Adult rumination syndrome: Differentiation from psychogenic intractable vomiting.

Rumination syndrome is known to exist in infants and mentally retarded adults since long time. In past few years, some reports appeared that showed its existence in adult patients also. It is frequently confused with the intractable vomiting in adults and misdiagnosis leads to delay in appropriate management. We are here describing the case of a female patient with rumination syndrome where specific points in the history delineated the presence of this illness and helped in appropriate management. The patient became symptom free soon after the diagnosis was reached.

Rumination syndrome: when to suspect and how to treat.

PURPOSE OF REVIEW: Rumination syndrome is a gastrointestinal disorder characterized by effortless regurgitation of recently ingested food. The disorder is rare, but likely under-recognized and leads to impaired quality of life among those affected. This review discusses recent studies which examined the pathophysiology, diagnoses and therapy of rumination syndrome. RECENT FINDINGS: The pathogenesis of rumination syndrome remains incompletely understood. Therapeutic options, which appear effective, include behavioral therapy with diaphragmatic breathing and pharmacotherapy with baclofen. A ran…

Rumination syndrome: a review of current concepts and treatments.

Rumination is a normal and common phenomenon among ruminant animals; but in humans, it is always regarded as symptom indicative of abnormal function of the upper gastrointestinal tract, and understanding of the mechanisms explaining this event are still evolving. Learning-based theories, organic factors such as gastroesophageal reflux disease and psychological disturbances (eg, depression, anxiety) and the role of life stresses have been postulated as potential mechanisms of rumination. In this review, we take the approach that rumination syndrome is a distinct and discrete functional gastrodu…

The rumination syndrome in adults. A characteristic manometric pattern.

The rumination syndrome is defined as a process in which a person chews regurgitated gastric contents and then either partially ejects or swallows them. We report 12 cases of rumination in which the clinical diagnosis was supported by esophageal and gastrointestinal motility studies. These patients showed a characteristic pressure spike-wave pattern that was associated with regurgitation and was recorded simultaneously at all manometric sites. These spike waves increased significantly in frequency (p less than 0.001) and amplitude (p less than 0.04) during the postprandial period. The underlyi…

[Recognition and Management of the Patient with Rumination Syndrome].

Rumination syndrome is a functional gastroduodenal disorder characterized by the repetitive, effortless regurgitation of recently ingested food into the mouth followed by rechewing and reswallowing or expulsion of the food bolus. In clinical practice, most patients report the presence of "reflux" or "vomiting" and, if a detailed history is not taken with directed questions, the symptoms are often misinterpreted; which leads to multiple complementary tests and unnecessary treatments that do not solve the problem. In patients with a clinical diagnosis of refractory gastroesophageal reflux diseas…

Rumination Syndrome in Children and Adolescents: A Mini Review.

Introduction: Rumination syndrome involves recurrent regurgitation of food and is believed to be underdiagnosed with patients experiencing long delays in diagnosis. It can be associated with significant social consequences, high rates of school absenteeism, and medical complications such as weight loss. The primary aims of the current review are to assess the literature regarding prevalence, pathophysiology, and treatment outcomes with a focus on neurotypical children and adolescents. Results: Population studies in children/adolescents, 5 years of age or older, range from 0 to 5.1%. There are …

Diagnosis and Treatment of Rumination Syndrome: A Critical Review.

Rumination syndrome (RS) is characterized by the repeated regurgitation of material during or soon after eating with the subsequent rechewing, reswallowing, or spitting out of the regurgitated material. Rumination syndrome is classified as both a "Functional Gastroduodenal Disorder" (by the Rome Foundation's Functional Gastrointestinal Disorders: Disorders of Gut-Brain Interaction, 4th edition) and a "Feeding and Eating Disorder" (by the Diagnostic and Statistical Manual of Mental Disorders, 5th edition). Rumination syndrome is a disorder that is often inaccurately diagnosed or missed, resulti…

The rumination syndrome: clinical features rather than manometric diagnosis.

BACKGROUND/AIMS: Rumination is infrequent in adults of normal mental capacity. Upper gastrointestinal manometry reportedly confirms the diag … ge, 35 months), 12 reported subjective improvement. In 14, the behavior persists. CONCLUSIONS: The rumination syndrome is underdiagnosed. With typical clinical features, gastroduodenal manometry seems unnecessary to confirm the diagnosis. Diagnosis and reassurance are important in management to avoid unnecessary tests and treatments.

Rumination syndrome and eosinophilic esophagitis in children: Defining the relationship.

OBJECTIVES: While rumination syndrome (RS) and eosinophilic esophagitis (EoE) can share overlapping symptoms, their association remains unclear. Our objective was to explore the relationship between RS and EoE and understand the clinical course of children with both conditions. METHODS: We conducted a retrospective cohort study of children with RS and EoE evaluated at our institution from 2016 to 2023. Patient characteristics, symptoms, treatments, and outcomes were compared. RESULTS: Among 230 children with RS, 22 (10%) had both RS and EoE (77% male, median age 18 years). Sixteen (70%) were…

Rumination syndrome in children and adolescents: diagnosis, treatment, and prognosis.

OBJECTIVES: To characterize the clinical features, results of diagnostic testing, and treatment outcomes for children and adolescents with rumination syndrome. METHODS: Review of the medical records for all 147 patients ages 5 to 20 diagnosed with rumination syndrome at our institution between 1975 and 2000. Data are presented as mean +/- the standard error of the mean. RESULTS: Sixty-eight percent were female. Age at diagnosis was 15.0 +/- 0.3 years. Symptom duration before diagnosis was 2.2 +/- 0.3 years, 73% missed school/work, and 46% had been hospitalized because of symptoms. Before dia…

Schistosomus reflexus syndrome: a heritable defect in ruminants.

Schistosomus reflexus (SR) is a rare and fatal congenital disorder. Primarily observed in ruminants, its defining features include spinal inversion, exposure of the abdominal viscera because of a fissure of the ventral abdominal wall, limb ankylosis, positioning of the limbs adjacent to the skull and, lung and diaphragm hypoplasia. Variable components of SR include scoliosis, cleft sternum, exposure of thoracic viscera, and abnormalities of the digestive and urogenital systems. This report presents the findings from an anatomical analysis of a female Holstein SR calf with thoracoschisis, scoli

Pituitary Abscess Syndrome in Ruminants: Nine Cases.

Pituitary abscess syndrome (PAS) is a rare neurological disorder, typically associated with progressive dysfunction of the cerebrum and brainstem. Reporting PAS in ruminants is essential to broaden the global veterinary understanding of this condition. The present study describes the epidemiological, clinical, laboratory, microbiological, and pathological findings of nine ruminant cases diagnosed with PAS. Neurological signs were the most prominent clinical manifestations and included altered mentation, tongue hypotonia, nystagmus, blindness, ear ptosis, circling, facial hypoalgesia, head pres
