New technologies for the treatment of obstructive arterial disease.

The well-known limitations of balloon angioplasty include unpredictable abrupt closure, chronic total occlusion, diffuse disease, and restenosis, among other factors. These limitations have prompted the development of new technologic approaches to angioplasty including laser applications for plaque ablation, mechanical device applications for plaque removal/debridement, and stent devices for structural maintenance of vascular lumen patency. Devices which directly apply laser energy for ablation of plaque material include a balloon-centered laser angioplasty system, excimer laser ablation cathe

Long-term outcomes of percutaneous paravalvular regurgitation closure after transcatheter aortic valve replacement: a multicenter experience.

OBJECTIVES: This study sought to evaluate acute and long-term outcomes of percutaneous paravalvular regurgitation (PVR) closure after transcatheter aortic valve replacement (TAVR). BACKGROUND: Severe symptomatic PVR is a predictor of all-cause mortality after TAVR. The current use of devices for transcatheter closure of PVR has been adapted from other indications without known long-term outcomes. METHODS: The study population consisted of a series of cases pooled together from an international multicenter experience. Patients underwent transcatheter implantation of a closure device for the t

Transcatheter occlusion of patent ductus arteriosus using a newly developed self-expanding device. Evaluation in a canine model.

RATIONALE AND OBJECTIVES: A new occlusion device designed for use in a patent ductus arteriosus was tested in an animal model. METHODS: The device consists of a self-expanding conical-shaped stainless steel mesh, with hooks encircling the expanding end acting to secure the device within the ductus. A screw thread at the apex allows attachment to a stylet, and a 6 F delivery system is used. Placement of the device was attempted in 20 canines that previously had a vascular graft surgically inserted between the descending thoracic aorta and main pulmonary artery, creating the patent ductus model

Device closure of atrial septal defects with the Amplatzer septal occluder: safety and outcome in infants.

OBJECTIVE: Device closure of secundum atrial septal defects is sometimes needed in young children; however, little is known about the safety and outcome of this procedure in infants. In this study, the safety and efficacy of secundum atrial septal defect closure with the Amplatzer septal occluder (AGA Medical Corp, Golden Valley, Minn) was evaluated in patients less than 1 year of age. METHODS: Between July 1999 and September 2006, atrial septal defect closure with the Amplatzer septal occluder was attempted in 15 infants at our institution. The patients ranged in age from 0.5 to 11.9 months

A Computational Model of Restenosis under Deployment of Rapamycin-eluting Stents.

Stents are meshed tubular devices placed inside blood vessels to keep them open after angioplasty. Since bare-metal stents show an increased probability of re-narrowing (restenosis) within a few months after treatment, so-called drug-eluting stents (DESs) were proposed. These devices gradually emit an antiproliferative drug (e.g., Rapamycin), thereby reducing the tissue growth rate, lowering the restenosis risk. Nevertheless, due to the decreased cell growth, the pace of wound closure is decreased simultaneously, increasing the risk of thrombosis. In this work, we present a computational model

Ruptured Sinus of Valsalva Aneurysm: Transcatheter Closure Through Retrograde Approach.

The ruptured sinus of Valsalva aneurysm (RSOV), a rare but well-recognized clinical entity, is invariably a form of left-to-right shunt due to rupture into right-sided chambers. It causes profound hemodynamic effects, especially when the rupture is acute. Like most other left-to-right shunts, it was only a matter of time before this rare defect also became amenable to transcatheter closure (TCC). Since the first report of TCC of RSOV by Cullen et al. in 1994 using the Rashkind umbrella, in recent times, there has been a spate of case reports, brief communications, and interesting case presenta

Percutaneous Decannulation for Venoarterial Extracorporeal Membrane Oxygenation Using a Perclose ProGlide Closure Device and a Balloon Catheter Without On-Site Cardiac Surgical Backup.

Surgical decannulation for venoarterial extracorporeal membrane oxygenation (VA-ECMO) is recommended as a standard weaning strategy considering large-sized cannulas (14-22 French) are inserted in VA-ECMO. However, we should be aware of complications such as bleeding and infection when removing an arterial cannula, especially in facilities without on-site cardiac surgical backup. Percutaneous closure devices for femoral arterial access sites are currently approved for the decannulation of a 10-French or smaller sheath. We reported a case of successful weaning off from ECMO using a combination m

Atrial arrhythmias in patients beyond 40 Years of age post atrial septal defect device closure.

BACKGROUND: Patients >40 years with an atrial septal defect (ASD) may have atrial arrhythmia which warrants treatment. It is unclear whether preventive left atrial appendage (LAA) occlusion should be performed simultaneously during ASD closure in these patients. OBJECTIVE: This study aimed to examine the incidence of de novo atrial arrhythmia in patients >40 years old post transcatheter closure of ASD. METHODS: This was a single-center, retrospective study involving all consecutive patients who have undergone device closure of ASDs at age >40 years. Standard 12-lead ECGs were performed pre-p

The utility of 3D printed models in complex percutaneous paravalvular leak interventions.

Paravalvular leaks (PVL) are seen in 5-17% of patients after surgical mitral and aortic valve replacement. This is usually well-tolerated in the majority of patients; however, up to 5% will require re-intervention due to either hemodynamically significant regurgitation or hemolysis requiring repeated blood transfusion. Transcatheter closure of PVLs is becoming the treatment of choice in many patients owing to the high risk of redo surgery, high rates of recurrence with the surgical approach, and substantial improvements in device technology and growing expertise in structural heart disease int

First experience with the new generation Edwards Sapien 3 aortic bioprosthesis: procedural results and short term outcome.

BACKGROUND: TAVR has become an established treatment for severe symptomatic aortic stenosis in patients with high surgical risk. The latest generation of the balloon-expandable Edwards Sapien device, the Sapien 3, together with its new transfemoral Commander delivery system has been designed to reduce paravalvular regurgitation and vascular access site complications. OBJECTIVES: To evaluate procedural results and short term outcome with the third generation Sapien 3 device. METHODS: We retrospectively evaluated 125 consecutive TAVR patients and analyzed the first 51 patients in whom we impla

Endovascular closure of patent foramen ovale: a critical appraisal of published trials.

OBJECTIVE: The treatment of cryptogenic stroke patients with a patent foramen ovale (PFO) is controversial. A critical review of these studies is presented. METHODS: A description of all trials comparing medical and endovascular treatment with closing devices is given. Additional pertinent studies are discussed to help construct a rational basis for treatment decisions. RESULTS: Initial negative trials evaluating PFO closure were followed by positive studies published in 2017 and 2018. All trials evaluated young patients (up to 60 years). Methodological problems are present in all trials inc

Transcatheter Closure of a Residual Mid-Leaflet Mitral Valve Cleft With a Siege Vascular Plug.

Transcatheter mitral valve interventions have emerged as valuable alternatives to surgical repair in high-risk patients, including transcatheter closure of mitral valve perforations. However, there is little experience with percutaneous mitral valve interventions in pediatric patients or patients with congenital heart disease. We report a case of transcatheter closure of a residual mid-leaflet mitral valve cleft in a 19-year-old patient with moderate mitral regurgitation through two defects in the anterior leaflet of a repaired cleft mitral valve using a retrograde approach and a novel device,

Lost and found: an unusual late complication of the Angio-Seal closure device.

We report an unusual case of breakage and embolisation of a femoral access closure device (Angio-Seal), which caused acute leg ischemia after percutaneous coronary intervention in a diabetic patient with documented peripheral vascular disease. Embolectomy from the superficial femoral artery revealed the polymer anchor split in two parts and embedded within a large (25x5 mm) thrombus.

Paravalvular leak closure for persisting aortic regurgitation after implantation of the CoreValve transcatheter valve.

Significant aortic regurgitation after TAVI results in lack of symptomatic and prognostic benefit from the procedure and generally requires intervention. While most of the regurgitations can be successfully targeted with standard techniques, occasional patients have restrictive calcification resistant to post-dilatation and significant regurgitation persists. We present a case of refractory aortic regurgitation successfully treated with percutaneous paravalvular leak closure. An 81-year-old man with symptomatic severe aortic stenosis underwent a transfemoral CoreValve TAVI in December 2009. He

Ultrasound-guided closure of the femoral artery during venoarterial decannulation using a large-bore closure device.

BACKGROUND: Peripheral femoro-femoral venoarterial extracorporeal membrane oxygenation (VA-ECMO) is viable for fast hemodynamic assistance during cardiogenic shock. Ultrasound-guided closure with a large-bore device (MANTA®) is a feasible option potentially replacing surgical arteriotomy closure in peripheral VA-ECMO decannulation. METHODS: This retrospective study included patients weaning from percutaneously inserted femoro-femoral VA-ECMO at the Helsinki University Hospital, Finland in 2012-2020. The primary endpoints were access-site complications, a composite of hematomas/seromas/surgica

The role of sartorius muscle flaps in the management of complex groin wounds.

A best evidence topic in vascular surgery was written according to a structured protocol. The question addressed was whether sartorius muscle flaps (SMF) can be effectively used in the setting of complex groin wounds with exposed prosthetic grafts for graft salvage and limb salvage. The literature review identified 33 articles reporting on the use of SMF for complex vascular wounds. Of these, 7 articles reporting on the use of 539 SMFs were considered the best evidence to answer the clinical question. Indications included surgical site infections with or without wound dehiscence, lymph leaks,

A study of bailout plug-based closure after failed suture-based closure in patients undergoing transfemoral TAVI.

BACKGROUND: Percutaneous suture-based arterial access site closure (ProGlide) is commonly applied in patients undergoing transfemoral transcatheter aortic valve implantation (TAVI). However, the failure of a suture-based vascular closure device (VCD) may require additional treatment. AIMS: We aimed to evaluate the efficacy and safety of bailout access site closure using a large-bore plug-based device (MANTA) in patients with failed suture-based closure during transfemoral TAVI. METHODS: Patients undergoing a bailout attempt with the MANTA VCD were identified from a prospectively enrolling, i
