Infectious disease complications of combat-related injuries.

BACKGROUND: : Strategies currently used to prevent morbidity and mortality associated with combat-related injuries include better training of frontline medical personnel, improved personal protective equipment, and deployment of fast-forward surgical assets with state-of-the-art equipment. These strategies allow patients to survive near-catastrophic injuries, placing a greater emphasis on the medical infrastructure to mitigate short- and long-term complications associated with these injuries. DISCUSSION: : Wounds have certain characteristics that promote the development of infections: the pre

Evidence for decontamination of single-use filtering facepiece respirators.

Single-use filtering face respirators (FFRs) are critical pieces of personal protective equipment for healthcare workers treating patients with suspected upper respiratory tract pathogens. Experiences during pandemics in the 2000s, as well as the ongoing COVID-19 pandemic caused by the SARS-2-CoV-2, have highlighted concerns over the pressures that sustained respiratory virus pandemics may have on supplies of FFRs globally. Decontamination of FFRs has been posited as one solution to support the re-use of FFRs with a growing body of literature over the last 10+ years beginning to examine both t

Efficacy of SG Shield in reducing droplet contamination during collection of oropharyngeal swab culture specimens.

INTRODUCTION: Oropharyngeal swabs for diagnosis of COVID-19 often induce violent coughing, which can disperse infectious droplets onto providers. Incorrectly doffing personal protective equipment (PPE) increases the risk of transmission. A cheap, single-use variation of the face shield invented by a Singaporean team, SG Shield, aims to reduce this risk. This manikin study aimed to study the efficacy of the SG Shield in combination with standard PPE. METHODS: A person attired in full PPE whose face and chest was lined with grid paper stood in front of an airway manikin in an enclosed room. A s

Boar Semen Contamination: Identification of Gram-Negative Bacteria and Antimicrobial Resistance Profile.

Bacterial contamination of boar semen occurs with some frequency in artificial insemination centers and may have a negative effect on the quality of the semen as well as on the sows' reproductive capacity. Normally, the source of bacterial contamination in pig seminal doses is the own boar. However, distilled water or laboratory equipment used to elaborate the seminal doses can be an important source of bacterial contamination. This study focused on the identification of gram-negative bacteria in boar semen, and impact on the quality of ejaculates obtained from boar, as well as on the establis

Reuse of single use medical devices.

During the ten years that I have worked at the Canadian Nurses Protective Society, we have had numerous calls from Operating Room nurses questioning the practice of re-using single use medical devices in their facilities and voicing their concerns about possible harm to patients, themselves and potential liability exposure. Interest in this topic has been rekindled by the recent Saskatchewan case involving the death of a patient from variant Creutzfeld-Jakob Disease (vCJD) and possible transmission of vCJD to 71 patients who may have been exposed to the same endoscope as the deceased while in

Transmission of SARS and MERS coronaviruses and influenza virus in healthcare settings: the possible role of dry surface contamination.

Viruses with pandemic potential including H1N1, H5N1, and H5N7 influenza viruses, and severe acute respiratory syndrome (SARS)/Middle East respiratory syndrome (MERS) coronaviruses (CoV) have emerged in recent years. SARS-CoV, MERS-CoV, and influenza virus can survive on surfaces for extended periods, sometimes up to months. Factors influencing the survival of these viruses on surfaces include: strain variation, titre, surface type, suspending medium, mode of deposition, temperature and relative humidity, and the method used to determine the viability of the virus. Environmental sampling has i

A qualitative risk assessment of cleansing and disinfection requirements after an avian influenza outbreak in commercial poultry.

1. During an avian influenza (AI) outbreak in the United Kingdom, the joint aim of the poultry industry and the Government is to eliminate and prevent the spread of infection, through control measures based on the current European Union (EU) Council Directive (2005/94/EC). An essential part of these measures is the cleansing and disinfection (C&D) of infected premises.2. This risk assessment assessed the differences in re-infection in a repopulated flock if the EU Directive is interpreted to permit secondary C&D to be undertaken either with or without dismantling complex equipment. The assessm

Transmission of neonatal listeriosis in a delivery room.

Two cases of neonatal listeriosis occurred in a hospital within a two-week period. Both infants were infected with the same organism, Listeria monocytogenes, type 1a, bacteriophage type 6 (lysotype 1652). Both infants were born in the same delivery room, 17 hours apart. The index patient became septic shortly after birth and died after 48 hours despite antibiotic therapy. The mother of the index patient was febrile and had chorioamnionitis. The second infant became ill with meningitis at 13 days of age. Neither infants nor mothers were attended by the same medical or nursing staff nor were the

An assessment of bacterial transmission via rebound tonometry: An in vitro pilot study.

BACKGROUND: Bacterial contamination of medical equipment is a significant risk in both human and veterinary medicine, leading to potential cross-contamination between clinicians, technicians, patients, and healthcare professionals. Pathogens can persist on surfaces, resulting in healthcare-associated infections. Tonometry used to measure intraocular pressure for diagnosing conditions like glaucoma and uveitis, is crucial in both human and veterinary care. However, there is limited information on bacterial transmission through reused tonometer probes in veterinary patients. This pilot study aim

Use of pesticides and risk perception of environmental contamination by farmers surrounding the Menoua River (West Cameroon).

Cameroonian farmers regularly use pesticides in farming to control pests, particularly along the riverbank of the Santchou agricultural area. The aim of this study was to assess the perception of farmers in West Cameroon (Santchou area) regarding the contamination risk in aquatic environments from the use of pesticides. A cross-sectional study via questionnaires was conducted among 150 selected farmers who cultivated fields along the banks of the Menoua River. Almost all the farmers (90%) used pesticides regularly, particularly herbicides, insecticides, and fungicides to combat pests. The main

Elimination of Porcine Epidemic Diarrhea Virus in an Animal Feed Manufacturing Facility.

Porcine Epidemic Diarrhea Virus (PEDV) was the first virus of wide scale concern to be linked to possible transmission by livestock feed or ingredients. Measures to exclude pathogens, prevent cross-contamination, and actively reduce the pathogenic load of feed and ingredients are being developed. However, research thus far has focused on the role of chemicals or thermal treatment to reduce the RNA in the actual feedstuffs, and has not addressed potential residual contamination within the manufacturing facility that may lead to continuous contamination of finished feeds. The purpose of this exp

Determining the best method for assessing microbiological contamination on flexible endoscopes without a working channel: a pilot study.

BACKGROUND: Flexible endoscopes (FEs) without a working channel can become extensively contaminated. Adequate reprocessing is critical and the quality of these processes can be verified by microbiological sampling. This study aimed to determine the ability to detect colony-forming units (CFUs) after sampling used FEs without a working channel for four different sampling techniques. METHODS: FEs without a working channel were collected after clinical use. Manual pre-cleaning with tap water was performed. A sample was collected from the distal 8-10 cm and tip of the FE by one of four sampling t

Occupational Exposure Assessment to Antineoplastic Drugs in Nine Italian Hospital Centers over a 5-Year Survey Program.

In the present study, surface contamination where antineoplastic drugs (ADs) are present was investigated, as occupational exposure risk is still an open debate. Despite recommendations and safety standard procedures being in place in health care settings, quantifiable levels of ADs are being reported in the recent literature. Thus, a survey monitoring program was conducted over five years (2016-2021) in nine Italian hospitals. The repeated surveys produced 8288 data points that have been grouped according to the main hospital settings, such as pharmacy areas and patient care units. Based on t

Steam Sterilization Chemical Indicators Are Not Adequate for Monitoring Real Steam Sterilization Cycles.

Chemical indicators are commonly used in hospitals to monitor steam sterilization conditions, indicating that medical devices are safe to be used. The results are stored for future evidence in the event of an infection incident root cause analysis. This type of indicator is also becoming an option for cycle monitoring in pharmaceutical steam sterilizers, improving cycle control. They are constructed and tested according to published standards, but contradictory results between chemical indicators and cycle printouts have a critical impact on process control. We found that Type 6 chemical indic

Evaluation of decontamination efficacy of four antineoplastics (ifosfamide, 5-fluorouracil, irinotecan, and methotrexate) after deliberate contamination.

The main objective was to determine the decontamination efficacy of quaternary ammonium, 0.1% sodium hypochlorite, and water after deliberate contamination with four antineoplastics (ifosfamide, 5-fluorouracil, irinotecan, methotrexate). A stainless-steel surface was deliberately contaminated with ifosfamide (15 µg), 5-fluorouracil (10 µg), irinotecan (1 µg), and methotrexate (1 µg). First, a single decontamination step with either water, quaternary ammonium, or 0.1% sodium hypochlorite was tested. Then, the effect of up to four successive decontamination steps with either quaternary ammonium
