Effect of nerve block combined with superficial general anaesthesia on anaesthetic dosage and anaesthetic awakening in paediatric surgery.

BACKGROUND: To compare the effects of nerve block composite general anesthesia and general anesthesia on the dosage of anesthetics and anesthesia awakening in pediatric orthopedic injury patients under anesthesia depth monitoring. METHODS: Forty pediatric patients with external humerus condylar fractures were randomly distributed into general anesthesia (GA) and nerve block combined with general anesthesia (GNA) groups. Patients in the GA group had induction of anesthesia with propofol at 2.5 mg/kg and remifentanil at 0.6 ug/kg. The Angel-6000D EEG anesthesia depth multi-parameter monitor mai

Rare intraoperative findings during the management of pediatric inguinal pathologies: a decade of experience.

BACKGROUND: Pediatric inguinal surgeries, including procedures for inguinal hernias, undescended testes, and hydroceles, are among the most common surgical interventions in children. While these conditions are well-understood, rare intraoperative findings and anatomical variations can complicate surgical management. This study reviews a decade of experience at a single institution to evaluate the incidence, management, and outcomes of such rare findings. METHODS: A retrospective observational study was conducted at a tertiary pediatric surgery center in Egypt, analyzing all inguinal surgeries

Exposure of PreserFlo Microshunt: Risk Factors and Surgical Management.

Preserflo MicroShunt (PMS) has emerged as a safe and effective subconjunctival implant for lowering intraocular pressure in glaucoma. Although most postoperative complications occur early and are transient, long-term data remain limited-particularly regarding rare but serious events such as device exposure or extrusion, which may threaten ocular integrity. This report presents 3 clinical cases of PMS exposure with distinct presentations, potential risk factors, and surgical management approaches. In the first case, a pseudophakic patient with primary open angle glaucoma developed exposure of t

Approach to Cataract Surgery in a Case with Microcornea and Coloboma.

OBJECTIVES: Cataract surgery in patients with microcornea and ocular coloboma poses unique surgical and postoperative challenges. The presence of a small corneal diameter, abnormal ocular anatomy, and the risk of postoperative complications require a highly individualized approach. This case highlights the difficulties encountered in biometry measurement and the development of capsular contraction syndrome following phacoemulsification surgery. CASE REPORT: A 38-year-old male with bilateral ocular coloboma, microcornea, and nystagmus presented with progressive vision loss. Phacoemulsification

Systolic Anterior Motion (SAM) Complicating Mitral Valve Repair: Current Concepts of Intraoperative and Postoperative Management.

Mitral valve repair is the gold standard for treatment of degenerative mitral regurgitation, such as that caused by leaflet prolapse, flail or annular dilatation. A variety of surgical techniques allow surgeons to achieve a high rate of success with mitral valve repair, even in complex cases, and mitral valve repair is associated with better long-term outcomes than valve replacement. However, in the setting of mitral valve surgical repair, systolic anterior motion (SAM) is a complication that determines the dynamic anterior movement of the mitral valve towards the interventricular septum durin

How many people work in your operating room? An assessment of factors associated with instrument recounts within plastic surgery.

BACKGROUND: Intraoperative instrument recounts are performed to avoid retained foreign surgical items. These additional counts, however, beget risks of their own, including prolonged operative times, exposure to radiation, and increased cost. Our study aimed to identify factors that increase the likelihood of instrument recounts during plastic surgery procedures, and use our findings to guide potential solutions for preventing unnecessary recounts across all surgical fields. STUDY DESIGN: This is a retrospective review of all plastic surgical cases in the main operating setting at New York Un

Renal Pelvic Hematoma Following Robot-Assisted Pyeloplasty: Two Case Reports.

Renal pelvic hematoma is an uncommon but important complication after robot-assisted pyeloplasty (RAP), with few reports in the literature. Here, we report two cases of renal pelvic hematoma following RAP: A 9-year-old girl (Case 1) and a 25-year-old man (Case 2), both diagnosed with left ureteropelvic junction obstruction. Both underwent transperitoneal Anderson-Hynes pyeloplasty using 5-0 absorbable monofilament sutures. Postoperative imaging revealed renal pelvic hematoma in each case. Both patients remained hemodynamically stable and were managed conservatively. In Case 1, the hematoma and

Triadelphia pulvinata colonisation: First report in an Indian expatriate returning from the Middle East.

Triadelphia pulvinata is a dematiaceous mold, seldom implicated in human disease. We report the first case of lung cavity colonization by T. pulvinata in a 43-year-old Indian expatriate who worked as a construction laborer in Dubai. The patient presented with chronic breathlessness and progressive hemoptysis. Imaging revealed a right lower lobe cavity with an intracavitary lesion and bronchiectasis. The patient underwent successful wedge resection, resulting in complete symptom resolution. Histopathology of the intraoperative tissue identified fungal elements within organized cavity and cultur

An Atypical Presentation of a Large Congenital Umbilical Cord Hernia Antenatally Diagnosed as an Omphalocele With the Sole Left Lobe of the Liver: A Case Report.

We report an infrequent case of a full-term male neonate presenting with a large congenital hernia of the umbilical cord (6 cm × 6 cm) containing only the entire left lobe of the liver. Prenatal ultrasonography suggested an omphalocele; however, the definitive postnatal diagnosis was established based on the presence of a narrow rim of normal skin encircling the umbilical ring and the proximal umbilical cord. Primary closure of the fascial defect was achieved successfully, with no intraoperative or postoperative complications observed. The patient recovered well postoperatively and was dischar

An Ominous Case of Uterine Rupture in an Unscarred Womb and Its Subsequent Management.

Uterine rupture is a rare but critical obstetric complication that demands a swift and decisive intervention to ensure the well-being of the mother and fetus. We present a case report detailing the surgical management of a bizarre uterine rupture in a multigravida female with two previous vaginal deliveries and a previously unscarred uterus. This case highlights the challenges of treating and diagnosing, particularly in the Indian setting, an antenatally unregistered patient with rare obstetrical complications. Emphasizing the clinical challenges faced and the multidisciplinary approach employ

Local anaesthetic systemic toxicity complicating intraoperative intercostal nerve blocks: What do clinicians need to know to prevent similar occurrence?

Local anaesthetic systemic toxicity is a life-threatening adverse event that may occur after administration of local anaesthetics through a variety of routes. Local anaesthetic systemic toxicity is always a potential complication and may occur with all local anaesthetics and in any route of administration. Local anaesthetic systemic toxicity primarily affects the central nervous system and the cardiovascular system and may be fatal. The following is a case of local anaesthetic systemic toxicity complicating surgeon-performed intraoperative intercostal nerve blocks at multiple levels, with a mi

Distal rectal skip segment Hirschsprung disease: Case report and review of literature.

Background: Lack of ganglion cells on adequate suction rectal biopsy is the gold standard for diagnosis of neonatal Hirschsprung disease (HD), and the presence of ganglion cells precludes such a diagnosis. Case report: A 10-day old male neonate presented with clinical symptoms concerning for HD. However, suction rectal biopsies demonstrated submucosal ganglion cells on the distal suction rectal biopsies (2 cm from anal verge) and not on the proximal (3 cm from anal verge), with similar findings on repeat biopsies. Clinical suspicion remained high, and diagnostic laparoscopy with intraoperative

Duplicate appendix with acute ruptured appendicitis: a case report.

Duplication of the appendix is a rare congenital anomaly that, in adults, is most often found incidentally during surgery for other reasons. Appendicitis in the duplicated appendix is very rare and has been reported less than 10 times in the medical literature. We describe a 33-year-old woman with worsening periumbilical pain, nausea, vomiting, and fever. Physical examination showed localized peritonitis in the right lower quadrant. She had an elevated white blood cell count with neutrophilia. Computed tomography showed acute ruptured appendicitis. Diagnostic laparoscopy showed 2 appendices at

In vivo oocyte retrieval in a young woman with ovarian cancer.

BACKGROUND: Oocyte cryopreservation is rapidly becoming an option for single women recently diagnosed with cancer who wish to preserve their fertility. We describe oocyte retrieval in young women with invasive ovarian cancer. CASE: A 23-year-old nulliparous woman had recurrence of a mixed ovarian germ cell tumor after unilateral oophorectomy. Concern of involvement of her remaining ovary and need for postoperative chemotherapy prompted decision to perform controlled ovarian hyperstimulation followed by intraoperative in vivo oocyte retrieval. CONCLUSION: Oocyte cryopreservation through contr

Iatrogenic facial nerve injury: the role of facial nerve monitoring.

Intraoperative facial nerve monitoring has emerged as a powerful tool for facilitation of surgery involving the facial nerve. This tool must be properly applied and maintained, however, in order to avoid untoward results and to maximize its potential benefits. Facial nerve monitoring is best suited for prevention of iatrogenic injury. Once injury has become established, its value may be severely limited, especially for moderate to severe injury, in which visual assessment of injury is still the most effective means to determine the need for repair. The most valuable derivative of facial nerve

Feasibility of a 'reversed' isolated forearm technique by regional antagonization of rocuronium-induced neuromuscular block: a pilot study.

BACKGROUND: The isolated forearm technique is used to monitor intraoperative awareness. However, this technique cannot be applied to patients who must be kept deeply paralysed for >1h, because the tourniquet preventing the neuromuscular blocking agent from paralysing the forearm must be deflated from time to time. To overcome this problem, we tested the feasibility of a 'reversed' isolated forearm technique. METHODS: Patients received rocuronium 0.6 mg kg(-1) i.v. to achieve muscle paralysis. A tourniquet was then inflated around one upper arm to prevent further blood supply to the forearm. S…

Recognizing Posterior Accessory Muscles During Posterior Ankle Arthroscopy: A Case Report.

The flexor hallucis longus (FHL) tendon is a key anatomical landmark in posterior ankle arthroscopy and a commonly used graft for chronic Achilles tendon rupture. Peroneocalcaneus internus (PCI) is a rare accessory muscle that is frequently overlooked due to its close anatomical relationship to FHL and its asymptomatic presence. Failure to recognize this variation may result in tendon misidentification, ineffective reconstruction, or neurovascular injury. We report a 72-year-old woman with a chronic Achilles tendon rupture treated with endoscopic FHL transfer. Preoperative MRI demonstrated an

Unexpected coil mass migration during transvenous embolization of a dural arteriovenous fistula resolved with guidewire-assisted snaring technique.

Endovascular embolization is a standard treatment for dural arteriovenous fistulas (dAVFs). Although it is considered relatively safe, intraoperative and postoperative complications can occur. Herein, a rare case of unexpected coil mass migration requiring a retrieval procedure during sinus occlusion for a transverse-sigmoid sinus dAVF (TSdAVF) is described. An 83-year-old man presented with worsening decline in cognitive function. Magnetic resonance angiography showed a TSdAVF. Since his symptoms seemed to be a result of the TSdAVF, transvenous embolization preserving the normal cranial venou
