BACKGROUND: Fixed flexion deformity of the proximal interphalangeal joint with or without hyperextension of the metatarsophalangeal joint is one of the most common foot deformities. Many operative options have been recommended. Complaints after operative procedures include a too straight toe, floating toe, painful toe recurvatum, mallet toe, pin track infection, broken hardware, and the necessity of removing hardware. A proximal interphalangeal joint arthrodesis for hammertoe deformity using a 2-mm absorbable pin for internal fixation is described.
METHODS: The results of 48 toe arthrodeses i

BACKGROUND: Successful fixation of the greater trochanter (GT) in total hip arthroplasty (THA) is a challenging task. A wide range of clinical results are reported in the literature despite advancements in fixation technology. Previous studies may have lacked adequate sample sizes to detect differences. This study evaluates nonunion and reoperation rates and determines factors influencing successful fixation of the GT using current-generation cable plate devices.
METHODS: This retrospective cohort study included 76 patients who underwent surgery requiring fixation of their GT and had at least

PURPOSE: Kirschner wires (K-wires) are widely used for fracture fixation in paediatric trauma surgery. Although infectious complications are uncommon, they remain clinically relevant. Subcutaneous burial of K-wires has been proposed to reduce the risk of infection, but robust data from large paediatric cohorts remain limited. This study aimed to evaluate infectious and non-infectious complications following routine use of buried K-wires in children.
METHODS: In this retrospective cohort study, paediatric patients (≤ 18 years) who underwent fracture fixation with routinely buried K-wires at a

INTRODUCTION: Metallic tension-band wiring with K-wires and cerclage: tension-band wiring (TBW) remains the gold standard for surgery for patella fractures. Re-operation due to symptomatic implants is common. Non-metallic options, such as suture tension-band fixation (STBF), could be a solution. This study aimed to compare an STBF group with a TBW group with respect to complications and implant removal, and to describe the clinical results of the STBF group.
METHODS: All patient files with a patella fracture operated at our institution from 1 January 2016 to 19 August 2019 were reviewed on 1

The loop anchor tension band wiring (TBW) technique is employed for olecranon fractures and aims to reduce the Kirschner wire (K-wire) migration rate. This study compared the postoperative outcomes and complication rates of olecranon fractures treated with Arbeitsgemeinschaft für Osteosynthesefragen (AO)-modified TBW versus the loop anchor TBW. This study enrolled 83 olecranon fractures treated with AO-modified TBW (n = 62) or loop anchor TBW (n = 21). K-wire migration, implant removal, time to fracture union, operation time, and subjective functional outcomes including the Mayo Elbow Performa

BACKGROUND/PURPOSE: Bar rotation remains a relevant complication following minimally invasive repair of pectus excavatum (MIRPE), with potential need for reoperation and risk of severe intrathoracic injury. Bridge fixation has been proposed as a stabilization strategy to prevent implant rotation; however, evidence supporting its effectiveness remains limited. This study aimed to evaluate the stability of bridge fixation in a large multicenter cohort.
METHODS: A retrospective multicenter study was conducted including patients undergoing chest wall repair with intrathoracic implants stabilized

We describe an oblique step-cut osteotomy technique for ulnar shortening. The technique requires no special instrumentation. It requires only an eight-hole 3.5-mm low-contact dynamic compression plate for fixation. Sixteen patients with posttraumatic ulnar impaction syndrome were treated. The average patient age was 40 years. The mean followup duration was 8 (range, 3 to 28) months. No nonunions occurred. Two patients required hardware removal for symptomatic hardware. Fourteen of the 16 patients reported persistence of occasional pain with some activities, but only 10 had any pain elicited at…

BACKGROUND: Although clavicular fractures are common, nonunion of the clavicle is a rare complication. However, it can be disabling, presenting mainly with pain, limitation of shoulder movement, and/or compression of the brachial plexus. The technical difficulty in securing adequate skeletal stabilization and the unique anatomic features of the clavicle pose a challenge for the orthopedic surgeon.
METHODS: Sixteen patients with symptomatic nonunion of the clavicle were studied. They were nine men and seven women with a median age of 34 years (range, 15-52 years). The average follow-up was 12.

A forearm plate retained for 50 years was removed because an acute inflammation mimicked chronic infection. The radiographs were very confusing. During the operation metallosis was obvious. Despite the general opinion that forearm plates should be kept in place, this probably should not apply to old "historical" hardware of uncertain chemical composition.

BACKGROUND: Sciatic neuropathy associated with acetabular fractures can result in disabling long-term symptoms. The purpose of this retrospective study was to evaluate the effect of sciatic nerve release on sciatic neuropathy associated with acetabular fractures and reconstructive acetabular surgery.
METHODS: Between 2000 and 2004, ten patients with sciatic neuropathy associated with an acetabular fracture were treated with release of the sciatic nerve from scar tissue and heterotopic bone. Additional surgical procedures included open reduction and internal fixation of the acetabulum (five pa

Non-union following intramedullary nailing of diaphyseal long bone fractures represents a serious complication necessitating timely and effective intervention. The situation becomes considerably more complex in the presence of a broken femoral nail, which poses additional technical challenges for the operating surgeon. Conventional management strategies typically entail removal of the existing nail, followed by re-fixation with a larger intramedullary nail, plate fixation, or external fixation constructs. Our case describes a 34-year-old male patient who presented with aseptic femur non-union

OBJECTIVE: Patients with chronically implanted deep brain stimulator (DBS) electrodes can encounter complications requiring hardware removal. We assessed the safety and efficacy of using implanted DBS electrodes to create a therapeutic lesion before their removal.
METHODS: Revision or removal of the DBS electrodes was required in two patients who had previously undergone DBS implantation. We conducted a series of in vitro experiments to confirm that the DBS electrodes could be used to generate radiofrequency lesions and to assess the relationship between radiofrequency parameters and lesion s…

Intramedullary (IM) fixation for the total wrist arthrodesis (TWA) is supposed to lower the hardware complication rate by eliminating soft tissue irritation. In this report, we present three patients with distal metacarpal screw migration requiring unplanned secondary surgery for screw removal in one patient while it was managed nonoperatively in the other two. All three patients had complete radiocarpal fusion by four months postop. There was no attempted third carpometacarpal (CMC) fusion in any of our patients. Screw migration was found between 1.5-3.5 months postop and remained stable unti
