The axillary pouch portal: a new posterior portal for visualization and instrumentation in the inferior glenohumeral recess.

Arthroscopic access to the inferior glenohumeral recess is necessary in several surgical procedures on the shoulder. Posteroinferior portals described for access to this region may pose a theoretic risk to the posterior neurovascular structures (outside-in technique) and to the articular cartilage (inside-out technique). The first author (D.N.B.) has devised a new posterior portal that permits direct linear access to the entire inferior glenohumeral recess. The portal is placed higher and more lateral compared with the previously described portals; this places it further away from the posterio

Posterior Labrum Repair: Two-Portal Technique.

BACKGROUND: Posterior labral injuries can cause shoulder pain and instability. Posterior labral tears account for up to 47% of all tears treated arthroscopically. Injuries can result from repetitive microtrauma, recurrent posterior subluxations, or traumatic posterior dislocations. Posterior labral injuries can be reliably repaired with arthroscopic surgery. A systematic review by Delong et al found improved return to play along with lower recurrence rates when repairing these injuries using an arthroscopic approach compared to an open approach. INDICATIONS: The main indication includes a sym

The throwing shoulder.

The shoulder is affected by numerous pathological conditions related to the high speeds and intra-articular stresses during the throwing movement. The most common are SLAP lesions and internal impingement. The treatment is preventive and conservative initially, starting in athletes at a young age and continuing throughout their entire sports career. It includes posterior capsule stretching, strengthening of the external rotators and scapular stabilizers, correcting any scapular dyskinesia and optimizing the static and dynamic balance of the thorax, pelvis and legs. Surgical treatment is consid

Posterior capsule opacification: a review of the aetiopathogenesis, experimental and clinical studies and factors for prevention.

Posterior capsule opacification (PCO, secondary cataract, after cataract) is a nagging postsurgical complication following extracapsular cat … PCO. We strongly believe that the overall incidence of PCO and hence the incidence of Nd:YAG laser posterior capsulotomy is now rapidly decreasing - from 50% in the 1980s and early 1990s to less than 10% currently. Superior tools, surgical procedures, skills and appropriate IOL designs have all helped to significantly reduce this complication. In this article, we revi …

Toric intraocular lenses: Expanding indications and preoperative and surgical considerations to improve outcomes.

Since the introduction of the first toric intraocular lens (IOLs) in the early 1990s, these lenses have become the preferred choice for surgeons across the globe to correct corneal astigmatism during cataract surgery. These lenses allow patients to enjoy distortion-free distance vision with excellent outcomes. They also have their own set of challenges. Inappropriate keratometry measurement, underestimating the posterior corneal astigmatism, intraoperative IOL misalignment, postoperative rotation of these lenses, and IOL decentration after YAG-laser capsulotomy may result in residual cylindric

In vitro biomechanics of the craniocervical junction-a sequential sectioning of its stabilizing structures.

BACKGROUND CONTEXT: Occipitocervical dislocations involve translations of the craniocervical joints. The relative contributions of each ligament to overall stability and the effects of the occipitoatlantal joint capsules on the pathologic translation are unknown. Although incidences of occipitocervical dislocations are rare after blunt trauma, they are usually fatal. When patients do survive these dislocations, the proper diagnosis is difficult, which in turn may increase the fatality rate. A biomechanical model may provide a greater pathologic understanding of craniocervical subluxation. PUR

[Secondary cataract and IOL type and the shape of its optic part].

Although cortical remains are purged from the capsule sac by special techniques and better IOL models are used for implantation, opacification of the posterior capsule (OPC) persists to be one of the most frequent complications in modern cataract surgery. The frequency rate of OPC depends on used evaluation criteria. A total of 165 eyes were examined after cataract extraction for OPC. Four types of IOL were used; they are hydrophobic acryl three-part IOL (AcrySof MAZOVA, n = 46), hydrophobic acryl all-in-one IOL (ArySofSA30AL, n = 38) and silicone three-part (CeeOn911A, Pharmacia, n = 42). Suction Break During Femtosecond Laser-Assisted Cataract Surgery and Misplaced Laser Beam Delivery to the Corneal Layers.

A 68-year-old man with senile cataract underwent femtosecond laser-assisted cataract surgery (FLACS) in his left eye. Only anterior capsulotomy and lens fragmentation were planned with a femtosecond laser. Docking of the patient interface and anterior capsulotomy were completed without any complications. During the lens fragmentation process, the patient could not resist the temptation to squeeze his eyes shut, which caused excessive pressure from the eyelids. As the procedure proceeded, a bubble was formed at the edge of the patient interface and became increasingly larger. In addition, wrink
