tumors of the spine, focusing on their imaging characteristics and differential diagnostic feature…

Cases and Controversies in Spine Trauma.

Spinal fractures, spinal cord injuries, and nerve root injuries are commonly encountered entities for spine surgeon. While many cases have well-researched presentations, outcomes, and standards of care there are many patients for which the appropriate clinical management is less clear. Other patients present with injuries with clear treatment preferences that are impossible to implement based on comorbidities or preference. The authors present a few recent cases of spinal injury where multiple treatment pathways could be considered and individual patient-related factors changed the course of t…

Treatment Strategies in the Osteoporotic Spine.

This article reviews the appropriate assessment and management of osteoporotic compression fractures and discusses the implications of osteoporosis on initial patient evaluation, medical optimization for surgery, selection of instrumentation, and surgical technique. Adverse outcomes associated with osteoporosis are discussed. Failure to appropriately evaluate, optimize, and treat spine patients with osteoporotic bone can lead to disastrous complications. Weakened bone can lead to implant failure through cage subsidence and screw pullout, as well as, peri-implant fractures, failure of deformity…

Adenocarcinoma of the appendix with metastasis to the spine.

There are a few reported cases of bone metastasis from adenocarcinoma of the appendix and there is none from Africa. We report this rare case of a 30-year-old man who presented with low back pain following a trivial fall and who subsequently developed generalized peritonitis for which he underwent laparotomy. The histology of the appendix and the mesentery confirmed adenocarcinoma of the appendix with metastasis to the mesenteric lymph nodes while an MRI scan confirmed spine metastasis. Carcinoma of the appendix, should, therefore, be considered as a differential diagnosis in spine metastasis …

Neuromonitoring for spine surgery in children.

Neuromonitoring or electrophysiologic monitoring is now an essential component of pediatric spine surgery due to the high number of spinal deformity indications in asymptomatic patients, for whom any neurological complication would be disastrous. Technological advances have led to the development of compact monitors that allow surgeons themselves to monitor the motor evoked potentials (MEP) perioperatively. This shift happened because it was difficult to always have a neurophysiologist in the operating room. Unfortunately, this also means that multimodal monitoring (sensory, mixed, D-wave, ele…

Use of Steroids in Spine Surgery.

Steroids are commonly used in spine pathologies. A broad range of providers from different specialties such as primary care, emergency medicine, and spine surgeons use steroids. The indications and controversies of steroid use are discussed in this article. A literature review was conducted on the use of steroids in spine pathologies. Steroids have been successfully used in anterior cervical discectomy and fusion (ACDF) to prevent dysphagia, in spinal cord injuries to improve neurological function, in acute back and neck pain for pain control, and in spinal metastasis. Steroid injections have …

Primary Benign Neoplasms of the Spine.

Benign tumours comprise the majority of primary vertebral tumours, and these are often found incidentally on imaging. Nonetheless, accurate diagnosis of these benign lesions is crucial, in order to avoid misdiagnosis as more ominous malignant lesions or infection. Furthermore, some of these tumours, despite their benign nature, can have localised effects on the spine including neural compromise, or can be locally aggressive, thus necessitating active management. Haemangiomas and osteomas (enostosis) are the commonest benign tumours encountered. Others include osteoid osteoma, osteoblastoma, fi…

Typhoid spondylodiscitis of the cervical spine.

Spinal infection comprises pyogenic and non-pyogenic spondylodiscitis. This condition may manifest with non-specific clinical symptoms, elevated infective parameters and imaging findings that are difficult to distinguish. The cornerstone of a definitive diagnosis and subsequent successful treatment lies in tissue analysis through culture and histopathological studies. In this context, we present a case of Salmonella pyogenic spondylodiscitis affecting the C5/C6 vertebrae, complicated by Salmonella bacteraemia and characterised by mechanical neck pain that curtails daily activities and overall

Chondrosarcoma of the spine-a case report.

CASE: A 73-year-old male patient presented with a 3-month history of back pain. In bone scintigraphy and the FDG PET-CT scan (fluorodeoxyglucose positron-emission computed tomography), highly suspect uptake levels were found in TH12-L1. Accordingly, an osteodestructive process was found on MRI (magnetic resonance imaging). Following a successfully performed biopsy of TH12, histologic analysis of the bone material revealed a chondrosarcoma (G1; T4N2M0). Complete resection of the tumor was successfully performed, since chondrosarcoma are resistant to radiation and chemotherapy. CONCLUSION: As c

Looking at the spine in controlled Acromegaly.

Skeletal fragility with high risk of vertebral fractures (VFs) is an emerging complication of growth hormone (GH) hypersecretion. VFs often coexist with spine arthropathy and both clinical conditions negatively impact on quality of life of acromegalic subjects. Management of spine osteopathy and arthropathy in acromegaly could be challenging since both complications can persist or even progress after biochemical control of disease. This article analyzes the latest evidence about possible pathophysiological links between VFs and spine arthropathy in active and controlled acromegaly, as well as …

[Application of navigation in the fractured spine].

OBJECTIVE: Computer navigation is used in patients with spine fractures to optimize the accuracy of pedicle screws and thereby reduce intra- and postoperative complications, such as injuries to vessels, nerves and accompanying structures. In addition, the ideal screw length and diameter for each pedicle can be detected to ensure optimal stability. INDICATIONS: Intraoperative navigation is suitable for the treatment of spine fractures, which require dorsal stabilization or fusion. It is primarily used for dorsal procedures ranging from the cervical to lumbar/sacral spine. CONTRAINDICATIONS: C…

Torticollis in Connection with Spine Phenotype.

PURPOSE: Torticollis is not of uncommon occurrence in orthopaedic departments. Various theories and studies concerning the pathogenesis of the deformity have been suggested. We aimed to highlight and discuss the underlying cervical and spine malformation complex in correlation with torticollis via radiographic and tomographic analysis and its connection with a specific syndromic entity. METHODS: Torticollis has been recognised in six patients (2 boys and 4 girls with an age range of 14-18 years), in addition to a couple of parents manifested persistent backpain. A variable spine malformation …

Stress Injuries of the Spine in Sports.

Spine sports stress injuries account for a significant amount of time loss at play in athletes, particularly if left unrecognized and allowed to progress. Spondylolysis makes up most of these stress injuries. This article focuses on spondylolysis, bringing together discussion from the literature on its pathomechanics and the different imaging modalities used in its diagnosis. Radiologists should be aware of the limitations and more importantly the roles of different imaging modalities in guiding and dictating the management of spondylolysis. Other stress-related injuries in the spine are also …

[Symptomatology of Disorders of the Cervical Spine].

Although advances in neuroimaging have result in great improvement in the diagnosis of cervical spine and cord disorders, it is still more difficult to localize symptoms in the cervical spine and cord than in the brain. Here, I discuss the following relatively new aspects in cervical spine and cord symptomatology: I. Headache and facial pain; II. Vertigo and dizziness; III. Peculiar hand symptoms due to high cervical spine and cord disorders; IV. Symptoms in the C3-C4 level; V. Postoperative C5 palsy; VI. Peculiar weakness due to lower cervical spine diseases; VII. False localizing sensory sig…

Intracanalicular Osteochondroma in the Lumbar Spine.

Osteochondroma is a common benign bone tumor that is relatively rare in the spine. Here, we report two cases of symptomatic solitary osteochondroma of the lumbar spine. The first case was a 61-year-old man who presented with a 2-year history of right leg numbness. Imaging findings showed that the cause of the radiculopathy was osteochondroma of the right inferior articular process at L4. The tumor was removed en bloc, and the numbness resolved. The second case was a 62-year-old man with osteochondroma of the right superior articular process at L5 that caused pain and numbness in the right leg.…

Aortic Aneurysm Eroding into the Spine.

Aortic aneurysms are usually asymptomatic until catastrophic rupture occurs. Ruptured abdominal aortic aneurysms classically present with acute back pain, shock, and a pulsatile abdominal mass. The natural history of some aortic aneurysms also includes a stage of contained rupture. This occurs when extravasation of blood from the ruptured aneurysm is contained by surrounding tissues. Here, the authors report the case of a chronic contained abdominal aortic aneurysm rupture that resulted in erosion of the spine.
