[Ectopic parathyroid glands in primary hyperparathyroidism].

The incidence of the ectopic parathyroid glands seems to be different between normal subjects and patients affected by primary hyperparathyroidism. We reviewed a serie of patients operated on for primary hyperparathyroidism in our Institute between 1985 and 1991, and the reports from literature about this mean; the frequence of ectopies is = 31% (10.3% in the upper- anterior mediastinum). These findings are similar to those reported in the literature. The higher incidence of ectopies in the hyperparathyroid group respect to the normal one and the surgical problems that it could cause represent…

Parathyroid glands: combination of (99m)Tc MIBI scintigraphy and US for demonstration of parathyroid glands and nodules.

PURPOSE: To determine the appropriate choice of imaging techniques for localization of nodular lesions of parathyroid glands. MATERIALS AND METHODS: First, computed tomographic (CT), magnetic resonance (MR), ultrasonographic (US), and technetium 99m methoxyisobutyl-isonitrile (MIBI) scintigraphic images in 49 patients with primary hyperparathyroidism were retrospectively evaluated. A single-blind, prospective study that included 16 patients with primary hyperparathyroidism was then conducted. MR, US, scintigraphic, and color Doppler US images of the neck were obtained and analyzed. RESULTS: …

Blood supply and parathyroid hormone secretion in pathological parathyroid glands.

The blood supply of pathologic parathyroid glands and the relation between parathyroid hormone secretion and parathyroid blood perfusion was studied during surgery for hyperparathyroidism. Blood flow in 39 single adenomas and 20 glands classified as primary or secondary hyperplasia were studied intraoperatively with laser Doppler flowmetry. The ipsilateral inferior thyroid artery was occluded during continuous flowmetry recording, which resulted in a 40% reduction of parathyroid blood flow in both groups. In 12 patients with single adenomas, intact parathormone (iPTH) was measured intraoperati…

Nitric Oxide Synthase in Human Parathyroid Glands and Parathyroid Adenomas.

Nitric oxide (NO) is a novel gaseous intercellular transmitter thought to play important physiological roles in the regulation of blood flow and hormone secretion in, for example, the pituitary, the thyroid, and the endocrine pancreas. Whether nitric oxide synthase (NOS) is present in the human parathyroid glands has not yet been demonstrated. In the present study, histologically normal, but functionally suppressed human parathyroid glands and parathyroid adenomas from patients with primary hyperparathyroidism were investigated by immunocytochemistry with antibodies against neuronal NOS and by…

Ultrasound Identification of Normal Parathyroid Glands.

Ultrasound has become indispensable for identification of thyroid and parathyroid pathology, but normal parathyroid glands have historically been considered too subtle to accurately detect. Inability to identify and protect parathyroid glands can result in hypoparathyroidism and hypocalcemia during thyroidectomy surgery as well as misinterpretation of central neck structures in the postoperative neck. Advances in ultrasound resolution have opened the door to novel applications for this technology. In this study, we report the first surgeon-performed ultrasound identification of normal parathyr…

Vitamin D receptors and parathyroid glands.

OBJECTIVE: To describe the function and metabolism of the vitamin D hormone and the role of the vitamin D receptor and the calcium-sensing r … he last several years, great interest has been generated about the interaction of vitamin D and the parathyroid glands, gastrointestinal tract, kidney, and bone in relation to calcium and parathyroid hormone levels. Vitamin D has an important role in calcium and parathyroid hormone metabolism. Likewise, the vitamin D axis appears to be involved with the development …

Radionuclide imaging of the parathyroid glands.

The parathyroid glands, which usually are situated behind the thyroid gland, secrete parathyroid hormone, or PTH, which helps maintain calcium homeostasis. Primary hyperparathyroidism results from excess parathyroid hormone secretion. In secondary hyperparathyroidism, the normal PTH effect on bone calcium release is lost. Serum PTH rises, causing generalized hyperplasia. In tertiary hyperparathyroidism, a complication of secondary hyperparathyroidism, normal feedback mechanisms governing PTH secretion are lost, parathyroid gland sensitivity to PTH decreases, and the threshold for inhibiting PT…

Bilateral oblique approach to parathyroid glands.

OBJECTIVE: To propose a simple and minimally invasive approach for parathyroid surgery. SUMMARY BACKGROUND DATA: Minimally invasive approaches to the parathyroid glands may involve preoperative morphologic explorations, perioperative biologic controls, or videocervicoscopy, a new method. METHODS: The authors describe 597 patients who underwent parathyroidectomy through an original bilateral oblique approach between 1976 and 1997. None underwent morphologic exploration or biologic perioperative monitoring. In primary hyperparathyroidism, the four glands are controlled and it is possible to ch…

Parathyroid glands in chronic aluminum intoxication.

In chronic renal failure, aluminum overload may influence parathyroid function. In a study of possible aluminum-induced parathyroid abnormalities, parathyroid glands from nine parathyroidectomized patients on hemodialysis were examined by light and electron microscopy and by X-ray microanalysis. Aluminum overload was assessed by the presence of stainable aluminum (aluminum surface, 23.3% +/- 11% of total surface) in bone biopsy specimens. The mean plasma aluminum concentration was 7.7 +/- 1.9 mumol/L. All patients but one had elevated plasma concentrations of immunoreactive parathyroid hormone…

Syncytial cells in canine parathyroid glands.

Multinucleated syncytial cells consisting of clusters of closely apposed, darkly staining nuclei and densely eosinophilic cytoplasm were present in parathyroid glands from 25 of 36 dogs (69%). They were most often located either peripherally in the parathyroid gland or adjacent to blood vessels. The numbers of nuclei per syncytial cell ranged from two to greater than 30. Syncytial cells had degenerative changes as suggested by vacuolated mitochondria, dilated rough endoplasmic reticulum, irregular nuclear outlines, and condensed nuclear chromatin. These ultrastructural changes contrasted sharp…

Surgical anatomy of human parathyroid glands.

In an autopsy study of 503 cases the parathyroid glands were dissected, and the number of glands in each case and the anatomic distribution of the glands were recorded. In 18 cases (3%) only three glands were found. In these cases the lower combined weight suggested that a fourth gland had been missed. In 421 cases (84%) there were four glands and in 64 cases (13%) there were supernumerary glands. Most often the supernumerary gland was a fifth gland, usually in the thymus. The anatomic distribution of the glands showed considerable constancy. The positions of the glands on the one side were sy…

Hypotensive actions of snake parathyroid glands.

Parathyroid gland extracts from four different snake species, Ptyas korros, Ptyas mucosa, Elaphe radiata, and Naja naja, were prepared and their hypotensive actions were assayed in anesthetized P. korros. All extracts were hypotensive and the responses, except that of N. naja, were dose related. Their potencies were, however, different in different species. These data suggest that the hypotensive property of the parathyroid hormone is probably widely distributed in submammalian vertebrates.
