[A child of microscopic polyarteritis nodosa effectively treated with intravenous methylprednisolone pulses and serial cyclophosphamide pulse therapy].

Microscopic polyarteritis nodosa (mPAN) is a rare disorder in pediatric field of systemic small vessel vasculitis, and affects skin and musculoskeletal system accompanied by progressive necrotizing glomerulonephritis. We described here a 15-year-old boy with positive anti-neutrophil cytoplasmic antibody (MPO-ANCA) and severe mPAN, who was effectively treated with intravenous methylprednisolone pulse therapy followed by monthly cyclophosphamide pulses for 1 year. The histologic examination of renal biopsy specimen showed a severely desolated disease characterized by fibrinoid necrosis, crescent…

Efficacy and limitations of additional steroid pulse therapy in IgA nephropathy patients whose hematuria did not remit on tonsillectomy and protocol steroid pulse therapy.

BACKGROUND: Hematuria is the essential symptom of IgA nephropathy that has been suggested to be associated with long-term renal prognosis, Tonsillectomy and steroid pulse therapy (TSP), which is widely practiced in Japan, is effective for achieving hematuria remission. However, some cases are refractory to TSP, and additional steroid pulse therapy (SP) administered to these cases to achieve remission of hematuria. Nonetheless, the clinical significance of additional SP is unknown. METHODS: In this retrospective study, we enrolled 99 patients from Okubo Hospital whose hematuria persisted follo…

Pulse volume recordings in outer ear canal in pulse synchronous tinnitus. A comparison between ears with Glomus tumour, serous otitis media, and normal ears.

With the aid of a volume flowmeter it is possible to record pulse synchromous volumetric changes in the outer ear canal. In 7 ears with glomus tumour in the tympanic cavity and in 5 with serous otitis media, such changes were larger than in 125 persons with a normal middle ear. By changing the ambient pressure in a pressure chamber and instructing the patients not to swallow, the drum can be pushed inward or outward. In all the cases of glomus tumour studied the pulse volumetric change was considerably affected when the drum was pushed inward or outward. In the normal patients the change was n…

Methods for Wearable-Derived Pulse Rate Measures and Their Application to Modeling the Relationship between Pulse Rate and Motor Activity in Narcolepsy Type 1.

INTRODUCTION: Narcolepsy type 1 (NT1) is a chronic, rare neurological disorder caused by loss of orexin neurons that disrupts the sleep-wake … omized, phase 2 clinical trial (NCT05687903) from participants with NT1. Epoch-level (minute-level) pulse rate data were rigorously screened for artifacts. Daily pulse rate measures (24-h, daytime, and nighttime averages) were derived. Numerical experiments were conducted to evaluate the stability of daily measures as a function of the proportion of val …

[Reproducibility of pulse oximetry in the fingers of the hands].

BACKGROUND: Partial oxygen saturation from pulse oximetry is most often obtained from the fingers of the hands; however, it is not known whether any of the fingers are ideal for estimating partial oxygen saturation. OBJECTIVE: To describe the partial oxygen saturation behaviour obtained from the fingers of both hands. MATERIAL AND METHODS: Using a cross-sectional analytical design, the partial oxygen saturation was consecutively estimated in the fingers of both hands. Demographic data were obtained from healthy subjects, and they were analyzed using repeated-measures analysis of variance; re…

Pulsed Field Ablation in Ventricular Arrhythmias.

Pulsed field ablation (PFA) is a novel nonthermal energy source for catheter ablation, which is potentially useful also for ablation of ventricular arrhythmias since it mitigates the risk of collateral damage to adjacent tissues and may better penetrate through the scar tissue. Solid-tip PFA within the great cardiac vein appears to be advantageous for arrhythmias originating from the left ventricular summit. The use of a large-footprint catheter that toggles between radiofrequency current and PFA may provide an additional advantage since it allows the development of larger lesions to modify my…

Anaesthesia for Pulsed Field Ablation Procedures.

Pulsed field ablation (PFA) is a novel, non-thermal technology used in catheter ablation procedures. Despite its growing clinical adoption, a standardised sedation protocol has not yet been established. Well-coordinated anaesthesia management is essential for procedural success, patient safety and comfort. This review examines various sedation strategies, highlighting their respective benefits and risks, and offers practical recommendations to optimise anaesthesia management during PFA procedures. PFA enables selective myocardial ablation while preserving surrounding structures such as the oes…

Pulsed ventricular tachycardia: a case study.

Ventricular tachycardia (VT) is an arrhythmia that originates from the ventricles of the heart and presents as a wide and prolonged QRS complex on the electrocardiograph of greater than 120 milliseconds, with a heart rate of over 100 beats per minute. VT can occur as a pulsed or pulseless rhythm. Pulseless VT occurs when the ventricles cannot effectively pump blood out of the heart, therefore resulting in no cardiac output. Pulsed VT can manifest with the patient presenting asymptomatically, or with symptoms of reduced cardiac output resulting from poor ventricular filling. There is the potent…

Pulsed Radiofrequency for the Treatment of Trigeminal Neuralgia.

PURPOSE OF REVIEW: Trigeminal neuralgia is a sudden, unilateral, stabbing pain in the distribution of one or more branches of the fifth cranial nerve, with an overall prevalence ranging between 0.03 and 0.3%. While conservative treatments may offer temporary relief, many patients experience chronic headaches associated with their neuralgia. Invasive treatments are available for patients with intractable neuralgia; however, they may cause permanent tissue damage and often do not provide relief. This article examines pulsed radiofrequency (PRF) ablation (RFA) of the trigeminal nerve as a minimal…

Intense pulsed light (IPL): a review.

BACKGROUND: Intense pulsed light (IPL) devices use flashlamps and bandpass filters to emit polychromatic incoherent high-intensity pulsed light of determined wavelength spectrum, fluence, and pulse duration. Similar to lasers, the basic principle of IPL devices is a more or less selective thermal damage of the target. The combination of prescribed wavelengths, fluences, pulse durations, and pulse intervals facilitates the treatment of a wide spectrum of skin conditions. OBJECTIVE: To summarize the physics of IPL, to provide guidance for the practical use of IPL devices, and to discuss the cur…

["Diastolic" heart failure and pulsed pressure].

Cardiac failure is the leading cause of hospital admission after 65 years of age. Several studies have confirmed the frequency of cardiac failure with normal systolic function ("diastolic" cardiac failure) in the elderly (nearly half the cases). The cause is commonly isolated systolic hypertension. The pulsed pressure depends on ventricular ejection, arterial rigidity and the precocity of reflected pulse waves. In the elderly, the pulse pressure is a powerful predictive factor for mortality and adverse cardiovascular events (acute coronary syndromes, cardiac failure and cerebrovascular acciden…

The effect of pulsed ultrasound on mandibular distraction.

This study evaluated the effect of pulsed ultrasound on tissue repair and bone growth during mandibular osteodistraction. Twenty-one rabbits were divided into three groups of 7. The distraction started 72 h after surgically severing both sides of the mandible and proceeded at a rate of 1.5 mm/12 h for 5 days. Group I received pulsed ultrasound (nominally 200 micros pulse of 1.5 MHz at a 1.1 kHz pulse repetition frequency, 30 mW/cm2) for 20 min on both sides of the mandible every other day (alternating sides). Group 2 received the same pulsed ultrasound treatment on one side of the mandible eve…

Developmental exposure of mice to pulsed ultrasound.

Exposure of pregnant mice on gestation day (gd) 8 to 1 MHz continuous-wave ultrasound (0, 0.05, 0.50, or 1.00 W/cm2) was reported previously to result in a slight (nonsignificant) increase in malformations. The present study was conducted in a similar fashion using pulsed ultrasound but was designed to maximize the likelihood of finding effects of gd 8 ultrasound exposure on prenatal development. Pregnant ICR mice (approximately 60 animals/group) were exposed on gd 8 to pulsed ultrasound with a center frequency of 1 MHz at levels of 0 (sham control), 0.05, 0.50, or 1.00 W/cm2 (spatial average,…
