Enteroviral meningitis is a common cause of aseptic meningitis in infants, yet respiratory deterioration, including central hypoventilation, is seldom emphasized. High-flow nasal cannula (HFNC) therapy may offer benefits beyond oxygen delivery. We report a case of a six-day-old term boy with enteroviral meningitis caused by coxsackievirus B2. He developed intermittent oxygen desaturation, bradypnea, and hypercapnia that were unresponsive to standard oxygen supplementation. HFNC rapidly stabilized his respiratory rate and oxygen saturation. This case suggests that HFNC may be a useful noninvasi…

The use of high-flow nasal cannula (HFNC) as an oxygen therapy post-extubation has demonstrated varying success rates across different surgical populations. This study aimed to identify the predictive factors influencing HFNC weaning outcomes in patients with postoperative extubation hypoxemia. We conducted a retrospective analysis of patients in a surgical intensive care unit, categorized into three major postoperative groups: cardiothoracic surgery, upper abdominal surgery, and other surgeries. Our analysis examined pre-extubation weaning profiles, vital signs before and after HFNC initiatio…

To determine if pre-operative high-flow nasal cannula (flow > 2 L per minute) or mechanical ventilation impact post-operative outcomes after the superior cavopulmonary connection. A single-center retrospective review of single-ventricle patients who underwent the superior cavopulmonary connection procedure at a tertiary care center over a 10-year period. Patients who underwent the procedure at greater than 2 years of age were excluded. The groups of interest were those who were on mechanical ventilation or high-flow nasal cannula. Patients in room air or on nasal cannula oxygen served as the c…

The Flow Regulated Nasal Delivery System (FRNDS) is a novel smart nasal cannula platform developed to enable noninvasive, continuous monitoring of tidal volume (VT) and detection of abnormal breathing events. Combined with peripheral capillary oxygen saturation (SpO₂) data modulation of oxygen flow can be accomplished. In a cohort of 57 adults, FRNDS-derived VT and minute ventilation were compared to reference measurements from respiratory inductance plethysmography (RIP) across a range of oxygen flow rates. While the cannula system tended to overestimate inspiratory and underestimate expirato…

BackgroundNeonates are more susceptible to acute respiratory failure than older children. It is unknown to what extent high-flow nasal cannula (HFNC) alters intrathoracic pressure (ITP), potentially decreasing cardiac output (CO) due to cardiopulmonary interactions. This study evaluated the impact of flow titration on tracheal pressure (a surrogate for ITP) and CO via HFNC in an established porcine model of neonatal respiratory failure. Unlike prior research, this study examines both ITP and CO demonstrating that although the ITP increased with higher HFNC flows, CO did not decrease.MethodsTwe…

BACKGROUND AND OBJECTIVES: Trigeminal neuralgia causes debilitating facial pain. Treatments, including decompression, radiosurgery, and minimally invasive percutaneous procedures such as mechanical compression, are low risk and offer immediate relief, making them a preferred option. Successful percutaneous treatment depends on safely and successfully accessing the foramen ovale. This study evaluated a novel electromagnetic (EM)-guided cannula used for percutaneous trigeminal rhizotomy.
METHODS: The cannula's accuracy and EM neuronavigation compatibility were evaluated in a cadaveric study. Su…

BACKGROUND: Hypoxemia is a relatively common complication in stable patients during fiberoptic bronchoscopy (FOB). To prevent this complication, high-flow nasal cannula (HFNC) has been described as an alternative to standard oxygen therapy. However, the advantages of HFNC over standard oxygen therapy in acute care patients receiving supplemental oxygen before FOB performed with an oral approach are unknown.
METHODS: We conducted an observational study that involved subjects with a presumptive diagnosis of pneumonia and a clinical indication for a bronchial aspirate sample. The type of oxygen …

BACKGROUND: Prophylactic high-flow nasal cannula (HFNC) oxygen therapy can decrease the risk of extubation failure. It is frequently used in the postextubation phase alone or in combination with noninvasive ventilation. However, its physiological effects in this setting have not been thoroughly investigated. The aim of this study was to determine comprehensively the effects of HFNC applied after extubation on respiratory effort, diaphragm activity, gas exchange, ventilation distribution, and cardiovascular biomarkers.
METHODS: This was a prospective randomized crossover physiological study in…

Rationale: The long-term effects of using a high-flow nasal cannula for chronic hypercapnic respiratory failure caused by chronic obstructive pulmonary disease remain unclear. Objectives: To assess whether long-term high-flow nasal cannula use reduces the number of exacerbations and improves other physiological parameters in patients with chronic hypercapnic respiratory failure caused by chronic obstructive pulmonary disease. Methods: We enrolled 104 participants (aged ⩾40 yr) with daytime hypercapnia (Global Initiative for Chronic Obstructive Lung Disease stages 2-4) receiving long-term oxyge…

High-flow nasal cannula (HFNC) is extensively used for acute respiratory failure. However, questions remain regarding its physiological effects. We explored 1) whether HFNC produced similar effects to continuous positive airway pressure (CPAP); 2) possible explanations of respiratory rate changes; 3) the effects of mouth opening. Two studies were conducted: a bench study using a manikin's head with lungs connected to a breathing simulator while delivering HFNC flow rates from 0 to 60L/min; a physiological cross-over study in 10 healthy volunteers receiving HFNC (20 to 60L/min) with the mouth o…

OBJECTIVES: Bronchiolitis is a viral respiratory infection that can progress to acute respiratory failure. This study evaluated the variability of hospital-wide high-flow nasal cannula (HFNC) usage outside of the ICU and its association with length of stay (LOS) and cost among pediatric patients admitted with bronchiolitis.
METHODS: This study included patients <2 years old admitted with bronchiolitis between September 1, 2018 and March 31, 2019. Hospitals were divided into groups based on the proportion of patients among those who had never been in the ICU who received HFNC (non-ICU HFNC usa…

Rhino-orbital mucormycosis has seen a huge resurgence in patients post COVID-19 infection. In patients with minimal orbital disease and especially with preserved vision, retrobulbar injections of amphotericin B can be of great help in controlling the disease. Instead of giving daily injections of amphotericin B using needles every time, we used an 18-gauge intravenous (IV) cannula with injection port and suture holes to deliver the amphotericin into the orbital space for a period of 5 days. Patients were more compliant and less distressed with this method compared with being given an injection…

A woman in her 50s who had been diagnosed with COVID-19 developed deep vein thrombosis in the left femoral vein extending into inferior vena cava (IVC). An IVC filter was placed to prevent fatal pulmonary embolism. Her respiratory failure subsequently deteriorated despite optimal mechanical ventilation and required venovenous extracorporeal membrane oxygenation (VV-ECMO) as a rescue therapy. Femoro-jugular VV-ECMO configuration was not suitable due to the IVC filter, hence a single-site venous cannulation using bicaval dual lumen (AvalonElite) cannula was selected. Placement of the Avalon cann…

INTRODUCTION: Despite adhering to criteria for extubation, up to 20% of intensive care patients require re-intubation, even with use of post-extubation high-flow nasal cannula (HFNC). This study aims to identify independent predictors and outcomes of extubation failure in patients who failed post-extubation HFNC.
METHODS: We conducted a multicentre observational study involving 9 adult intensive care units (ICUs) across 5 public hospitals in Singapore. We included patients extubated to HFNC following spontaneous breathing trials. We compared patients who were successfully weaned off HFNC with…

Background: High flow nasal cannula (HFNC) is commonly used as non-invasive respiratory support in critically ill children. There are limited data to inform consensus on optimal device parameters, determinants of successful patient response, and indications for escalation of support. Clinical scores, such as the respiratory rate-oxygenation (ROX) index, have been described as a means to predict HFNC non-response, but are limited to evaluating for escalations to invasive mechanical ventilation (MV). In the presence of apparent HFNC non-response, a clinician may choose to increase the HFNC flow …
