Serum sodium level fluctuations following the resection of childhood-onset craniopharyngioma.

BACKGROUND: Craniopharyngiomas are low-grade malignancies (WHO I) in the sellar region. Most cases of childhood-onset craniopharyngioma are … antan Hospital and Beijing Children's Hospital and identified predictors of severe hyponatremia and hypernatremia. RESULTS: The mean age of the cohort was 8.35 ± 4.35 years. The prevalence of hypernatremia, hyponatremia, and their severe forms (serum Na+  > 150 mmol/L and serum Na+  < 130 mmol/L) within 14 days after surgery was 66.3%, 72.8%, 37.1%, and 40.6%, …

Sodium restriction versus daily maintenance replacement in very low birth weight premature neonates: a randomized, blind therapeutic trial.

To test the hypothesis that restriction of sodium intake during the first 3 to 5 days of life will prevent the occurrence of hypernatremia and the need for administration of large fluid volumes, we prospectively and randomly assigned 17 babies (mean +/- SD: 850 +/- 120 gm; 27 +/- 1 weeks of gestation) to receive in blind fashion either daily maintenance sodium or salt restriction with physician-prescribed parenteral fluid intake. Maintenance-group infants received 3 to 4 mEq of sodium per kilogram per day; restricted infants received no sodium supplement other than with such treatments as tran…

Hypernatremic hemorrhagic encephalopathy.

A 12-year-old juvenile diabetic was inadvertently given 500 ml of hypertonic saline intravenously. He developed hypernatremia, hyperosmolality, metabolic acidosis, and hyperglycemia. Seizures and stupor ensued, followed by coma and death. Computerized cranial tomography revealed numerous small subcortical hemorrhages that were verified postmortem.

Diabetes insipidus uncovered during conservative management of complicated acute appendicitis.

Diabetes insipidus (DI) arises from impaired function of antidiuretic hormone, characterized by hypovolemia, hypernatremia, polyuria, and polydipsia. This case is a reminder of the rare but challenging obstacle that undiagnosed DI poses in fasting surgical patients, requiring prompt recognition and vigilant management of marked homeostatic imbalances.

Juvenile-onset gout and adipsic diabetes insipidus: A case report and literature review.

The prevalence of juvenile-onset gout has been increasing. Hereditary factors and secondary diseases should be considered in these patients. Adipsic diabetes insipidus (ADI) is characterized by arginine vasopressin (AVP) deficiency, which results in hypotonic polyuria, and dysfunction of thirst osmoreceptors, which results in failure to generate a thirst sensation in response to hypernatremia. We herein report a case of a boy with gouty arthritis, refractory hyperuricemia, prominent hypernatremia, a high creatinine concentration, and a history of surgery for a hypothalamic hamartoma. The patie…

Isotonic saline solution as maintenance intravenous fluid therapy to prevent acquired hyponatremia in hospitalized children.

OBJECTIVES: This review aims to evaluate if the use of hypotonic saline solutions as maintenance intravenous fluid therapy in hospitalized children increases the risk of hyponatremia, if the administration of isotonic fluids is able to protect against acquired hyponatremia and if the isotonic solutions increase the risks of deleterious effects such as hypernatremia or fluid overload. SOURCES: We researched the relevant literature on the PubMed (Jan 01 1969 to Jul 13 2011), EMBASE (1989 to 2011) and Cochrane Library (1989 to 2011) databases. Furthermore, references of selected studies were inc…

V2 vasopressin receptor deficiency causes changes in expression and function of renal and hypothalamic components involved in electrolyte and water homeostasis.

Polyuria, hypernatremia, and hypovolemia are the major clinical signs of inherited nephrogenic diabetes insipidus (NDI). Hypernatremia is commonly considered a secondary sign caused by the net loss of water due to insufficient insertion of aquaporin-2 water channels into the apical membrane of the collecting duct cells. In the present study, we employed transcriptome-wide expression analysis to study gene expression in V2 vasopressin receptor (Avpr2)-deficient mice, an animal model for X-linked NDI. Gene expression changes in NDI mice indicate increased proximal tubular sodium reabsorption. Ex…

Cranial ultrasound and CT findings in infants with hypernatremic dehydration.

We present two newborn infants with hypernatremic dehydration with central nervous system (CNS) involvement. Both patients showed similar imaging findings, demonstrating generalized brain parenchymal abnormality and multifocal areas of hemorrhage or hemorrhagic infarction. These findings are compatible with previously described CNS pathologic findings in hypernatremia.

Central Diabetes Insipidus in Suprasellar Germ Cell Tumor Paradoxically Presenting With Oliguria.

Central diabetes insipidus may present as oliguric acute kidney injury (AKI), particularly in patients with communication difficulties. While polyuria is common in AKI recovery, persistent or unexplained hypernatremia should prompt suspicion for alternative diagnoses such as unmasked diabetes insipidus to ensure timely recognition and appropriate management.

Central Diabetes Insipidus and Brain Herniation Secondary to Cryptococcal Meningitis in a Dog.

Central diabetes insipidus (CDI) should be considered in neurologic patients with persistent polyuria and rising sodium concentrations despite appropriate fluid therapy. Earlier recognition, serial urine concentration monitoring, and desmopressin administration may help prevent severe hypernatremia, although outcomes may remain poor in patients with progressive intracranial disease.

Clinical and diagnostic complexities of adipsic arginine vasopressin deficiency.

Adipsic arginine vasopressin deficiency (AAVP-D) is a rare hypothalamic disorder, with inadequate vasopressin production and impaired thirst, resulting in sodium and water dysregulation. It poses multiple challenges, including increased morbidity and mortality. We present a 24-year-old female who developed confusion after neurosurgical debulking of a central neurocytoma. Investigations revealed hypernatremia alongside raised plasma osmolality, low urine osmolality, polyuria, and adipsia. Transient AAVP-D was suspected; it was successfully treated with intravenous desmopressin, which resolved p…

Oral rehydration therapy for low birth weight neonates suffering from diarrhea in the intensive care unit.

A total of 62 low birth weight (LBW) neonates (29 boys and 33 girls) suffering from diarrhea in our neonatal intensive care unit were includ … ithin 24 h. Diarrhea lasted less than 1 day in 92% of cases. The regimen corrected hyponatremia and hypernatremia. Only two neonates developed mild asymptomatic hypernatremia. No child became edematous. Using the WHO solution, ORT was safely given to LBW neonates, sparing painful and potentially hazardous invasive techniques, and resulted in excellent outcomes.

SGLT2 inhibitor in a type 2 diabetes mellitus patient coexisted with central diabetes insipidus following hyperosmolar hyperglycemic state.

BACKGROUND: Central diabetes insipidus (CDI) is a rare complication following a hyperglycemic hyperosmolar state (HHS) in patients with type 2 diabetes mellitus (T2DM). The coexistence of T2DM and CDI can lead to diagnostic challenges, particularly when the patients present with persistent hypernatremia without a sense of thirst. CASE PRESENTATION: This case report describes a young woman with T2DM and HHS who developed persistent hypernatremia without thirst. The diagnosis of CDI was delayed until she exhibited polydipsia, consuming up to 10 L of water per day, following the administration o…

Masked Diabetes Insipidus Hidden by Severe Hyponatremia: A Case of Pituitary Metastasis of Lung Adenocarcinoma.

BACKGROUND Hyponatremia is an electrolyte disorder frequently encountered by clinicians. Secondary adrenal insufficiency due to pituitary me … smopressin administration was required to prevent water depletion and to immediately ameliorate the hypernatremia. CONCLUSIONS This is the first case report of masked diabetes insipidus that demonstrated high-grade hyponatremia. Secondary adrenal insufficiency can mask the hypernatremia that is a typical manifestation of diabetes insipidus. Physicians should co …

Central diabetes insipidus in hypoxic brain damage.

We describe two children who after cardiopulmonary arrest developed hypernatremia at the terminal stage. Urinary antidiuretic hormone concentration was very low, indicating central diabetes insipidus. These cases illustrate the necessity of alertness to the development of central diabetes insipidus in patients with severe hypoxic brain damage.

Hypocalcemic tetany due to sodium phosphate ingestion in acute renal failure.

The ingestion of a sodium phosphate laxative preparation (Phosphosoda, Fleed) in a patient who had acute renal failure resulted in hypocalcemic tetany primarily due to severe hyperphosphatemia. Hypernatremia and a marked anion gap were present and attributable in part to the sodium phosphate ingestion of this preparation in the presence of renal failure.
