[IgG4-related systemic sclerosing disease: a review].

IgG4-related systemic sclerosing disease (SSD) is a multisystemic condition characterized by an increased number of IgG4-producing plasma cells which occurs mainly in older men. SSD involves particularly pancreas, hepatobiliary system, salivary glands and retroperitoneum. Microscopically, the findings include lymphoplasmacytic inflammation, fibrosis and vascular changes in the form of obliterative phlebitis. Using immunohistochemistry, an increased number of IgG4-positive plasma cells might be detected in affected tissues. Since SSD frequently mimicks a malignancy both clinically and radiologi…

Pulmonary veno-occlusive disease. Morphological changes suggesting a viral cause.

A 47-year-old man with a history of industrial exposure and interstitial lung disease was admitted for acute pulmonary decompensation. Clinical course was characterized by severe dyspnea at rest, fever, hypoxemia, and elevated pulmonary arterial pressures. At autopsy, pulmonary problems were explained by a selective veno-occlusive process. Associated with pulmonary phlebitis was cerebral vasculitis and lymph node enlargement with erythrophagocytosis suggesting underlying viral infection. Pulmonary veno-occlusive disease should be considered in cases of pulmonary fibrosis, pulmonary hypertensio…

Safety of peripheral intravenous administration of hypertonic saline: a systematic review and meta-analysis.

BACKGROUND: Hypertonic saline solution is critical for managing symptomatic hyponatremia and increased intracranial pressure (IICP). Though administered via central venous catheters (CVCs) traditionally, peripheral administration is a viable alternative, reducing delays and CVC-associated risks. This review evaluates the safety of peripheral hypertonic saline, focusing on infusion-related adverse events. METHODS: A systematic search of MEDLINE, Embase, and Cochrane Library (up to December 2024) identified studies on peripheral hypertonic saline in adults. Studies reporting phlebitis, infiltra…

Ultrasound assessment of short peripheral catheter failure.

INTRODUCTION: Short peripheral catheters (SPCs) are affected by a high complication rate that leads to catheter failure. Currently, the Visual Infusion Phlebitis score (VIP) is the most used tool to verify the presence of inflammatory complications (phlebitis and thrombophlebitis). However, ultrasound signs (US) may be an attractive alternative. OBJECTIVE: This study aims to evaluate the sensitivity and specificity of US and VIP score = 1 in identifying and recognizing early signs of SPC failure. The time to positivity for US and VIP scores was assessed as a secondary outcome. METHODS: An ob…

[Superficial femoral vein ligation and distal venous thrombectomy as surgical therapy for recent thrombophlebitis of the lower extremities. A clinical study (author's transl)].

Three hundred and eighty-three patients underwent superficial femoral vein ligation and distal venous thrombectomy in the treatment of deep venous thrombosis of the lower extremities. Two hundred and forty-eight patients were reviewed clinically. The results are quite satisfactory; no surgical mortality and no fatal pulmonary embolism. On long term follow-up the clinical symptoms such as pain, swelling, varicosities and ulceration are much less marked in the operated group when compared with the post-phlebitis syndrome which develops when the deep venous thrombosis is allowed to extend into th…

The enigmatic immunoglobulin G4-related disease and its varied cardiovascular manifestations.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic fibroinflammatory disease characterised by multiorgan lymphoplasmacytic infiltration, obliterative phlebitis and storiform fibrosis. It can be associated with cardiovascular pathology. The objective of this narrative review is to summarise the published literature on cardiovascular manifestations of IgG4-RD and to provide a basis for diagnosis and management of the condition by the practising cardiologist. We propose the following categorisations of cardiovascular IgG4-RD: aortitis, medium-vessel arteritis, pulmonary vascular disease, ph…

Fibrous thyroiditis--an immunopathological study.

Riedel's thyroiditis is a rare disease which has in the past been confused with the much commoner fibrous variant of Hashimoto's thyroiditis. We have compared the histological and immunohistochemical characteristics of three cases diagnosed as Riedel's thyroiditis with five cases of the fibrous variant of Hashimoto's thyroiditis. The major distinguishing features on light microscopic study were that Riedel's thyroiditis showed (a) extra-thyroid extension of the fibrous tissue, (b) a phlebitis with distension of the vein lumen by lymphoid or fibrous tissue, and (c) relatively normal surviving t…

[Injury of femoral artery complicated with infection from injection of heroine].

Drug addiction has been one of the serious social problems. The peripheral phlebitis caused by drug injection is common, but the occurrence of pseudoaneurysm with infection of femoral artery from injection injury was rarely reported in China. From January 1995 to March 1996, six cases of injury of femoral artery with infection from heroine injection were admitted. The characteristics of the injury were described. The therapeutic measures and details of attention to be needed were discussed. It was concluded that this type of injury was different from the injury caused in agricultural, industri…

Involvement of subcutaneous veins in lepromatous leprosy.

Venous involvement in 31 patients with lepromatous leprosy has been studied in biopsies from clinically involved and clinically normal subcutaneous veins from the forearm. Twenty-nine of these showed histological evidence of leprous phlebitis. The earliest lesion was intimal cell hyperplasia with the presence of acid-fast bacilli in small groups in the intimal cells. This gradually progressed to total occlusion of the vein by lepromatous exudate. The results indicate much greater involvement of veins and possibly other components of the vascular system in patients with lepromatous leprosy than…

Ultrasonography of the extremities and pelvic girdle and correlation with computed tomography.

Ultrasonography was performed on 54 patients with lesions of the extremities and pelvic girdle. Computed tomography (CT) was employed in 8 patients. Focal space-occupying lesions such as tumors, abscesses, hematomas, aneurysms, and popliteal cysts (dissecting or otherwise) were delineated by ultrasound and their extent defined. Differentiation from diffuse changes such as lymphedema, cellulitis, or phlebitis and diagnosis of aneurysms were possible. Popliteal cysts were usually quite characteristic, while correlation with the clinical features suggested the correct diagnosis in the case of the…

Autoimmune pancreatitis.

Autoimmune pancreatitis (AIP) is a benign, IgG4-related, fibroinflammatory form of chronic pancreatitis that can mimic pancreatic ductal adenocarcinoma both clinically and radiographically. Laboratory studies typically demonstrate elevated serum IgG4 levels and imaging studies reveal a diffusely or focally enlarged pancreas with associated diffuse or focal narrowing of the pancreatic duct. The pathologic features include periductal lymphoplasmacytic inflammation, obliterative phlebitis, and abundant IgG4-positive plasma cells. The treatment of choice for AIP is steroid therapy. Diagnostic crit…
