Novel 3-D helix-flexible nerve guide conduits repair nerve defects.

Flexible nerve guide conduits (NGCs) are desired in peripheral nerve reconstruction near the joints. Inspired by the engineered structure of the helical tube, we addressed the problems of conventional NGCs often mentioned in clinical feedback. We developed a type of helix-flexible NGC (HF-NGC) and evaluated its mechanical properties as well as its flexibility, and it performed excellently during in vitro tests. During the in vivo tests, HF-NGCs and conventional nonflexible NGCs (NF-NGCs) were implanted in a rat sciatic nerve defect model. A short-term investigation showed that Schwan cells (SC

Extracellular Vesicles in Peripheral Nerve Regeneration: From Biology to Therapeutic Engineering.

Peripheral nerve injuries (PNIs) pose a significant clinical challenge, often resulting in irreversible functional deficits due to limited spontaneous regeneration. While current therapeutic approaches offer partial solutions, their efficacy remains suboptimal. In recent years, extracellular vesicles (EVs) have emerged as bioactive carriers capable of orchestrating complex regenerative processes without the risks associated with live-cell transplantation. Derived from sources, EVs deliver a repertoire of functional cargos that modulate immune responses, promote axonal regrowth, enhance remyeli

An immunological study on chromogranin A and B in human endocrine and nervous tissues.

Antisera were raised against synthetic peptides derived from the primary amino acid sequence of human chromogranin B. These antisera recognized in one- and two-dimensional immunoblotting a component previously designated as chromogranin B. In human chromaffin granules, the major endogenous processing product of chromogranin B is formed by proteolytic cleavage of the protein near the C-terminus. Immunohistochemical localizations were obtained with antisera against human chromogranins A and B and against a synthetic peptide corresponding to the B sequence. In human tissues, chromogranin B is co-

Quantitative evaluation of rat sciatic nerve degeneration using high-frequency ultrasound.

In this study, we evaluated the utility of using high-frequency ultrasound to non-invasively track the degenerative process in a rat model of peripheral nerve injury. Primary analyses explored spatial and temporal changes in quantitative backscatter coefficient (BSC) spectrum-based outcomes and B-mode textural outcomes, using gray level co-occurrence matrices (GLCMs), during the progressive transition from acute to chronic injury. As secondary analyses, correlations among GLCM and BSC spectrum-based parameters were evaluated, and immunohistochemistry were used to suggest a structural basis for

Intense Focused Ultrasound Preferentially Stimulates Transected Nerves Within Residual Limbs: Pilot Study.

OBJECTIVE: Identifying pain generators in tissue deep in the skin can require uncomfortable, complicated, and invasive tests. We describe pilot studies testing the hypothesis that ultrasound image-guided, intense focused ultrasound (ig-iFU) can noninvasively and differentially stimulate the end of transected nerves in the residual limbs of amputee patients. DESIGN: We applied iFU to the transected nerve ending as individual pulses with a length of 0.1 seconds using a carrier frequency of 2.0 MHz. After targeting, we gradually increased the iFU intensity to reach consistent patient-reported st

Collagen nerve protector in rat sciatic nerve repair: A morphometric and histological analysis.

Peripheral nerve repair is often complicated by fibroblastic scar formation, nerve dysfunction, and traumatic neuroma formation. Use of bio-absorbable protective wraps may improve outcomes of these repairs. This study histologically compared the incidence of neuroma formation, connective tissue proliferation, and axonal regrowth in transected rat sciatic nerves repaired with and without tubular collagen nerve sleeves. Twenty Sprague-Dawley rats underwent unilateral sharp sciatic nerve transection and microscopic nerve repair with four epineural sutures and were randomly treated with or without

Another (Internal) Epineurium: Beyond the Anatomical Barriers of Nerves.

The epineurium has been accepted as the outer anatomical barrier of the peripheral nerves. Our objective was to characterize the microanatomy of the layers surrounding nerves using different tissue-specific staining methods. Two hundred forty-two cross sections of human sciatic and median nerves, and brachial plexuses of eight fresh unembalmed cadavers, were examined. The samples were fixed in formaldehyde solution and stained with hematoxylin-eosin, Masson's trichrome, or epithelial membrane antigen under standard conditions. Because epithelial membrane antigen only stains the perineurium, we

Sensory reanimation of the hand by transfer of the superficial branch of the radial nerve to the median and ulnar nerve.

BACKGROUND: It remains a surgical challenge to treat high-grade nerve injuries of the upper extremity. Extra-anatomic reconstructions through the transfer of peripheral nerves have gained clinical importance over the past decades. This contribution outlines the anatomic and histomorphometric basis for the transfer of the superficial branch of the radial nerve (SBRN) to the median nerve (MN) and the superficial branch of the ulnar nerve (SBUN). METHODS: The SBRN, MN, and SBUN were identified in 15 specimens and the nerve transfer performed. A favorable site for coaptation was chosen and its lo

Dual Functional Electroconductive Biofortified Electrospun Scaffold Functionalized With MWCNTs and Bacopa Monnieri for Accelerated Peripheral Nerve Regeneration.

Peripheral nerve injuries (PNIs) often lead to semi or complete loss of motor, sensory and autonomic functions. Although autografts are still the best option for PNI repair, their use is restricted due to the morbidity and availability of donor nerves. Because electrospun scaffolds may replicate the structure of native extracellular matrix (ECM), they provide a viable alternative. The present study represents a dual-functional scaffold that combines the neuroprotective and antioxidant capabilities of Bacopa monnieri extract with the mechanical and electrical characteristics of polycaprolactone

Peripheral Nerve Regeneration: Current Status and New Strategies Using Polymeric Materials.

Experiments concerning peripheral nerve regeneration have been reported since the end of the 19th century. The need to implement an effective surgical procedure in terms of functional recovery has resulted in the appearance of several approaches to solve this problem. Nerve autograft was the first approach studied and is still considered the gold standard. Since autografts require donor harvesting, other strategies involving the use of natural materials have also been studied. Nevertheless, the results were not very encouraging and attention has moved towards the use of nerve conduits made fro

Quantitative analysis of the anatomy of the epineurium of the canine recurrent laryngeal nerve.

The purpose of this investigation was to determine the amount of epineurium surrounding the recurrent laryngeal nerve (RLN) compared with a limb nerve, that to flexor hallicus longus (NFHL). Nerve samples were obtained from 10 adult dogs and studied using scanning electron microscopy and light microscopy to measure the relative proportion of epineurium and the relative proportions of adipose and collagenous tissue comprising the epineurium in both nerves. Significantly greater relative epineurial cross-sectional areas and adipose content were found in the RLN than in the NFHL. Based on observa

Engineered aligned endothelial cell structures in tethered collagen hydrogels promote peripheral nerve regeneration.

Vascularisation is important in nerve tissue engineering to provide blood supply and nutrients for long-term survival of implanted cells. Furthermore, blood vessels in regenerating nerves have been shown to serve as tracks for Schwann cells to migrate along and thus form Bands of Büngner which promote axonal regeneration. In this study, we have developed tissue-engineered constructs containing aligned endothelial cells, or co-cultures of both endothelial cells and Schwann cells to test whether these structures could promote regeneration across peripheral nerve gaps. Type I rat tail collagen ge…

Preserved fascicular architecture predicts neuroma pain: a morphometric study.

Painful neuromas remain a major clinical challenge after limb amputation and peripheral nerve trauma. While histological features such as inflammation, fibrosis, and axonal sprouting have been proposed as contributors to neuropathic pain, direct clinicopathological correlations remain inconsistent. The role of internal nerve architecture, particularly the proportion of preserved, organized fascicular tissue, has not been quantitatively assessed in relation to pain intensity. To address this gap, this study investigates whether the relative amount of organized versus unorganized nervous tissue

Adipose Derived Stem Cells Reduce Fibrosis and Promote Nerve Regeneration in Rats.

Peripheral nerve regeneration is critical and challenging in the adult humans. High level of collagen infiltration (i.e., scar tissue), in the niche of injury, impedes axonal regeneration and path finding. Unfortunately, studies focusing on the modulation of scar tissue in the nerves are scarce. To address part of this problem, we have evaluated the differentiated adipose derived stem cells (dASCs) for their antifibrotic and regenerative effects in a 10 mm nerve gap model in rats. Three different animal groups (N = 5) were treated with fibrin nerve conduits (empty), or seeded with dASCs (F + d
