Determinants of bedwetting trajectories between 4 and 7 years - A birth cohort analysis.

INTRODUCTION: Enuresis is frequent in school-aged children and results from a complex interaction between genetics, biological and psychosocial factors. This study aims to analyze bedwetting trajectories between 4 and 7 years of age and to evaluate the impact of biological and developmental characteristics of the child and sociodemographic factors in those bedwetting trajectories. METHODS: Data from 5433 children from the Generation XXI population-based birth cohort was analyzed. Four bedwetting trajectories were defined: normative (acquired nighttime bladder control at 4 years and no enuresi…

Modeling the cost-benefit of nerve conduction studies in pre-employment screening for carpal tunnel syndrome.

OBJECTIVE: To assess the features of adolescent bedwetters, as few data are available on enuresis in this age group. PATIENTS AND METHODS: A specific database for adolescents and young adults was created with the collaboration of various specialists (paediatricians, urologists, gynaecologists, psychiatrists). Questions focused on family and personal history, stressful events, age of attaining urinary and fecal control, characteristics of enuresis (primary vs secondary, monosymptomatic vs enuresis associated with daytime urinary symptoms), school performance, diagnostic examination and physica…

4. Bedwetting and toileting problems in children.

Bedwetting (nocturnal enuresis) is common. It occurs in up to 20% of 5 year olds and 10% of 10 year olds, with a spontaneous remission rate of 14% per year. Weekly daytime wetting occurs in 5% of children, most of whom (80%) also wet the bed. Bedwetting can have a considerable impact on children and families, affecting a child's self-esteem and interpersonal relationships, and his or her performance at school. Primary nocturnal enuresis (never consistently dry at night) should be distinguished from secondary nocturnal enuresis (previously dry for at least 6 months). Important risk factors for …

Retention control training in the treatment of younger versus older enuretic children.

This study compared the efficacy of retention control training (RCT) in two groups of children suffering from nocturnal enuresis: a younger group of 15 children aged 4 to 5 years and an older group of 15 children aged 7 to 8 years. To control for family history, frequency, and type of enuresis, only children with primary nocturnal enuresis who had a family history of enuresis and who wet the bed 4 to 7 nights per week were included. RCT was found to be an effective treatment for young children who were closer to the final stage of bladder development, but not for older children. RCT may be use…

[Development of bladder capacity, nocturnal urinary volume and urinary behavior in nonenuretic and enuretic children].

Bladder capacity, nocturnal urinary volume and nocturnal urinary behavior were investigated in 1751 nonenuretic and 282 enuretic children in … dren. It was not likely from this evidence that an immature bladder capacity was a primary cause of enuresis. 3. For the estimation of the maximum bladder capacity in the study of enuresis, urinary output in the morning immediately after awakening with sufficient urinary sensation was thought to be more reasonable than that in the daytime, because output i …

Psychiatric comorbidities of children with elimination disorders.

Enuresis and encopresis can be stressful for children and parents. We investigated the comorbid psychiatric disorders and the emotional and behavioral symptoms associated with elimination disorders. A total of 97 children and adolescents (aged 4-17 years) with an elimination disorder participated in this study. The elimination disorder group consisted of three subgroups: 50 subjects with enuresis nocturna, 26 with encopresis, and 21 subjects with enuresis+encopresis. The control group with no elimination disorder comprised 50 healthy subjects. All children were interviewed by a child and adole…

Attitudes of enuretic children towards their illness.

UNLABELLED: The aim of this study was to establish the attitudes of enuretic children towards their illness and to assess the correlation between children's attitudes and biological and medical variables describing the clinical picture and course of enuresis. Fifty children were included (31M, 19F), aged 8-18 y (mean 12.74 y), all with the diagnosis of enuresis. The reference groups consisted of 60 children with asthma and 40 children with chronic heart disease, aged 8-12 y. The Polish version of the Child Attitude Toward Illness Scale (CATIS) was administered to all the children and structure…

Desmopressin treatment: current status.

Desmopressin has a proven pharmacological effect in most enuretic patients, although a clinical response is not seen in all patients. Numerous questions about the current treatment status of desmopressin include the specific anti-enuretic effect of desmopressin, the effect of desmopressin on sleep and the use of desmopressin as a possible cure for enuresis. The Swedish Enuresis Trial has produced some very positive results on the long-term use of desmopressin, showing a 61% response rate (> 50% reduction in wet nights). Desmopressin has proven to be highly effective when used in combination wi…

Nocturnal polyuria is related to absent circadian rhythm of glomerular filtration rate.

PURPOSE: Monosymptomatic nocturnal enuresis is frequently associated with nocturnal polyuria and low urinary osmolality during the night. Initial studies found decreased vasopressin levels associated with low urinary osmolality overnight. Together with the documented desmopressin response, this was suggestive of a primary role for vasopressin in the pathogenesis of enuresis in the absence of bladder dysfunction. Recent studies no longer confirm this primary role of vasopressin. Other pathogenetic factors such as disordered renal sodium handling, hypercalciuria, increased prostaglandins and/or …

Evaluation of voiding dysfunctions in children with chronic functional constipation.

There are controversial results about the role of dysfunctional bowel emptying in disorders of the urinary tract like urinary tract infection (UTI), vesicoureteral reflux (VUR) and enuresis. Constipation may cause UTI, enuresis and VUR due to the uninhibited bladder contraction. The aim of this study was to investigate the frequency of nocturnal enuresis, UTI and instability symptoms in chronic functional constipation (CFC). This study included 38 children with CFC and 31 children as the control group. Detailed past and present history of UTIs or symptoms pointing to this diagnosis, enuresis, …

[Value of combined urodynamic examination of patients with unsatisfactory outcomes of surgical treatment of prostatic adenoma].

Forty three patients with unsatisfactory outcomes of surgical treatment of benign prostatic hyperplasia (BPH) were examined urodynamically. Thirty five patients complained of daily and nocturnal pollakiuria, incomplete evacuation of urine from the bladder, imperative voiding. Eight patients had postoperative enuresis. Seventeen control patients had satisfactory surgical results. The urodynamic findings showed that postoperative dysuria was caused by detrusor pathology: detrusor hyperactivity (n = 27, 62.8%), contractile hypoactivity (n = 4, 9.3%), infravesical obstruction (n = 4, 9.3%). Diures…

Comparison of health-related quality of life in children and adolescents with monosymptomatic nocturnal enuresis under therapy versus allergic bronchial asthma, diabetes mellitus type I, and juvenile idiopathic arthritis - a KINDL-R-based study.

BACKGROUND: Monosymptomatic nocturnal enuresis (MNE) affects 10-15% of children at school age. Influences on psychological well-being and self-confidence were repeatedly described, but effects on health-related quality of life (HRQoL) have been rarely reported. The aim of this study is to examine the HRQoL under therapy by using KINDL-R and to compare it with three recognized chronic diseases. METHODS: Questionnaires were sent to all patients with MNE of the special outpatient clinic for enuresis (age 7-17 years; groups: I: 7-13, II: 14-17 years; at least 3 months of therapy, no achieved dryn…

[Evaluation and treatment of the enuretic child: eight years' experience].

Enuresis is a common functional problem among children which is defined as a complete involuntary voiding of urine at an age which control should be present. Bed wetting generally resolves with increasing age, but the restriction in social life and the psychological secondary problems, so frequent in older patients, justify an appropriate treatment of the problem in the child over seven. At children's Hospital of Florence University an enuresis service exists since 1983, and during these years 541 children applied to the structure. 326 children completed the treatment, among these there were 2…

The stop-flow technique: a way to measure detrusor strength.

Maximum intravesical isometric pressure, obtained by stopping the urinary flow in progress, measures detrusor strength and allows an accurate evaluation of bladder contraction. Micturition studies were performed on 7 normal subjects and 48 patients with urinary obstruction, enuresis, neurogenic vesical dysfunction or stress incontinence. In the case of obstruction maximum isometric pressure appears to relate to the degree of detrusor compensation and allows a prognosis as to the future of detrusor function after the obstruction is removed. An unexpected large proportion of patients with enures…

A Comparative Study of Vitamin D Serum Levels in Monosymptomatic Enuretic Children and Non-Enuretic Children.

BACKGROUND: Enuresis refers to urinary incontinence during sleep in children over 5 years of age. The pathogenesis of enuresis is complex. It is related to low functional bladder capacity, nocturnal bladder overactivity, and high arousal threshold, and it is also related to the immaturity of the central nervous system. Also, it was related to nocturnal polyuria and decreased nocturnal urine osmolality. There are many clinical observations that link nocturnal enuresis with developmental delay in language and physical growth. Vitamin D is important for the critical biological actions involving n…
