Best Practices in Patient Safety and Communication.

Emergency medicine is a high-risk area of medical practice, with a high rate of preventable adverse events. This is multifactorial, hinging on the myriad system and processes issues that complicate emergency care. Strong teamwork and communication have been identified as critical components for safe care in emergency medicine. Health care professionals and leaders within emergency medicine can implement solutions aimed at cultivating a strong safety culture, creating processes and system-based approaches to improve patient safety. This article provides an overview of the evidence-based approac…

Patient safety culture in Norwegian nursing homes.

BACKGROUND: Patient safety culture concerns leader and staff interaction, attitudes, routines, awareness and practices that impinge on the risk of patient-adverse events. Due to their complex multiple diseases, nursing home patients are at particularly high risk of adverse events. Studies have found an association between patient safety culture and the risk of adverse events. This study aimed to investigate safety attitudes among healthcare providers in Norwegian nursing homes, using the Safety Attitudes Questionnaire - Ambulatory Version (SAQ-AV). We studied whether variations in safety attit…

Evidence-based patient safety advisory: malignant hyperthermia.

As more and more routine plastic surgery procedures move from the hospital to outpatient surgery facilities, plastic surgeons must be aware of the risk factors for life-threatening events that might occur in this setting. This awareness includes recognition of the signs and symptoms and the management of a rare but life-threatening condition, malignant hyperthermia. This article reviews the current understanding of the concepts pertinent to malignant hyperthermia diagnosis and treatment in the outpatient setting and current standards and recommendations for physicians and support personnel reg

A safety support system for patient location.

We have developed a new electronic location support system for wandering elderly patients who may be walking, be stationary, or moving at higher speed in vehicles. Our system functions in buildings, and uses a low power mobile phone terminal along with a caregiver's mobile phone. The system is carried by the patient whose location is detected from the location of the system antennae communicating with the person's mobile phone system (PHS). The caregiver sets the patient's movement area, with specialized computer software installed on the mobile phones. When the patient leaves this preset area

Examining markers of safety in homecare using the international classification for patient safety.

BACKGROUND: Homecare is a growth enterprise. The nature of the care provided in the home is growing in complexity. This growth has necessitated both examination and generation of evidence around patient safety in homecare. The purpose of this paper is to examine the findings of a recent scoping review of the homecare literature 2004-2011 using the World Health Organization International Classification for Patient Safety (ICPS), which was developed for use across all care settings, and discuss the utility of the ICPS in the home setting. The scoping review focused on Chronic Obstructive Pulmona…

Investigating antibiotics in the NICU and patient safety.

In recent years, a number of observational studies have been published in neonatology-focused journals that modify American Academy of Pediatrics (AAP) evidence-based guidelines, as outlined in the Red Book and other publications authored by the AAP Committee on the Fetus and Newborn and the AAP Committee on Infectious Diseases, for the treatment of neonatal infectious diseases. Specifically, studies that decrease the treatment length or advocate for the early transition of intravenous to oral antibiotics in all neonates with suspected or culture-positive sepsis or pneumonia should be regarded

Safety outcomes and patient convenience in outpatient parotidectomy.

BACKGROUND: Parotidectomies have historically been performed on an inpatient basis despite being well-tolerated surgeries with minimal postoperative wound care and low rates of complications at high-volume institutions. Past studies have supported the safety of outpatient surgery for parotidectomy but have been limited to superficial parotidectomy and have not addressed the patient experience surrounding the surgical intervention such as pre-operative and post-operative care and communication. PURPOSE: This study assesses the impact of outpatient superficial, deep, and partial parotid surgery
