This case report demonstrates the lack of correlation between clinical sensitivity to insect venoms and immunologic reactivity as indicated by the presence of venom-specific IgG. Lung function and immunologic studies have been performed in 9 patients with bird breeder's hypersensitivity pneumonitis at the time of diagnosis and at intervals from 8 to 30 months after they ceased exposure to the provoking antigen. All 9 patients had evidence of small airway obstruction at the time of diagnosis. Complete recovery of lung function occurred in 4 patients. Hemophagocytic lymphohistiocytosis (HLH) is a rare and life-threatening syndrome that may be triggered by hereditary factors, autoimmune and immunologic disorders, infectious diseases, malignancies and medications. Suspicion of the disease and early treatment is of paramount importance. Since the presentation of HLH with only skin involvement is rare, early diagnosis may be difficult. A pathologically confirmed HLH case that presented with maculopapular skin lesions after certolizumab treatment is being reported in this presentation.

Idiopathic nephrotic syndrome represents up to 30% of adult glomerulopathies. However, its prognosis according to remission, relapse and renal failure remains unchanged since the 80s and prediction remains difficult. Physiopathology of adult idiopathic nephrotic syndrome is complex and multifactorial, including immunologic and environmental factors and a putative permeability-circulating factor, still unknown. In this point of view, we propose to summarize actual knowledge about idiopathic minimal change disease and focal and segmental glomerulosclerosis physiopathology.

Psoriatic arthritis (PsA) is an inflammatory arthritis associated with psoriasis. It is classified with the human leukocyte antigen B27 seronegative spondyloarthropathies because it shares certain features with other conditions included in that group. Specific features of PsA, however, distinguish it from both rheumatoid arthritis and other spondyloarthropathies. Although the cause and pathogenesis of PsA are not known, genetic, immunologic, and environmental factors are thought to play a role, and provide a rationale from therapeutic modalities.

Lichen ruber planus is considered a T-cell mediated immunologic reaction with unclear pathogenesis. Drugs and infections (especially hepatitis B and C) are well-known provocation factors. After an infection with scabies a 13 year old boy developed a generalized lichen planus including bullous lesions. For the first time scabies is presented as a trigger for lichen planus. In childhood, lichen planus is characterized by atypical clinical features and sometimes a severe course.

The presence of cooperative hormonal-immunologic reactions in the female reproductive tract is substantiated. The features of their regional morphologic manifestations in menarche, hormonal pregnancy and non-complicated delivery are described. The importance of specific hormonal factors in the pathogenesis of ascending genital infections at various stages of the gestational period is shown. The possibilities of a pathomorphologic method in the evaluation of the hormonal -- immunological status when examining a gynecological-obstetrical material are determined.

Ulcerative colitis and Crohn's disease are idiopathic conditions that represent an overlapping clinical spectrum that is commonly referred to as inflammatory bowel disease (IBD). Among the etiopathogenic hypotheses that have been suggested, infectious, genetic, and immunologic factors have received the most attention in the modern era--sometimes in combination. This article discusses current concepts regarding the etiology, demographics, and general clinical characteristics of IBD. The latter discussion covers signs/symptoms, laboratory abnormalities, and endoscopic findings.

Adequate kidney biopsy was studied in 54 cases of leprosy, of which 45 were of lepromatous leprosy, 4 of tuberculoid type and 5 belonged to borderline type of leprosy. Membranous glomerulonephritis (31.5%) was the commonest type of glomerular lesion followed by mesangioproliferative glomerulonephritis (11.1). It is concluded that renal lesions can occur in any type of leprosy. Reaction of renal tissues to M. leprae could be due to various local immunologic or physiological factors.

Endometriosis is a chronic, estrogen-dependent inflammatory disorder affecting ~10% of reproductive-age women. It is underdiagnosed, with delays of 5-12 years. Symptoms include dysmenorrhea, chronic pelvic pain, dyspareunia, dyschezia, dysuria, fatigue, and infertility with endometriosis present in up to 50% of infertile patients. Periocular infection is a rare complication of strabismus surgery. We describe a case of necrotizing Tenon's capsule infection after uncomplicated strabismus surgery in a boy with Down syndrome and blepharitis. Pathologic diagnosis was severe acute necrotizing inflammation with Gram positive coccal forms. Resolution of infection occurred after surgical debridement and intravenous and topical antibiotics. Work-up revealed lymphopenia related to Down syndrome. Patients with Down syndrome may have risk factors for postoperative infection including blepharitis and immunologic abnormalities.

Drug-induced agranulocytosis was defined as a severe selective neutropenia caused by an unexpected drug reaction. Metamizole was the most common nonopioid analgesic drug associated with agranulocytosis. It was also associated with combined blood dyscrasias and other severe immunologic disorders. The risk of agranulocytosis by metamizole seemed to be considerably higher than estimated formerly. Modern management with broad-spectrum antibiotics and haematopoietic growth factors reduced the mortality in those patients. Two cases of agranulocytosis caused by metamizole were reported.

MRI, 1H-MR spectroscopy, immunologic, and virologic studies were performed in search of prognostic factors of disease evolution in patients with progressive multifocal leukoencephalopathy (PML). Acute lesions of PML survivors showed twofold higher standard score of the ratio of myoinositol (a glial marker) to creatine compared with lesions in patients whose disease progressed. Concomitantly, JC virus-specific cytotoxic T lymphocytes were only detected in the blood of PML survivors. These results suggest that inflammation limits disease progression.

A large number of HPV types infect humans. They infect squamous epithelia of the skin and mucous membranes. Infections are widespread in sexually active populations. Some HPV types have oncogenic potential. Infections with HPV-16, HPV-18, and some additional types are risk factors for cervical cancer. The HPV-6 and HPV-11 types are responsible for most of the genital condylomas and nearly all respiratory papillomas. The immunologic responses to HPVs are not well understood.

The authors report a case of combined mesenteric and retroperitoneal fibrosis resulting in vascular and digestive compressions. Diagnosis was established by laparoscopy and biopsy. No etiologic factors were found. Such an association is exceptional and suggests that the disease is systemic. The possibility of fibrosis arising far from the mesenterium (mediastinum, extrahepatic biliary tract, thyroid, orbit) has led some authors to suggest that this condition be termed Idiopathic Systemic Fibrosis. An immunologic origin has been speculated in the literature.

Observations in humans and animal studies support the theory that immunologic surveillance plays an important role in limiting the development of skin malignancies. These immune responses undergo progressive diminution with age. In addition, other factors, such as bereavement, poor nutrition, and acute and chronic exposure to ultraviolet light, can further diminish immune mechanisms. We believe these decreases in immune function in combination with the carcinogenic effect of ultraviolet light are important in the production of skin cancers.
