A meta-analysis of comparisons of various surgical treatments for moyamoya diseases.

PURPOSE: Ischemia is one of the most familiar complications in the different procedures for moyamoya disease (MMD), but the optimal surgical approaches for MMD remain unknown. We aimed to evaluate the efficiency of various surgical treatments. METHODS: A literature search word was performed through four databases such as Cochrane Library, Web of Science, PubMed, and EMBASE for the literature published until May 2021. The I2 statistic was used to assess heterogeneity. A random/fixed-effects model was used to pool. RESULTS: There are a total of 18 studies including three surgical treatments su

Grapefruit Training Model for Distal Anterior Cerebral Artery Side-to-Side Bypass.

OBJECTIVE: Simulation models enable trainees to master microsurgical skills before performing surgeries. Vascular bypass is a critical component of cerebrovascular and many nonneurologic procedures. However, most available bypass training models lack important spatial, tactile, and physiologic aspects of real surgery. Animal and placental models provide true physiology but are expensive. While some models adequately simulate superficial temporal artery-middle cerebral artery bypass, there is no model for side-to-side distal anterior cerebral artery bypass. The objective is to create a realisti

Treatment of a Fusiform Anterior Cerebral Artery Aneurysm With an A3-A3 Bypass: 2-Dimensional Operative Video.

Distal anterior cerebral artery (ACA) aneurysms remain a relatively rare and challenging entity for both open microsurgical and endovascular treatments. These lesions provide a significant surgical challenge because of the narrow interhemispheric surgical field, limited proximal control, common fixation of the aneurysm to the adjacent cingulate gyrus, and possible involvement of an unpaired azygos A2. These challenges result in a known association between distal ACA aneurysms and higher operative morbidity. This video demonstrates indirect obliteration of a prior ruptured fusiform right A2 ane

Continuous Interrupted Double Throw Suturing Method: A Novel Suturing Technique for Extracranial-Intracranial Bypass.

OBJECTIVE: Despite failure of the EC/IC Bypass Study Group to demonstrate effectiveness in minimizing future stroke events, superior temporal artery-medial cerebral artery (STA-MCA) bypass remains an essential treatment for complex giant intracranial aneurysms, tumors, moyamoya disease with ischemia, and atherosclerotic steno-occlusive disease with hemodynamic cerebrovascular insufficiency. The objective of this report is to describe a novel suturing technique for STA-MCA bypass that helps reduce donor-recipient anastomosis time, allowing for a well-organized systematic workflow. METHODS: Ste

The occipital artery for posterior circulation bypass: microsurgical anatomy.

OBJECT: The microsurgical anatomy of the occipital artery (OA) was studied to describe the diameter, length, and course of this vessel as it pertains to revascularization procedures of the posterior cerebral circulation. METHODS: The authors studied 12 OAs in 6 cadaveric heads that had been injected with colored latex. They evaluated the OA's ability to serve as a conduit for extracranial-intracranial (EC-IC) bypass in the posterior circulation. They measured the length of the OA and its diameter at common sites of anastomosis and compared these values with the diameters of the recipient vess

The current status of the treatment for hemorrhagic type moyamoya disease based on a 1995 nationwide survey in Japan.

The optimal treatment regimen for hemorrhagic type Moyamoya disease has yet to be clearly established. Furthermore, it remains unclear as to whether or not bypass surgery can help prevent future intracranial hemorrhaging in Moyamoya patients. In Japan, several treatment options, such as conservative, medical, and surgical intervention, have been employed for the treatment of hemorrhagic type of Moyamoya disease. In this study, 282 hemispheres with hemorrhagic onset were analyzed based on a 1995 nationwide survey by the Research Committee on Spontaneous Occlusion of the Circle of Willis (Moyamo

[Moyamoya disease with persistent primitive hypoglossal artery. Case report].

A 44-year-old female developed aphasia and visual disturbance of the left eye. Four-vessel angiography showed occlusion of the extra- and intracranial portions of the right internal carotid artery, severe stenosis of the left internal carotid fork, bilateral moyamoya vessels, and a persistent primitive hypoglossal artery on the left side. Measurement of cerebral blood flow by 133xe inhalation revealed decreased flow and no response to acetazolamide loading in the left cerebral hemisphere. The patient underwent superficial temporal artery-middle cerebral artery anastomosis, first on the left an

Direct Versus Indirect Bypass for Moyamoya Disease.

Moyamoya disease is a progressive occlusive vasculopathy that involves the supraclinoid internal carotid arteries and Circle of Willis, and results in the formation of collateral vessels at the skull base. The progressive nature of this disease leads to cerebral ischemia and sometimes intracerebral hemorrhage. The treatment of moyamoya disease is mainly surgical revascularization, using revascularization techniques that include direct, indirect, and combined strategies. Here we discuss the available options for revascularization as well as our opinions regarding the surgical management of pati

Long-term outcomes after revascularization surgery for adult moyamoya disease: Protocol for systematic review and meta-analysis.

INTRODUCTION: Moyamoya disease (MMD) commonly presents with cerebral ischemia or hemorrhage. Revascularization surgery, including direct, indirect, and combined bypasses, remains the mainstay of MMD treatment aiming to reduce stroke recurrence. However, the long-term outcomes of these interventions are still controversial. Herein, we designed the protocol for a systematic review and meta-analysis comparing the three different revascularization surgeries in adult MMD patients for more than 5 years long-term outcomes. METHODS: Our systematic review and meta-analysis protocol follows the PRISMA-

Evaluation of the contribution of individual arteries to the cerebral blood supply in patients with Moyamoya angiopathy: comparison of vessel-encoded arterial spin labeling and digital subtraction angiography.

PURPOSE: Vessel-encoded arterial spin labeling (VE-ASL) is able to provide noninvasive information about the contribution of individual arteries to the cerebral perfusion. The aim of this study was to compare VE-ASL to the diagnostic standard digital subtraction angiography (DSA) with respect to its ability to visualize vascular territories. METHODS: In total, 20 VE-ASL and DSA data sets of 17 patients with Moyamoya angiopathy with and without revascularization surgery were retrospectively analyzed. Two neuroradiologists independently assessed the agreement between VE-ASL and DSA using a 4-po

Evaluation of Hemodynamics Before and After Revascularization in Hemorrhagic Moyamoya Disease: A Computed Tomography Perfusion Imaging Case Study.

OBJECTIVE: To explore the feasibility of computed tomography perfusion imaging (CTP) for evaluating hemodynamics in hemorrhagic moyamoya disease (MMD). METHODS: The retrospective analysis included 25 patients with hemorrhagic MMD who underwent brain CTP examination. Two experienced radiologists manually delineated regions of interest (ROIs) in the bilateral frontal lobe, temporal lobe, brain tissue adjacent to the hemorrhagic foci, and brainstem as a control region. The perfusion values for all ROIs were extracted, including cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit

[Diagnosis and Management Guidelines for Moyamoya Disease].

Moyamoya disease(MMD) is a rare cerebrovascular disease characterized by progressive stenosis of the internal carotid artery terminus and an abnormal formation of a vascular network at the base of the brain. Superficial temporal artery-middle cerebral artery(STA-MCA) bypass, either as a direct or combined revascularization procedure, is a reasonable management choice for patients with symptomatic MMD. STA-MCA bypass prevents cerebral ischemic attacks by improving cerebral blood flow. Recent evidence further suggests that direct revascularization reduces the potential risk of rebleeding in pati

[History and Current Status of Direct or Combined Revascularization Surgery for Moyamoya Disease].

We reviewed the history and development of revascularization surgery for moyamoya disease, particularly STA-MCA bypass. In the early stages, medical treatments, such as vasodilators, were ineffective. In the 1970s, indirect surgical methods were introduced but have shown limited success. Direct bypass techniques have evolved with advancements in microsurgery, and STA-MCA bypass has become the standard treatment. Surgery improves the collateral blood flow and reduces the risk of stroke, especially in patients with ischemia. Despite being generally safe, perioperative complications, such as cere

[EC-IC bypass for occlusion of the internal carotid artery].

BACKGROUND: There are barely any alternative treatment options to the drug treatment of hemodynamically caused cerebral ischemia, as in occlusion of the internal carotid artery. OBJECTIVE: For secondary prevention of an ischemic stroke due to carotid occlusion and hemodynamic instability, extracranial-intracranial (EC-IC) bypass surgery can be an important option in selected patients. MATERIAL AND METHODS: The development, study situation, indications and surgical technique for placement of an EC-IC bypass in cases of occlusion of the internal carotid artery are presented. RESULTS: With app
