Transcatheter closure of atrial septal defect (ASD) has become an accepted alternative to surgery. A number of complications associated with ASD device closure have been recognized but most are rare or minor in severity. We report a rare complication of atrial septal laceration during transcatheter closure of secundum ASD. We discuss the diagnostic confusion, which resulted in the decision for surgical correction.

Benign lipomatous lesion of the heart includes an heterogeneous group of entities including neoplastic, congenital and reparative phenomena. Among these lipomas and lipomatous hypertrophy of the atrial septum ( LHIS) represent the most common lesion. Patients suffering from LHIS are often asymptomatic, however atrial fibrillation, congestive heart failure and supraventricular tachycardia are typical findings. Here we present a rare case of LHIS symptomatic for asthenia and dyspnea.

Double inter-atrial septum is an exceedingly rare congenital cardiac abnormality. We describe a case of transient ischaemic attack in a 53 year-old female found to have double inter-atrial septa on transthoracic and transoesophageal echocardiography. The midline inter-atrial chamber enclosed by the two septa was found to be continuous with the left atrium, with stasis in this accessory chamber predisposing to thrombus formation and cardio-embolic events. The case highlights the importance of transoesophageal echocardiography in the investigation of stroke, particularly in younger individuals.

Both balloon atrial septostomy and static balloon dilation are often ineffective in creating a large atrial septal defect in patients with a thickened atrial septum. In such situations, blade atrial septostomy and atrial septal stenting are alternative treatments. We have devised and performed a new technique, called wire atrial septostomy, which uses a thin soft wire to create a large defect. The details of the procedure are presented.

Intact atrial septum (IAS), occurring in ∼10% of patients with hypoplastic left heart syndrome (HLHS), conveys significant neonatal morbidity and mortality. Perinatal interventions have been described, but outcomes remain poor. We present a fetus with HLHS with IAS who underwent immediate novel postnatal atrial appendage anastomosis, thus achieving rapid left atrial decompression. (Level of Difficulty: Advanced.).

OBJECTIVES: Hypoplastic left heart syndrome (HLHS) with an intact atrial septum (IAS) is a rare finding, reported in only 1% of pathologic specimens with hypoplasia of the aortic tract complex. In newborns with left heart obstruction, the existence of an interatrial communication is very important for oxygenated blood to be distributed to the body and to prevent pulmonary congestion. We describe the echocardiographic features of 4 new cases of a rare atrial septal malformation consisting a double atrial septum with a midline chamber between the left and right atrium. Half of the cases had major left-sided obstructive lesions. Transthoracic, transesophageal, intracardiac, and fetal echocardiographic features of this anomaly are demonstrated. Previous descriptions, embryologic speculations, and clinical considerations are discussed.
