Among 1,972 patients operated upon because of lumbar disc herniation in a 7-year period (1971-1978), cauda equina syndrome was diagnosed in 26 patients. 22 patients had follow-ups 4-72 months postoperatively (average 37 months). At follow-up half of the patients had histories of normal micturition and only 6 patients made serious micturition complaints. One third were free from lumbar or sciatic pain, two thirds complained of distressing pain and neurological symptoms. The urodynamic investigation revealed complete bladder emptying in all but 1 patient. In half of the patients normal detrusor …

Patients with advanced HIV/AIDS are at high risk for Primary central nervous system lymphoma (PCNSL) due to impaired immune surveillance and … -associated systemic lymphoma with secondary central nervous system (CNS) involvement presenting as cauda equina syndrome - a neurosurgical emergency with potential for irreversible paralysis. This case underscores the importance of early, comprehensive spinal imaging in HIV-positive patients with new-onset myelopathy or cauda equina symptoms.

Cauda equina syndrome (CES) is one of the emergency conditions that can lead to devastating permanent functional disabilities, if misdiagnosed. Multiple studies have questioned the reliability of clinical assessment in diagnosing CES, whether some of the features should be considered to be potential red flags. Bladder dysfunction can reflect CE compromise. The post-void residual (PVR) volume bladder scan is useful in CES diagnosis, but to date there has been no single systematic review supporting its use. Furthermore, there is no clear cut-off point to consider PVR statistically significant. T…

Cauda equina syndrome seems to be a very rare complication of spinal manipulations. Only few cases, in fact, were referred in literature in the past decades. Most of them are very old and poorly documented evoking doubts about the pathogenetic relationship between the spinal maneuvers and the onset of the syndrome. We observed and treated a 42-year-old patient who complained a rapid onset of saddle hypoparesthesia and urine retention only a few hours after the spinal manipulation performed for L5-S1 herniated disc. The comparison of the two following MRIs performed before and after the manipul…

Posterior épidural migration of a lumbar disc herniation is rare. Clinical symptoms frequently indicate severe radicular compression. Cauda equina syndrome is very unusual. Three cases of cauda equina syndrome from posterior epidural migration of a lumbar disc herniation along with a review of the literature are presented to illustrate the Imaging features of this entity and related diagnostic difficulties. Finaly, early surgical management is the treatment of choice for optimal symptomatic recovery.

The leukemias may cause neurologic dysfunction through either direct invasion of the nervous system or indirectly through cytopenias, or it may occur as a result of the necessarily vigorous treatment programs for leukemia. We report here a 24-year-old acute lymphoblastic leukemia patient who in her second cycle of hyper-CVAD chem therapy regimen (high-dose Ara-C and high-dose methotrexate) received intrathecal methotrexate and two days afterwards was diagnosed as having Cauda Equina syndrome (CES). A lumbo-sacral MRI imaging with gadolinium was performed and there was a remarkable enhancement …

The role of urgent surgery in improving the outcome of cauda equina compression following lumbar central disc prolapse remains controversial … urgery may even add to rather than alleviate morbidity. This article reviews the pathophysiology of cauda equina syndrome, its definition, and the controversies surrounding management. Where urinary retention with overflow incontinence extists at presentation we believe that urgent decompression confers no benefit.

Three dogs were evaluated in our study using magnetic resonance imaging (MRI) to reveal the anatomical deformity and the degree of the lesion of cauda equina. In all dogs, MRI revealed soft tissue, such as cauda equina, epidural fat, and intervertebral disc, at the lumbosacral region clearly without contrast medium. Our results suggest that MRI has some advantages in evaluating cauda equina syndrome in dogs.

The cauda equina syndrome (CES) is an infrequently recognised and poorly understood complication of ankylosing spondylitis (AS). We report a case of CES with enlarged caudal sac and multiple posterior arachnoid diverticula eroding the laminae and spinous processes of the lumbosacral vertebrae in a patient with long-standing AS. The diagnosis was established using computerised tomography (CT) and magnetic resonance imaging (MRI).

The authors report a patient who was on oral anticoagulants because of mitral valve disease and who developed paraplegia from subarachnoid bleeding involving the cauda equina. The differential diagnosis, investigations and treatment of the cauda equina syndrome are described.

Redundant nerve roots of the cauda equina (RNR) are considered to be extremely rare. This finding was reported for the first time in 1953 by Verbiest and since then several articles have being published on this subject. In an extensive review of the literature we have found 60 cases of RNR, 54 cases of which were operated including two new cases that we report in the present article. The RNR consist of elongated, tortuous and serpiginous nerve roots of the cauda equina. This disorder mainly affects males and the clinical history ranges from few months to three decades. The illness often start

Combined spinal anesthesia and postoperative epidural analgesia is widely used in orthopedic surgery. Uncommon but serious neurologic complications of neuraxial anesthesia (NA) include direct trauma during needle or catheter insertion, central nervous system infections, and neurotoxicity of local anesthetics. Cauda equina syndrome (CES) is a rare complication after NA but can result in severe neurologic deterioration that may require surgical intervention. We present a case of a 69-year-old woman with postpolio syndrome who developed CES after combined spinal anesthesia and postoperative epidu…

PURPOSE: Sexual dysfunction is a recognised but underreported outcome following cauda equina syndrome (CES), with existing evidence fragmented and methodologically heterogeneous. Its true prevalence and clinical significance remain unclear. To systematically evaluate the prevalence, characteristics, and predictors of long-term sexual dysfunction following CES.
METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Studies reporting sexual function outcomes following CES were identified from database inception to 16 December 2025. Eligible studies included adult patie…

Giant lumbar disc herniations can produce severe sciatica and neurological deficits, in both acute and chronic stages, potentially resulting in symptoms of cauda equina syndrome, depending on their specific location.

This case report aims to discuss the prompt recognition and the possible mechanism of acute cauda equina syndrome (CES) following intrathecal bupivacaine administration for cesarean section. A 30-year-old primigravida developed sudden-onset bilateral lower-extremity weakness, numbness, and paresthesia, along with urinary and bowel incontinence, after an uncomplicated cesarean delivery under spinal anesthesia. Magnetic resonance imaging revealed engorgement and gadolinium enhancement of the cauda equina nerve roots, consistent with a diagnosis of CES. The patient was immediately started on high…

Background: Cauda equina syndrome (CES) is a rare but serious neurosurgical emergency. Back pain and sciatica are common presentations to A&E. Get It Right First Time (GIRFT) published guidance in February 2023 regarding the red flag symptoms and symptom duration requiring emergency MRI. This framework has been utilised by the orthopaedic service within our hospital. The aim of this study was to assess whether implementing GIRFT guidance could reduce CES related admissions without impacting the diagnostic yield for identifying this emergency. Methods: Retrospective review of all emergency MRI s…

Cauda equina syndrome (CES) results from compression of the cauda equina (CE) nerve roots and presents with a spectrum of neurological deficits. We report four cases of CES with symptom durations ranging from 3 days to 1 month at presentation. The clinical features included urinary incontinence, reduced perianal sensation, lower limb weakness, and erectile dysfunction in select cases. Despite delays in surgical intervention for some patients, all demonstrated significant postoperative improvement in bladder function and lower limb strength. Bladder sensation began to recover within 5-20 days p…
