Office-Based Ureteral Stenting Using a Single-Use Flexible Cystoscope Under Local Anesthesia: A Two-Center Prospective Study on Feasibility and Patient Experience.

INTRODUCTION: To evaluate the feasibility, safety, and patient satisfaction of Double-J (DJ) ureteral stenting using a flexible cystoscope under local anesthesia (LA). MATERIALS AND METHODS: We analyzed prospectively collected data from all patients who underwent DJ stent insertion or replacement using flexible single-use cystoscope under LA between February 2022 and September 2024 at two tertiary referral centers. Failure was defined as the inability to effectively complete the scheduled stent insertion or replacement. Pain was assessed using the Visual Analog Scale, whereas overall patient

[Influence of phenazopyridine on the well-being of patients during and after cystoscopy].

INTRODUCTION: Cystoscopy is one of the most common procedures in urology. There is no single approach to pain relief. In the literature, there are conflicting data on the efficiency of intra-urethral gels. The use of non-steroidal anti-inflammatory drugs, intravenous sedation, and nitric oxide analgesia has been described. Phenazopyridine has been known for a long time. Acting on the bladder mucosa, it has a local analgesic effect. AN evaluation of phenazopyridine intake prior to cystoscopy in order to decrease pain during procedure and facilitate subsequent urination was performed. MATERIALS

Evaluation of the anti-conflict, reinforcing, and sedative effects of YT-III-31, a ligand functionally selective for α3 subunit-containing GABAA receptors.

BACKGROUND: In recent years, pharmacological strategies have implicated α3 subunit-containing GABAA (α3GABAA) receptor subtypes in the anxiety-reducing effects of benzodiazepines, whereas transgenic mouse approaches have implicated α2 or α5 subunit-containing GABAA receptors. AIMS: We investigated the role of α3GABAA subtypes in benzodiazepine-induced behaviors by evaluating the anti-conflict, reinforcing, and sedative-motor effects of the novel compound YT-III-31, which has functional selectivity for α3GABAA receptors. METHODS: Female and male rhesus monkeys were trained under a conflict pr

Factors associated with periesophageal vagal nerve injury after pulmonary vein antrum isolation.

BACKGROUND: Periesophageal vagal nerve injury is recognized as a rare complication in atrial fibrillation ablation procedures. We investigated the factors associated with the occurrence of symptomatic periesophageal vagal nerve injury after pulmonary vein antrum isolation. METHODS AND RESULTS: Overall, 535 consecutive patients who underwent sole pulmonary vein antrum isolation were included. Point-by-point radiofrequency applications were applied using irrigated-tip catheters under minimal sedation without esophageal temperature monitoring. In the initial 165 patients, the ablation settings f

Initial Experience with Renal Denervation for the Treatment of Resistant Hypertension - The Utility of Novel Anesthetics and Metaiodobenzylguanidine Scintigraphy (MIBG).

BACKGROUND: The Symplicity-HTN 3 trial failed to show significant difference in blood pressure (BP) lowering between patients undergoing catheter-based renal denervation (RDN) and the sham-procedure arm of the study. However, there is still optimism about the role of RDN in the treatment of resistant hypertension, because identification of patients with increased sympathetic activity thus being good RDN responders, improvements in the RDN procedure and new technology RDN catheters are all expected to lead to better RDN results. We present our initial experience with RDN for the treatment of re

Self-expanding metal stents for colonic obstruction: experiences from 104 procedures in a single center.

PURPOSE: In the past, colonic obstruction caused by malignancy most often resulted in high-risk operations, usually involving two-step procedures or leaving the patient with a stoma in case of disseminated disease. METHODS: Between May 1997 and January 2003, 104 procedures with self-expanding metal stents have been performed in 96 patients at our institution. The goals of the procedure were either postponement of emergency operation or definitive palliative treatment. Surgeons with combined endoscopic and fluoroscopic technique performed all procedures. In most cases no analgesia or only slig

Accuracy and interobserver agreement of small-caliber vs. conventional esophagogastroduodenoscopy for evaluating esophageal varices.

BACKGROUND AND STUDY AIMS: Advances in endoscope design have allowed high-quality imaging using small-caliber endoscopes (< 6 mm), and these have been proposed as providing an accurate modality for evaluating esophageal varices in several small studies. We aimed to evaluate the accuracy and interobserver agreement of small-caliber esophagogastroduodenoscopy (EGD) compared with conventional EGD for evaluating esophageal varices in a large prospective cohort. PATIENTS AND METHODS: A total of 115 patients with end-stage liver disease and/or portal hypertension were prospectively enrolled into th

Breaking the Taboo: Four Cases of Thoracic Segmental Spinal Anesthesia in Modern Surgical Practice.

Thoracic segmental spinal anesthesia (TSSA) is an old neuraxial technique that has recently regained attention as a possible alternative to general anesthesia (GA) in selected surgical patients. Its segmental nature, low-dose local anesthetic requirement, and ability to preserve spontaneous ventilation may be particularly relevant in high-risk patients in whom airway manipulation, endotracheal intubation, positive-pressure ventilation, and systemic anesthetic exposure could represent an additional physiological burden. We report four selected high-risk patients undergoing procedures from diffe
