Pseudohemothorax induced by residual contrast medium mimicking aortic dissection rupture.

Hemothorax is an urgent condition, and its accurate diagnosis and the identification of the cause are important. Herein, we report a case of a 74-year-old man with end-stage renal disease who was presented with high-concentration pleural effusion owing to residual contrast medium. The case required differentiation from hemothorax owing to an aortic dissection and its rupture. In patients with end-stage renal disease, noncontrast-enhanced computed tomography after contrast-enhanced computed tomography may result in high-concentration pleural effusion owing to the existence of residual contrast …

Three faces of costal exostoses: case series and review of literature.

The 3 cases presented here involve 3 unusual and different complications of costal exostoses: spontaneous hemothorax, pneumothorax, and pericardial effusion. All of the cases presented with chest pain to the emergency department. Two of the cases have hereditary multiple exostoses. All but one of the patients required surgical intervention. Complications of costal exostoses are rare, but hemothorax, pneumothorax, and pericardial effusions can occur.

Intercostal artery laceration following thoracentesis.

Intercostal artery laceration is an unusual complication following thoracentesis, and has been reported only in elderly patients. We report a case of a 78-year-old man who developed a massive hemothorax following thoracentesis. Post-thoracentesis radiograph revealed a substantial increase in pleural fluid, and emergency chest tube insertion identified the hemothorax. He underwent right thoracotomy for repair of the intercostal artery laceration.

Ruptured bronchial artery aneurysm associated with bronchiectasis: a case report.

A massive hemothorax from a ruptured bronchial artery aneurysm (BAA) is very rare. Only 12 cases of ruptured mediastinal BAA have been reported. This case study describes a 77-year-old female with bronchiectasis who presented with anemia, hypertension, hemothorax, and a mediastinal mass. A chest tube was inserted through which 2 liters of unclotted blood was drained from the left pleural cavity. An enhanced computed tomography scan revealed a ruptured 3-cm diameter mediastinal aneurysm of a bronchial artery supplying the left lower lobe. Transcatheter artery embolization (TAE) with multiple mi…

[A case of intrapleural rupture of pulmonary arteriovenous malformation with shock].

We report a case of a 59-year-old male who suddenly developed massive right hemothorax and shock due to pulmonary arteriovenous malformation (AVM). He was admitted as an emergency case because of severe back pain, cyanosis and cold sweat. Although shock with massive right hemothorax was found, echocardiographic and X-ray computed tomographic examination showed no dilatation of the aorta, no intimal flap in the aorta and no pericardial effusion which suggested aneurysm. Pulmonary arteriography, performed subsequently, disclosed intrapleural rupture of the right pulmonary AVM. The patient recove…

Late-onset vital complication after the Nuss procedure for pectus excavatum.

The Nuss procedure is the most popular technique for correction of pectus excavatum recently. Life-threatening complications associated with the procedure are very rare. We report a 13-year-old boy who developed late-onset bilateral hemothorax with hypovolemic shock 5 months after the Nuss procedure. In literature review, this is the first case of the late-onset life-threatening bilateral hemothorax with hypovolemic shock ever reported.

Complications related to subclavian catheters for hemodialysis. Report and review.

Personal experience with subclavian vein cannulations for hemodialysis are given, and the pertinent literature on the subject is reviewed. Two hundred subclavian dialysis catheters were placed in 148 patients who kept them in place for a total of 2,798 days. Immediate complications were two pneumothoraxes and two hemothoraxes due to subclavian artery puncture. Seventeen cases of bacteremia were related to subclavian catheter infections. In 1 case, a complication of sepsis was a vertebral osteomyelitis. Clinical evidences of subclavian vein thrombosis occurred in 5 cases. Life-threatening compl…

[Impact of the practice of "Extended Focused Assessment with Sonography for Trauma" (e-FAST) on clinical decision in the emergency department].

BACKGROUND: We aimed to show the sensitivity of Extended Focused Assessment with Sonography for Trauma (e-FAST) for detection of pneumothorax, hemothorax and intraabdominal injury. We also investigated the relationship between e-FAST and need for invasive treatment. METHODS: This study included patients who experienced multiple trauma. The emergency physician, who had no clinical information about the patient, performed e-FAST. Findings on a supine chest X-ray and invasive interventions were recorded. The results of abdomen and thorax computed tomography (CT) were reviewed (the size of the pn…

Endovascular repair of aortic lesions associated to video-assisted thoracoscopy surgery after chest trauma.

AIM: Trauma of the thoracic aorta for blunt trauma shows a very high incidence of mortality. Hospital mortality rate after aortic open surgery is between 15% and 30%. Endovascular management represents an alternative treatment Associated lesions are usually seen in those critical patients. Hemothorax may be present. The authors propose a combined treatment of endovascular repair for the aortic lesion and video-assisted thoracoscopy surgery (VATS) for the treatment of chest bleeding complications. METHODS: The authors report a series of three patients with post-traumatic aortic lesion and hemo…

Spinal cord infarction following central-line insertion.

Hemothorax is a recognized complication of central line insertion into the jugular or subclavian vein. We describe a case of hemothorax consequent upon acute dialysis catheter insertion, which resulted in spinal cord infarction and quadriplegia. We postulate that the extensive mediastinal shift induced after insertion of the catheter resulted in stretching of the veins draining the cord with a resultant drop in perfusion pressure and infarction. This case highlights a hitherto unreported complication of this procedure.

[Wilson-Jones type angiosarcoma with marked response to intrapleural administration of interleukin-2].

A 64-year-old woman was admitted to our hospital with a hemothorax after one year of therapy for angiosarcoma that had arisen from the skin of the head. The hemothorax was believed to have resulted from metastasis of the angiosarcoma. Interleukin-2 (IL-2) at a dose of 20 x 10(4) U once daily was administered intrapleurally. Clinical improvement was first observed on the second day after the first dose of IL-2. Increases in natural and in lymphokine-activated killer activity of lymphocytes in pleural effusion were found on the eighth day. Starting on the 16th day of IL-2 therapy, no more fluid …

Delayed retained hemopneumothorax secondary to rib fracture with pulmonary perforation treated by video-assisted thoracoscopic surgery: a case report.

Retained hemothorax is a complication of thoracic trauma caused by incomplete pleural drainage and early fibrotic organization, potentially progressing to trapped lung and functional impairment if untreated. A 60-year-old man sustained fractures of the eighth, ninth, and tenth right ribs after a fall, one associated with pulmonary injury. Without initial medical care, he presented 15 days later with progressive dyspnea. Chest radiography showed right lung collapse with massive hemopneumothorax. Closed thoracostomy drained 4000 ml of blood, but lung re-expansion failed due to organized hemothor…

The diagnostic problem associated with blunt traumatic azygous vein injury: delayed appearance of right haemothorax after blunt chest trauma.

Azygous vein injury (AVI) associated with blunt chest trauma is rare, but it can become a very serious problem if diagnosis and treatment are delayed. However, in 13 reported cases of AVI, including the present case, right hemothorax was not found on the initial chest X-ray film, even though its delayed appearance was confirmed in 3 out of 13 patients (23.1%). Thus, the diagnosis of AVI can be hampered because of delayed right hemothorax (DRH).

Blunt trauma with flail chest and penetrating aortic injury.

Blunt chest trauma with flail chest is common. The mortality attributes initially to the associated pulmonary contusion, massive hemothorax and later to the occurrence of adult respiratory distress syndrome. We report a case of flail chest with segmental fractures near the costovertebral junction and delayed hemothorax attacked 14 h later. The final diagnosis of the penetrating aortic injury by detached rib fragment was appreciated by aortogram. Unfortunately, active aortic hemorrhage made prompt thoracotomy in vain for life salvage.

Hemopneumothorax detection through the process of artificial evolution - a feasibility study.

BACKGROUND: Tension pneumothorax is one of the leading causes of preventable death on the battlefield. Current prehospital diagnosis relies on a subjective clinical impression complemented by a manual thoracic and respiratory examination. These techniques are not fully applicable in field conditions and on the battlefield, where situational and environmental factors may impair clinical capabilities. We aimed to assemble a device able to sample, analyze, and classify the unique acoustic signatures of pneumothorax and hemothorax. METHODS: Acoustic data was obtained with simultaneous use of two …

Urgent thoracotomy for thoracic hemorrhage after posterior correction surgery in adolescent idiopathic scoliosis: case report and literature review.

BACKGROUND: Posterior correction surgery for adolescent idiopathic scoliosis (AIS) carries a risk of rare, life-threatening complications. We present a case of massive tension hemothorax following AIS surgery, highlighting the diagnosis and management challenges. CASE DESCRIPTION: A 14-year-old girl with AIS (82° thoracic curve) underwent posterior correction and fusion surgery from T3 to L1. Intraoperative bleeding was approximately 800 ml. Shortly after extubation, the patient developed acute respiratory distress and hypotension. Examination and chest radiography confirmed a left-sided tensi…
