26 children with 29 lower limb fractures, between the age of 2 and 15 years, were treated with an external fixator. The average time to union and removal of the fixator was 71 days for femoral and 73 days for tibial fractures. Complications included pin problems in nine cases, two devices had to be replaced with a cast and one refracture. Leg length discrepancies were noted in five children and radiological malalignment in three. CASE: We describe a case of femoroacetabular impingement syndrome treated with labral repair and arthroscopic femoral osteoplasty in a 30-year-old man. The case was complicated by resection of 50% of the femoral neck resulting in high risk of an impending femoral neck fracture. The patient was treated with a vascularized fibula autograft and internal fixation.
CONCLUSION: Complications of femoral neck over-resection can threaten the viability and structural integrity of the hip joint but may successfully be treated with vascularized fibula autograft.

Twenty-nine patients with tibial non-union or delayed union were treated by onlay grafting, using long struts from the ipsilateral fibula and without disturbing the non-union site. Internal fixation and excessive dissection can be avoided. Follow-up in 28 patients showed solid healing in all; 20/28 patients united the fracture within 7 months. Removal of the fibula as a distracting force, combined with the principles of onlay grafting advocated by Phemister, is effective surgical treatment.

CONCLUSIONS: When nailing far-distal extra-articular tibia and fibula fractures, adding a second interlock provides more stability than adding a fibular plate. Distal fibula plating may have minimal biomechanical effect in extreme nailing.

Superior dislocation of the fibular head associated with a fractured tibia occurred in four cases with a direct, severe trauma to the tibia. In one case there was permanent peroneal nerve damage. This injury is somewhat analogous to the fracture-dislocation of the Monteggia type in the forearm. To diagnose secondary dislocations of the fibula and treat the condition early, the knee joint should be carefully examined before and after X-ray of all fractures of the tibia.

A six-month-old, male Bernese Mountain Dog in which radius-ulna and tibia-fibula concomitant fractures were treated each with a 3.5 mm Locking Compression Plate (LCP) is presented. Both fractures were approached and plated medially. The tibial fracture had to be revised with a 4.5 mm intramedullary nail and a new 3.5 mm LCP at the second post-operative day because of fixation breakdown. The follow-up radiographs taken at days 14 and 53 revealed uneventful healing of both fractures. Implants were removed 53 days after surgery.

CASE: A 48-year-old man with multiple injuries sustained a Gustilo type 3A right open distal humerus fracture with a 4-cm wound, contralateral radius shaft, and ilium fractures. Following wound irrigation on the day of injury, second look debridement showed crushed triceps and severe contamination extending into the distal humerus medullary canal. After 6 days of initial plate fixation, he developed signs of deep infection.
