We describe the histopathologic features of spinal cord lesions in 10 cases of progressive supranuclear palsy (PSP) and review the literature. Histologic examination revealed atrophy with myelin pallor in the anterior funiculus and anterolateral funiculus in the cervical and thoracic segments in eight of the 10 cases, whereas the posterior funiculus was well preserved. The degrees of atrophy of the anterior funiculus and the anterolateral funiculus correlated with that of the tegmentum of the medulla oblongata. Serious intra-abdominal injuries in neonates are very rare. In addition, the signs and symptoms of hemoperitoneum caused by bleeding from solid viscera are vague and nonspecific and often are not recognized before the onset of hypovolemic shock or death. In this report, we describe a 2-day-old infant who presented with shock and pallor who had a ruptured spleen, presumably from birth. We also review the literature and the importance of recognizing this injury in the emergency department setting.

In the past, little attention has been given in the imaging literature to spinal cord findings in the human immunodeficiency syndrome. This article summarizes the major pathologic conditions found in the spinal cord in patients with AIDS and compares them to MR features. The details of toxoplasma myelitis, spinal cord lymphoma, HIV myelitis, and tract pallor are demonstrated. A review of literature and a description of the pathologic cord changes will help the reader understand the alterations of the spinal cord in patients with AIDS.

A patient with chronic metal intoxication is described, presenting during four years after the cessation of her exposure to industrial substances, maculo-papular eruptions with several ulcerated lesions and excoriations on her abdomen and buttocks. She also had pallor of her face, greyish-dark discoloration of the hair, while the fingernails were brittle and sensitive. Scrupulous physical examination excluded further cutaneous involvement. The immunological workup revealed both phenotypic and functional defects in cellular immunity.

Cutaneous manifestations of chronic renal failure (CRF) were studied in 20 cases. Generalised pruritus was observed in 8 patients, ichthyotic skin changes of varying degree in 18 patients and hyperpigmentation of sun-exposed skin in 7 patients. Scaling, pallor and hair loss correlated strongly with the severity of CRF. Skin biopsy performed in 5 cases revealed mild hyperkeratosis, reduction of prickle cell layer and flattening of rate ridges. No significant dermal changes were detected.

This case depicts an unusual presentation of septo-optic dysplasia. A four-year-old female presented with monocular nystagmus and temporal optic disc pallor in her left eye. Despite a normal sized optic nerve head, magnetic resonance imaging (MRI) showed a hypoplastic intraorbital and intracranial left optic nerve in the absence of a septum pellucidum. She was subsequently diagnosed with septo-optic dysplasia with sectoral optic nerve head hypoplasia.

A patient undergoing left mastectomy and immediate latissimus dorsi breast reconstruction under combined paravertebral block and general anaesthesia developed transient, well-demarcated, right-sided hemifacial erythema and sweating, and left-sided Horner syndrome postoperatively. This "harlequin" appearance occurs because of a normal or excessive vasodilatory, thermoregulatory response to heat or emotion mediated by an intact sympathetic pathway on the erythematous side, together with relative pallor of the pharmacologically blocked side.

Coombs' positive hemolytic anemia is exceedingly rare in tuberculosis. We herein report a patient with tuberculosis associated with Coombs' positive hemolytic anemia that was responded to antituberculosis therapy. She was admitted to the hospital because of recent-onset fatigue, weakness, nonproductive cough, pallor and scleral jaundice. Coombs positive hemolytic anemia and pulmoner tuberculosis was diagnosed. Following antituberculosis therapy, laboratory and clinical finding related to autoimmune hemolytic anemia disappeared.

OBJECTIVE: To determine whether characteristics of optic nerve swelling assist in distinguishing between optic neuritis and anterior ischemic optic neuropathy.
METHOD: Optic nerve stereophotograph review by masked observers.
RESULTS: Altitudinal swelling, pallor, arterial attenuation, and hemorrhage are found more commonly in anterior ischemic optic neuropathy than in optic neuritis.
CONCLUSION: Optic disc appearance does help to distinguish anterior ischemic optic neuropathy from optic neuritis, although there are overlapping features.

UNLABELLED: We describe a 17-month-old child with dumping syndrome after plication of the right diaphragm. He presented with periods of abdominal distension and pallor, recurrent convulsions, glucosuria and refusal of feeding. After changing the diet the symptoms disappeared.
CONCLUSION: Although dumping syndrome in children is rare, early recognition is important. Serial determination of blood glucose after bolus feeding can lead to the diagnosis. Treatment should consist of dietary changes.

A 13 year-old boy was hospitalized because of persisted fever had pallor for three months. Abdominal ultrasound, intravenous pyelogram, and abdominal computerized axial tomography scan disclosed a space occupying lesion located postero-superior to the bladder. Peripheral arterial occlusive disease predominantly involves the lower extremities. The greatest incidence occurs in males over 50 years of age. Manifestations include intermittent claudication, diminished or absent pulsations, pallor, and trophic changes. Patients are particularly vulnerable to infection, ulcer formation, and gangrene. The treatment program aims to promote circulation, and to prevent tissue damage and complications. Some patients have reconstructive surgery, which includes prosthetic grafts as well as bypass procedures.

A 12-year-old girl was brought to the Dhaka Hospital of ICDDR,B with diarrhoea. Incidentally, the parents provided a history of repeated episodes of pallor and jaundice since she was two and half years old. Three of her family members had similar problems. History, clinical examination, and laboratory findings of the girl and her family members suggested a case of hereditary spherocytosis. To our knowledge, this is the first report of such a case in Bangladesh.
