Primary intraventricular oligodendroglioma (IVO) is a rare form of clear cell neoplasm and diagnosis is challenging because other clear cell neoplasms such as central neurocytoma must be ruled out. We report a large cell neuroendocrine carcinoma arising in the ampulla of Vater. The patient, a 74-year-old woman, presented with a 3-cm ulcerated mass located in the ampullary region. She died of disease 8 months after surgery. Microscopically, the tumor was extensively necrotic. It was composed of islands and trabeculae irregularly infiltrating the muscular wall of the duodenum. Here we present a 61-year-old woman with a history of renal cell carcinoma after nephrectomy and complete response to immunotherapy. Follow-up MRI revealed a contrast-enhanced lesion in the pancreatic tail, suspicious for a pancreatic neuroendocrine tumor. CASE: An 88-year-old woman presented with worsening right hip pain over 2 years. Radiographs were unremarkable, but magnetic resonance imaging identified a femoral neck lesion. A computed tomography-guided biopsy revealed a zellballen pattern of cells positive for chromogranin and synaptophysin. Further workup, including a metaiodobenzylguanidine scan confirmed focal uptake at the right femoral neck. Neuroendocrine tumor (NET) of the major duodenal papilla is a rare occurrence. However, that of the minor duodenal papilla is even rarer. To date, only a few cases have been reported.
