Breathing Through a Straw: A Case of Adult Croup at High Altitude.

Croup in adults is a rare but potentially life-threatening condition. We describe a case of croup in a 40-y-old female climber, likely precipitated by exposure to cold, dry air at high altitude. To our knowledge, this is the first documented case of croup in an adult associated with exposure to high altitude. This case highlights the importance of early recognition and timely intervention in managing this uncommon presentation in an adult of a common childhood condition.

Icy Hot: A Case of Unexpected Heat Illness.

Finish-line collapse is a unique phenomenon among athletes and includes a broad working differential. We explore the case of a young, healthy runner who collapsed at the end of a half marathon in the fall from exertional heat illness. This illness covers a wide range of severity from the mild symptoms of heat cramps to heat exhaustion and the extreme of life-threatening heat stroke. Development of these conditions is often associated with patient-specific acquired or congenital vulnerability factors such as extremes of age, fitness, obesity, medication use (eg, antipsychotics, anticholinergics

Paraquat Poisoning from a Tertiary Care Center in Northern India: A Case Series.

Paraquat, a nonselective herbicide introduced in the 1960s, remains widely used in agricultural settings in many low- and middle-income countries due to its low cost and effectiveness-despite being classified as a restricted-use pesticide in many parts of the world. Its easy availability contributes to frequent intentional ingestion. Its highly toxic nature, especially when ingested, poses a significant public health challenge, particularly in rural India, where regulatory oversight is limited and public awareness is low. We report a case series of 20 patients with paraquat poisoning who prese

Physiologic, Psychological, Nutritional, and Rest-Activity Observations During 120 Days of Low-Tech Off-Grid Living in the Baja California Desert: A 2-Participant Field Case Study.

Off-grid low-tech living is increasingly proposed for arid environments, yet longitudinal field data documenting health, nutrition, and rest-activity organization under real-world conditions remain scarce. Our objective was to describe physiologic, nutritional, psychological, and rest-activity trajectories during 120 d of low-tech off-grid desert living. This was a prospective observational case study of 2 participants (female, 29 y old; male, 39 y old) conducted in Baja California (January-April 2023). Pre-, mid-, and post-mission assessments included bioimpedance body composition, incrementa

Blister Eradication Looking at Impact of Experimental Versus Established Regimens (BELIEVER).

IntroductionFriction foot blisters are one of the most common injuries encountered by hikers and runners. In expedition length ultramarathons, blisters account for nearly 74% of medical visits. While most blisters are of minor medical significance, they can impair concentration, decrease athletic performance, and potentially become debilitating. In this study, we compared two materials for blister treatment, Elastikon and RockTape. We postulate RockTape may be non-inferior to Elastikon and could have additional benefits in cost and utility.Methods91 participants (62 M, 29 F) were enrolled whil

A Bite by a Beaded Lizard, Heloderma spp. (Helodermatidae), Resulting in Local Envenomation and Boutonnière Deformity.

Beaded lizards (Heloderma spp) are venomous lizards capable of inflicting local and systemic envenoming. An exotic pet collector presented with a reportedly prolonged bite by a presumed Guatemalan beaded lizard (Heloderma charlesbogerti). He reported immediate symptoms of envenoming, including severe pain, paresthesia, local swelling, dizziness, and nausea. Follow-up assessments conducted over a period of up to 4 y revealed a persistent boutonnière deformity of the right middle finger. This case report describes envenomation caused by the uncommonly documented bite of a beaded lizard (Heloderm

Wild Felid Attacks on Humans in South and Central America: A 2014-2025 Case Series in French Guiana and Literature Review.

Wild felids are apex predators that play a crucial role in tropical ecosystems. In South America, interactions between humans and large felids-particularly jaguars (Panthera onca) and pumas (Puma concolor)-are increasingly reported, raising public concern. However, attacks on humans remain poorly documented, and their frequency is likely overestimated. This study assessed the scale of wild felid attacks in French Guiana and South America through a case series and literature review. We conducted a retrospective observational study of all wild felid attacks managed in healthcare facilities in Fr

Throw What? Evaluation of Different Rescue Tools Used by Middle-Aged and Older Bystanders During a Drowning Incident: A Pilot Study.

BackgroundLate middle-aged and older adults face a heightened risk of drowning due to declines in physical capacity and comorbidities. They often act as caregivers for grandchildren or individuals of a similar age, which may affect their drowning risk and ability to perform safe rescues. The "reach, row, throw, don't go" paradigm remains central to bystander rescue strategies. Identifying effective aquatic rescue devices for laypersons is a key unresolved issue.MethosThis pilot study evaluated the effectiveness of several aquatic rescue devices-specifically a ring buoy, a rescue buoy, and a pl

Post-Shower Cardiovascular and Oxygenation Responses During an Antarctic Inland Traverse: A Single-Participant Repeated-Measures Study.

IntroductionPolar field hygiene may require transitions between severe cold, warm-water exposure, and sheltered recovery. Physiological responses to this sequence are poorly described.MethodsA healthy 35-year-old man completed 13 approximately 5-min showers using approximately 10 L of water at 40 °C late in a 78-day inland Antarctic traverse. Systolic and diastolic blood pressure, heart rate, and peripheral oxygen saturation (SpO2) were measured after 20 min of seated rest before showering, at the first post-shower measurement obtained within 5 min of shower completion, and every 5 min thereaf

Injury Patterns in Water Sports on the Levantine Coast: A Single-Center Study.

IntroductionThe Levantine coast is characterized by environmental and demographic factors-microtidal regime, shifting sandbars, and crowded beaches. These may shape water-sport injury patterns, yet region-specific data are lacking. We described the types, anatomical sites, and sport-specific distribution of injuries along the Levantine coast.MethodsWe conducted a prospective observational study at a single Levantine coastal emergency department. Injuries from surfing, kitesurfing, windsurfing/wing-foiling, swimming/freediving, kayaking, and motorized boards were recorded in real time, includin

Acceptance of Risk and Confidence Assessing Avalanche Terrain and Conditions: A Large Cross-Sectional Study.

IntroductionThe COVID-19 pandemic affected the ski industry globally, including limiting access to ski resorts and prompting more skiers and snowboarders to explore the backcountry. In this study, we quantified the willingness to take risks (risk propensity) and self-perceived ability to assess hazards in the backcountry and to explore correlations between these factors.MethodsWe based our study on a previously reported data set gathered under the supervision of our senior author, who collected anonymous responses to a 29-question online survey completed by 4792 self-identified backcountry ski

Potassium Chlorate at High Altitude: Lost to History.

The use of potassium chlorate and other chlorates from the nineteenth century until the present is reviewed with an emphasis on use at high altitude. Chlorates that undergo high-temperature thermal decomposition are a source of supplemental oxygen for inhalation. The oral uses of potassium chlorate on mountaineering expeditions have ranged from simple lozenges for pharyngitis to the erroneous idea that oral ingestion could provide supplemental oxygen and prevent or treat mountain sickness. Furthermore, today's aircraft and spacecraft still employ chlorates and perchlorates to produce emergency

B-line Prevalence Is Higher with Increases in Altitude but Does Not Correlate with AMS Symptomatology.

IntroductionPulmonary B-lines can be seen on lung ultrasound (LUS) and indicate the presence of pulmonary edema, as is seen in high-altitude pulmonary edema (HAPE). While asymptomatic B-lines have been observed in high-altitude travelers without clinical HAPE, their prevalence and relationship to high-altitude physiology remain poorly understood. This study aims to characterize B-line development in high-altitude travelers and its relationship to physiologic changes.MethodsThis observational study investigated adult trekkers in Nepal's Khumbu Valley. An 8-point LUS was performed to obtain a B-

Mohave Rattlesnake Envenomation Causing Allergic Myocardial Infarction (Kounis Syndrome) and Neurotoxic Respiratory Failure.

Mohave rattlesnakes (Crotalus scutulatus) are known to inhabit the deserts of southwestern United States, with their territory extending into southern Mexico. Envenomation can impart dramatic local and systemic effects on the body, ranging from local skin necrosis to neurotoxicity, depending on the protein complex in the venom. Kounis syndrome, or allergic angina, is an allergic response to a specific insult that results in myocardial injury often in the form of coronary artery vasospasm but can include myocardial infarction and coronary stent thrombosis. To our knowledge, there have not been

High Altitude Retinopathy in an Emergency Physician Working at Everest Base Camp: A Case Report.

High altitude travel can lead to high altitude retinopathy (HAR), characterized by retinal vascular dilatation, edema, and hemorrhage, particularly in young, physically active individuals with poor acclimatization. Although it is usually asymptomatic unless it affects the macula of the eye, it sometimes may lead to permanent vision loss. This report discusses a 31-year-old Belgian male at Everest Base Camp who developed scotoma in his right eye at 5300 m. Ophthalmoscopic evaluation revealed cotton wool spots in the eye and a macular bleed in the right eye, raising the suspicion of HAR. There a
