The Danish Infection Cohort: a resource for population based infectious disease epidemiology.

Infections are a major cause of morbidity and mortality, but many aspects of the epidemiology of infections are unknown or unclear because key data are lacking. We created The Danish Infection Cohort by linking Danish National Health Survey data (including self-reported behavioral, health, and other information) to data on diagnoses and medications in Danish national health registries. We included individuals ≥ 18 years of age in surveys from 2010, 2013, 2017, and 2021. We identified evidence of infection in the 365 days post-survey by using two definitions: hospital-diagnosed infections, via

Community-based audits of snake envenomations in a resource-challenged setting of Cameroon: case series.

BACKGROUND: Snakebites are a major cause of mortality and morbidity worldwide with the highest mortality burden in poor rural areas of sub-Saharan Africa. Inadequate surveillance systems result in loss of morbidity and mortality data in these settings. Although rarely reported in these resource-constraint environments, community-based audits are recognised pivotal tools which could help update existing data and indicate key public health interventions to curb snakebite-related mortality. Herein, we present two cases of snakebite-related deaths in a rural Cameroonian community. CASE PRESENTATI

Bridging organizational boundaries in complex care pathways: lessons from a Lean and Safety Management approach.

BACKGROUND: Complex care processes that span hospital and community settings represent a growing organizational challenge for healthcare systems, particularly in the management of chronic conditions. These pathways involve interdependent activities distributed across multiple facilities, professional groups, and levels of care, increasing the risk of fragmentation, inefficiency, and patient harm. While Lean Healthcare (LH) has been widely adopted to improve healthcare performance, its application has largely remained confined to single hospital units, with limited attention to care integration

Retrospective study of hospitalized patients with chronic leg ulcers in Hawaii.

The authors describe 226 patients with chronic leg ulcers at a community hospital over a 14-year period. Most leg ulcers were ischemic or venous in origin and located below the ankle. Fifty-five percent of patients were diabetic and the majority of patients were hypertensive. The cases required prolonged hospitalization for systemic antibiotics and vascular surgery. Only 2.3% of patients required above the knee amputation, and 7% needed below-the-knee amputation.

Impact of Infectious Diseases Consultation on Clinical Outcomes of Patients with Staphylococcus aureus Bacteremia in a Community Health System.

Staphylococcus aureus bacteremia (SAB) causes high rates of morbidity and death. Several studies in academic health settings have demonstrated that consultations from infectious diseases specialists improve the quality of care and clinical outcomes for SAB. Few data that describe the impact in resource-limited settings such as community hospitals are available. This retrospective cohort study evaluated the adherence to quality-of-care indicators and the clinical outcomes for SAB in a five-hospital community health system (range of 95 to 272 available beds per hospital), for patients with versu

Learning Caught in the System: An Interactionist Qualitative Study of Workplace-Based Clinical Education in an Acute Hospital Ward.

Educational activities are increasingly implemented in clinical nursing practice with the expectation that teaching will improve care. Drawing on Biesta and van Braak's theoretical perspective on learning as professional formation, this study critically examines a video-based educational activity as a case of workplace-based clinical education in an acute hospital ward in Denmark. Based on interviews and an interactionist, abductive analytical approach, the analysis shows how learning in clinical settings is shaped by organisational, epistemic and socio-material conditions, challenging assumpt

Transforming the Patient Grievance Review Process by Addressing Both Patient and Physician Well-Being.

Patient grievances are an inevitable part of health care delivery, yet existing review processes do not focus on the unforeseen harms that grievances can have on the health care team. At a large academic children's hospital, a human-centered approach was used to redesign the grievance review process. Through listening sessions with frontline physicians and the formation of a multidisciplinary work group, the authors developed and implemented a novel approach to responding to patient concerns related to physician care, while also supporting the physicians. The redesigned process focused on comm

An observation of two NHS trusts' use of organisational wellbeing interventions during the COVID-19 pandemic: lessons learnt.

BACKGROUND: Healthcare workers' (HW') mental health during pandemics may be challenged due to the demands of their professional work. There is a need for effective organisational wellbeing strategies so that HW' mental health is safeguarded. In this article, we sought to examine approaches to implementing wellbeing strategies in hospital trusts, which we hope will improve future workplace planning. METHODS: A pilot study comprising semi-structured interviews with five management staff in two NHS trusts (which employ approximately 5,000 staff each) in north-west England (NWE) were conducted to

Social determinants of health (SDOH)-conditions where individuals live, learn, work, and socialize-significantly influence health disparities and patient care outcomes. SDOH can be categorized into five key domains: economic stability, access to quality education, access to quality healthcare, neighborhood environments, and social support. Structural barriers such as economic inequality, limited transportation, and discrimination directly impact patients' willingness and ability to access care. Transportation barriers, notably among low-income and elderly populations, often result in delayed o

Maternal and Neonatal Mortality in Somalia: A Persistent Women's Health Crisis Requiring Primary Health Care and Community Health System Reform.

This policy-oriented commentary aims to interpret the drivers of maternal and neonatal mortality in Somalia through the Three Delays Model and the World Health Organization health-system building-block framework and to propose a Somalia-specific policy framework for strengthening the continuum of care. It draws on purposively selected peer-reviewed literature, national and international reports, and health-system evidence. Its distinctive contribution is to frame maternal and neonatal mortality as both a women's health and reproductive-health equity crisis and a priority for health-system refo
