Replantations and revascularizations on the level of the arm are performed in the smallest number. The achieved functional results are poor. The paper presents a result achieved in a patient who sustained an avulsion amputation of an upper arm in the year 1974. The first long-term control was made 11 years after the revascularization of the limb which rated the result as III acc. to Chen's score system. The patient was steadily highly motivated for hand rehabilitation process. The second control performed 33 years after the accident proved the increase of sensibility, muscle strength as well a

The Os Styloideum (OS) is an accessory carpal bone located in the dorsal aspect of the wrist between the base of the second and third metacarpal bones and the trapezoid and the capitate. It is frequently partially coalited with the metacarpal bones and is usually asymptomatic. It may become symptomatic after a traumatic fracture, developing swelling, edema, and pain. We report 3 cases with a fracture of an OS partial coalition with the third metacarpal bone with signs of delayed consolidation. They all complain of persistent pain at the dorsal aspect of the hand that increased with activity. O

Osteomyelitis of the hand is rare, even more so in the carpal bones. Patients with rheumatoid arthritis (RA) have a higher infection rate overall, and up to a 14-fold increase in the incidence of septic arthritis of the hand. The destruction of immunologic barriers, such as cartilage and joint capsules, as well as the use of immunosuppressive medications will have an impact on the higher incidence of articular infections and osteomyelitis in these patients. Infection in these cases is often overlooked because of the similarity of presentation to an acute event of RA. When osteomyelitis is pres

Pancreatic adenocarcinoma is an aggressive malignancy that accounts for over 90% of pancreatic cancers. It is associated with a poor prognosis and a 5-year survival rate of less than 10%. In 2023, there were an estimated 64,050 new cases of pancreatic cancer and 50,550 related deaths. Common sites of metastasis include the liver, lungs, lymph nodes, and peritoneal cavity. While bone metastases can occur, metastatic disease involving the bones of the hand is exceedingly rare. In this report, we present a case of metastatic pancreatic cancer involving the trapezium bone of the hand, along with a

OBJECTIVE: The purpose of this study was to evaluate the lifetime evolution of the radioclinical features in a large family with van Buchem disease.
DESIGN AND PATIENTS: The study population included 13 patients, ranging between 6 and 69 years. The evolution of the clinical features has been assessed by retrospective analysis of the clinical records of the patients. The age-related evolution of the cortical hyperostosis and defective modeling at the tubular bones was evaluated by morphometric analysis of hand films in 9 patients, compared with 9 control individuals. The authors report a case of massive osteolysis of all of the metacarpal bones of the right hand in a 26 year old man. The disease commenced 9 years previously with painful symptoms associated with inflammation and the initial X-rays showed periosteal reaction of the 2nd and 3rd metacarpals. Massive osteolysis of all of the metacarpal bones, including the 1st metacarpal, developed progressively over a period of one year, although no signs of osteolysis were observed in the carpal bones or phalanges. A surgical operation, performed to realign the ring and little fingers, revealed a 4 mm thick p

Osteoid Osteoma is benign bone forming tumor which commonly occurs in long bones of lower limb. Presence of these rare tumors in the distal phalanx of the digits of the hand is considered a rare phenomenon. In hand, they usually present as chronic pain, swelling, nail enlargement and increase in size of digit. Diagnosis is challenging with clinical examination and usual imaging modalities and often confused with glomus tumor, enchondroma, infection, trauma and rheumatic disease. Surgical excision of the tumor, if present in hand, is the treatment of choice that aids in coming to the exact diag

Schmid metaphyseal chondrodysplasia (Schmid MCD, MIM 156500) is caused by mutations in the COL10A1 gene and is clinically characterized by short stature, bowed legs, and a waddling gait. Radiographic findings include anterior cupping, sclerosis and splaying of the ribs, diffuse metaphyseal flaring, and irregularity that is most pronounced at the knees, coxa vara, and femoral bowing. We reviewed the radiographs of Schmid MCD patients at the International Skeletal Dysplasia Registry in Los Angeles for evidence of hand involvement. We found hand involvement in 47% (7/15) of cases included in our

The tendency to regard lesions of the flexor tendons of the hand as difficult to repair and those of the extensor tendons as simple to treat is completely arbitrary. Extensor tendons have the advantage of running an almost entirely extrasynovial course, which facilitates the repair. On the other hand, they are thin, superficial structures, and when damaged they tend rapidly to become adherent to the underlying bones and joints. The excursion of the extensor tendons of the hand is considerably less than that of the flexors; thus, a loss of length is more difficult to compensate for. In our expe

Scar contracture deformity, which can lead to dysfunction of hand and low quality of life, is one of the common complication after hand burns. The prevention measures of scar contracture after hand burns include large skin grafting, prevention of infection, insistence on wearing pressure gloves, use of silicone sheets, wearing orthosis, accepting proper physical therapy, and early functional exercise. The primary treatments of postburn contractures of the hand are surgery, drugs, laser treatment, and rehabilitation therapy. Excision of scars, release of muscle, joints or bones, and soft tissue

Distraction osteogenesis was used to correct congenital hand deformities in 12 digits. The patients' ages at the time of distraction ranged between 2.5 and 7 years. A total of nine patients underwent lengthening of 12 bones. The average period of distraction was 31.1 +/- 17.6 days. The average length of distraction was 23.6 +/- 7.3 mm. Seventeen percent of the patients after lengthening had an angular deformity that required remodeling of the regenerate. No infections were noted. Distraction osteogenesis provides a useful tool for reliable and predictable correction of osseous defects with vas

The Herbert bone screw is a new device, designed to produce compression and rigid fixation of small cancellous bone fragments. Being completely buried within the bone, it may be introduced through the articular surface of the joint. A second operation to remove the implant is not required. Although originally developed for fixation of scaphoid fractures, its use has been extended to other carpal bones and small joint fusions in the hand and wrist. The author's experience with this technique shows that excellent results may be achieved when rigid internal fixation allows for early mobilization

Fractures of the carpal bones are uncommon. On standard radiographs fractures are often not recognized and a computed tomography (CT) scan is the diagnostic method of choice. The aim of treatment is to restore pain-free and full functioning of the hand. A distinction is made between stable and unstable carpal fractures. Stable non-displaced fractures can be treated conservatively. Unstable and displaced fractures have an increased risk of arthritis and non-union and should be stabilized by screws or k‑wires. If treated adequately, fractures of the carpal bones have a good prognosis. Unstable a

Hypertrophic osteoarthropathy is characterised by digital clubbing and periosteal reaction of long bones. Most cases are associated with malignancy or other conditions such as congenital heart disease, liver cirrhosis, pulmonary fibrosis, biliary atresia and inflammatory bowel diseases. We report a middle-aged man found to have 15-year history of clubbing of the fingers and toes on his routine check-up for dyspepsia. Skiagram of hand joints showed periosteal apposition without any periosteal reaction of long bones. The search for a secondary cause of clubbing remained negative. The primary or

Four boys of the same family (three first cousins and their uncle) had the same syndrome of multiple malformations. They had hypertelorism, antimongolian slant of the palpebral fissures, low-set ears, and Pierre Robin syndrome. The ossification of the skull vault was defective, with broad sutures and fontanelles. The ribs were sinuous, the clavicles were long, thin, and sloping, and the vertebrae were flattened. There were abnormal bowing with hyperostosis of the long bones, faulty ossification of the bones of hand and feet, and "fanned-out" toes. The disorder was lethal within a few weeks. Th

Longitudinal lacerations involving the finger commissures and adjacent intermetacarpal clefts (including the adjacent tendons, the intrinsic muscles, the deep transverse metacarpal ligament, adjacent arteries, nerves, bones and joints), present multiple problems not seen in the unusual hand lacerations. Thorough cleansing, delayed closure, accurate repair of all injured structures with prompt appropriate rehabilitation results in excellent appearance and early function.

A new X-ray sign of gonadal dysgenesis is described. In this series nine out of ten patients with Turner's syndrome have a coarse, reticular pattern of the carpal bones. A comparison with other previously described roentgenographic characteristics on hand and wrist films of these children leads to the conclusion that the new sign is more reliable and specific.

BACKGROUND: Metachondromatosis is a condition that causes gross conical metaphyseal expansion (sometimes irregular), cortical thinning, exostoses. Metachondromatous lesions occur mainly in the extremities and are roughly symmetrical. The lesions can involve the bones of the hand and all long bones in the arms and legs. The distribution in this case additionally involved the acromion process and ischia. The bone changes, although dramatic, can be confused with other types of metaphyseal dysplasia such as Gaucher disease and multiple exostoses.
OBJECTIVE: This paper will review the literature w

The longitudinal epiphyseal bracket is a rare ossification defect of tubular bones in the hand and foot. This deformity contains an abnormal secondary ossification center, which may lead to progressive shortening and angular deformity of involved bones. This article describes the results from early treatment of this deformity before the secondary ossification center ossifies. Four patients with seven involved bones (5 metatarsals and 2 phalanges) were treated with surgical excision of the longitudinal epiphyseal brackets, without corrective osteotomy. The average age at surgery was 16 months (
