OBJECTIVE: Totally endoscopic surgery compared with the conventional heart operation in children is described in this article to find a preferable treatment for congenital heart diseases.

We have experienced open heart surgery of severe valvular aortic stenosis associated with atrial septal defect in a 78-year-old female patient. This is the oldest case in Japanese literature. Because balloon valvuloplasty can not produce improvement in hemodynamic and clinical status, open heart surgery was performed as the final decision. The operative procedure consisted of aortic valve replacement with a 19 mm St. Jude Medical valve and direct closure of atrial septal defect. Postoperative course was excellent. The patient is doing well at the two years and three months of follow-up period.

A 40-year-old woman underwent an atrial septal defect closure 4 years before presentation. During the operation, juxtaposition of the atrial appendages was found simultaneously but no obvious communication was found between the appendages. She recently experienced desaturation on exercise, and the residual communication was found between the juxtaposed atrial appendages. The residual communication was closed from the right to the left atrium. Herein, we report the rare case of juxtaposition of the atrial appendages with residual communication between them after an atrial septal defect closure.

BACKGROUND: Noonan syndrome-associated hypertrophic cardiomyopathy (NS-HCM) is often lethal when presenting severely in infancy, especially when associated with structural heart disease. The neonatal nurse is a vital link in the recognition and management of infants born with congenital heart disease. This article examines the acyanotic cardiac defects with increased pulmonary blood flow: patent ductus arteriosus, atrial septal defect, ventricular septal defect, and endocardial cushion defects. The hemodynamics, clinical presentation, differential diagnosis, surgical repair, and medical management of each defect are presented. Parental teaching needs are identified and discussed.

This cross-sectional study assessed distribution and pattern of echocardiography confirmed congenital heart disease, among 593 pediatric patients in outpatient departments of a tertiary care hospital in eastern India.
