Defined by the presence of bilateral vestibular schwannomas (VS) in the great majority of cases, NF2 is a complex genetic disease with unpredictable behavior. Since the 1980s, VS surgery outcomes have dramatically improved, and large VS were surgically removed to save lives or prevent disabling sequelae as well as small-/mid-sized tumors to avoid functional adverse events such as facial paralysis and deafness. Then, rehabilitation of hearing could be achieved by either auditory brainstem or cochlear implantations. However, new technologies such as radiosurgery have shown that conservative trea…

The impact of deafness on visual attention has been widely discussed in previous research. It has been noted that deficiencies and strengths of previous research can be attributed to temporal or spatial aspects of attention, as well as variations in development and clinical characteristics. Visual attention is categorized into three networks: orienting (exogenous and endogenous), alerting (phasic and tonic), and executive control. This study aims to contribute new neuroscientific evidence supporting this hypothesis. This paper presents a systematic review of the international literature from t

In this article, we travel back to the early days of experimental use of cochlear implants (CIs) in the 1970s, when unsettled expectations of the device and broad investigations of its effects began to settle and center on speech outcomes. We describe how this attention to speech outcomes coalesced into specific understandings of what CIs do, and how implicit or explicit understandings of CIs as bionic devices that normalize hearing influenced research on and expectations of CIs into the present. We conclude that accumulated evidence about what is known and unknown about experiences and materi

Mutations in the PCDH15 gene, encoding protocadherin-15, are among the leading causes of Usher syndrome type 1 (USH1F), and account for up to 12% USH1 cases worldwide. A founder truncating variant of PCDH15 has a ∼2% carrier frequency in Ashkenazi Jews accounting for nearly 60% of their USH1 cases. Although cochlear implants can restore hearing perception in USH1 patients, presently there are no effective treatments for the vision loss due to retinitis pigmentosa. We established a founder allele-specific Pcdh15 knockin mouse model as a platform to ascertain therapeutic strategies. Using a dual

OBJECTIVE: Eosinophilic otitis media (EOM) is an intractable middle ear disease recognized by an eosinophil enriched middle ear effusion and mucosa. Although precise pathogenesis of EOM remains unclear, it is characterized by type 2 inflammation. Since IgG4 is an IgG subclass induced by type 2 cytokines such as IL-4 and IL-13, we sought to characterize and compare local IgG4 expression in patients with and without EOM.
METHODS: Twelve patients with bilateral profound hearing loss, 9 of which underwent a cochlear implant surgery, were enrolled in this study (6 with EOM and 6 without EOM). The

Inner ear diseases are not adequately treated by systemic drug administration mainly because of the blood-perilymph barrier that reduces exchanges between plasma and inner ear fluids. Local drug delivery methods including intratympanic and intracochlear administrations are currently developed to treat inner ear disorders more efficiently. Intratympanic administration is minimally invasive but relies on diffusion through middle ear barriers for drug entry into the cochlea, whereas intracochlear administration offers direct access to the colchlea but is rather invasive. A wide range of drug deli

Bone anchored hearing aids are well established for canal atresia, otosclerosis and chronic suppurative otitis media. Refinements in technique to maximise gain while keeping the complications to a minimum are desirable. This study was taken up in order to explore the potential advantage of a second or spare fixture placed at the time of primary surgery. A group of patients who underwent BAHA insertion at The Emmeline Centre for Cochlear Implants and Bone Anchored Hearing Aids, Addenbrookes hospital, Cambridge, UK with the placement of a spare fixture between 1999 and 2002 were compared to thos

BACKGROUND: Concomitant developmental disability is common in deaf and hard of hearing (DHH) children. We describe the early developmental profiles of DHH children and explore factors that may be predictive of these profiles.
METHODS: We report on data from DHH children aged 0-66 months who are participants of a longitudinal child hearing databank in Victoria, Australia. Developmental profiles were measured using the Ages and Stages Questionnaire (ASQ) across five domains (communication, gross motor, fine motor, personal social and problem solving). We reported descriptive statistics and used

OBJECTIVE: Although there are useful guidelines defining the boundary between damaging and non-damaging electrical stimulation they were derived from acute studies using large surface area electrodes in direct contact with cortical neurons. These parameters are a small subset of the parameters used by neural stimulators. More recently, histological examination of cochleae from patients that were long-term cochlear implant users have shown evidence of particulate platinum (Pt). The pathophysiological effect of Pt within the cochlea is unknown. We examined the response of the cochlea to stimulus

Classroom environments pose significant challenges for hearing-impaired children, where background noise, simultaneous talkers, and reverberation degrade speech perception. Most deep learning-based speech separation algorithms are developed for adult voices in simplified conditions, neglecting both the higher spectral similarity of children's voices and the acoustic complexity of classrooms. We address this gap using MIMO-TasNet, a compact, low-latency, multi-channel architecture suited for real-time processing in bilateral hearing aids or cochlear implants. We simulated naturalistic classroom

PURPOSE: This study examined the internal consistency reliability, interrater reliability, and concurrent validity of the norm-referenced Test of Early Written Language-Third Edition (TEWL-3) to determine if it is an appropriate measure to use when determining if elementary children who are deaf and hard of hearing (DHH) meet grade-level writing expectations.
METHOD: Participants included 111 second-grade children across three groups: children with typical hearing (CTH), children who use cochlear implants, and children who use hearing aids. Reliability and validity were compared across all gr

INTRODUCTION: Balance disorders are a major challenge today as they greatly affect people's quality of life. Although medical treatment and vestibular rehabilitation are insufficient in many cases, significant improvements in the treatment of vestibular pathologies have been achieved in recent decades. New trends toward minimally invasive procedures have led to the study of direct treatment at the vestibular level, such as the use of vestibular implants. Great efforts have been made to acquire a thorough knowledge of these organs. However, the surgical anatomy and exact spatial orientation of

HYPOTHESIS: A high-fidelity, inexpensive middle ear simulator could be created to enhance surgical training that would be rated as having high face validity by experts.
BACKGROUND: With rapid prototyping using additive manufacturing technology (AMT), one can create high-resolution 3-dimensional replicas of the middle ear at low cost and high fidelity. Such a simulator could be of great benefit for surgical training, particularly in light of new resident training guidelines.
METHODS: AMT was used to create surgical middle ear simulator (SMS) with 2 different materials simulating bone and soft

It has been shown that frequency-place mismatch has detrimental effects on English speech recognition. The present study investigated the effects of mismatched spectral distribution of envelopes on Mandarin Chinese tone recognition using a noise-excited vocoder. In Experiment 1, speech samples were processed to simulate a cochlear implant with various insertion depths. The carrier bands were shifted basally relative to the analysis bands by 1-7 mm in the cochlea. Nine normal-hearing Mandarin Chinese listeners participated in this experiment. Basal shift of the carriers only slightly affected t

Although normal-hearing (NH) and cochlear implant (CI) listeners are able to adapt to spectrally shifted speech to some degree, auditory training has been shown to provide more complete and/or accelerated adaptation. However, it is unclear whether listeners use auditory and visual feedback to improve discrimination of speech stimuli, or to learn the identity of speech stimuli. The present study investigated the effects of training with lexical and nonlexical labels on NH listeners' perceptual adaptation to spectrally degraded and spectrally shifted vowels. An eight-channel sine wave vocoder wa

The electrical stimulation of the acoustic nerve plays an essential role in the selection of patients for cochlea implantation. The performance of a deaf ear under the conditions of electrical stimulation can be assessed by means of the pre-operative promontory test using a transtympanic needle electrode. Five components of this test were applied to 48 patients: the determination of perception threshold and upper comfortable level for pulse and burst stimulation, the pitch test, the temporal difference test and the gap detection test. The mean values emerging from these examinations may be use

PURPOSE: Bionic ear implants provide a solution for deafness. Patients treated with these hearing devices are often children who require close follow-up with frequent functional and radiological examinations; in particular, multislice computed tomography (MSCT). Dental volumetric cone-beam CT (CBCT) has been reported as a reliable technique for acquiring images of the temporal bone while delivering low radiation doses and containing costs. The aim of this study was to assess, in terms of radiation dose and image quality, the possibility of using CBCT as an alternative to MSCT in patients with
