Mechanical Support in Early Cardiogenic Shock: What Is the Role of Intra-aortic Balloon Counterpulsation?

PURPOSE OF REVIEW: We aim to summarize recent insights and provide an up-to-date overview on the role of intra-aortic balloon pump (IABP) counterpulsation in cardiogenic shock (CS). RECENT FINDINGS: In the largest randomized controlled trial (RCT) of patients with CS after acute myocardial infarction (AMICS), IABP did not lower mortality. However, recent data suggest a role for IABP in patients who have persistent ischemia after revascularization. Moreover, in the growing population of CS not caused by acute coronary syndrome (ACS), multiple retrospective studies and one small RCT report on s

Left Ventricular Unloading During Extracorporeal Membrane Oxygenation in Patients With Cardiogenic Shock.

BACKGROUND: Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is a widely used form of mechanical circulatory support in patients with refractory cardiogenic shock. A common drawback of this modality is a resultant increase in left ventricular afterload. OBJECTIVES: The purpose of this meta-analysis was to examine the efficacy and safety of left ventricular unloading strategies during VA-ECMO in adult patients with cardiogenic shock. METHODS: The authors performed a systematic search of studies examining left ventricular unloading during VA-ECMO in Medline, EMBASE, and the Cochrane

Hemodynamic effects of standard versus larger-capacity intraaortic balloon counterpulsation pumps.

OBJECTIVE: Several recent trials have examined the clinical utility of intraaortic balloon counterpulsation pumps (IABPs) in cardiogenic shock and acute coronary syndromes. More recently, a larger-capacity 50 cc IABP was introduced into practice. No data comparing the hemodynamic effects of the 40 cc vs 50 cc IABP exist. Our aim was to explore the hemodynamic effects of the 50 cc IABP in real-world clinical practice. METHODS: Demographic, hemodynamic, and laboratory data were retrospectively examined in 26 consecutive subjects treated with a 50 cc IABP and compared with 26 patients receiving

Partial mechanical circulatory support in children.

Partial mechanical support devices are capable of partially unloading only one ventricle, often the systemic one, in the setting of acute circulatory failure. They are rarely used in the pediatric population, as the mode of circulatory failure in patients with congenital heart disease often involves biventricular or a predominantly right ventricular component. The devices include intra-aortic balloon pumping, Impella, TandemHeart, and CentriMag. They are rarely used as a bridge-to-recovery, but more often as a bridge-to-decision, or bridge-to-conversion to full mechanical support systems, such…

Tako-tsubo cardiomyopathy.

A 48-year-old woman presented to her local hospital with chest pain. Her electrocardiogram showed incomplete right bundle branch block, diffuse ST segment elevation and QTc prolongation. Shortly after admission, she became hypotensive and was referred for a coronary angiogram. Her coronary arteries were normal, but left ventriculography showed morphological changes typical of apical ballooning, as found in tako-tsubo cardiomyopathy. An echocardiogram confirmed complete akinesis of the left ventricular apex, with hyperkinesis of basal segments and an ejection fraction of 35% to 40%. Dynamic lef

Application of VA-ECMO combined with IABP in a patient with cardiac arrest following fulminant myocarditis.

BACKGROUND: Fulminant myocarditis is a severe cardiac condition characterized by rapid progression and high mortality. In recent years, advancements in medical technology, particularly the application of mechanical circulatory support, have significantly improved patient outcomes. CASE PRESENTATION: A 36-year-old male patient was admitted with a history of “fever for 5 days and chest tightness and pain for 2 days”.He was diagnosed with acute myocarditis and subsequently experienced cardiac arrest. After resuscitation, he was transferred to the medical ICU, where he received VA-ECMO and IABP su

Temporary Mechanical Circulatory Support.

BACKGROUND: Cardiogenic shock (CS) remains a leading cause of early death in acute cardiac care, with 30-day mortality frequently approaching 40-50% despite advances in revascularization and critical care pathways. Contemporary CS management has progressed through standardized definitions, staging systems, and phenotyping frameworks that support earlier recognition and a more disciplined escalation strategy. However, evidence-based therapies that consistently reduce mortality remain limited, and device-related complications and heterogeneous etiologies continue to constrain outcome gains. SUM

Perioperative mechanical circulatory support.

With increases in patient acuity, the use of devices that provide Mechanical Circulatory Support (MCS) is increasing rapidly. By extension, anesthesiologists are increasingly called upon to manage patients supported by MCS. The type of MCS can be categorized into left ventricular support, right ventricular support, biventricular support, and whether or not respiratory support (oxygenation and ventilation) is provided. Examples of MCS types include intra-aortic balloon pump (IABP), venoarterial extracorporeal membrane oxygenation (VA ECMO), venovenous ECMO, and transvalvular microaxial flow pum

Cardiogenic Shock and the Use of Percutaneous Mechanical Assist Devices.

Percutaneous mechanical assist devices are used in patients with cardiogenic shock. The purpose of this article is to familiarize critical care nurses with the various types of percutaneous mechanical assist devices, including the intra-aortic balloon pump, the Impella device, extracorporeal membrane oxygenation, and the TandemHeart device. Each type of device requires specific nursing care. In a patient with cardiogenic shock, the monitoring, care, and interventions provided by an experienced critical care nurse can make the difference between survival and death.

Temporary circulatory support for cardiogenic shock.

Cardiogenic shock can occur due to acute ischaemic or non-ischaemic cardiac events, or from progression of long-standing underlying heart disease. When addressing the cause of underlying disease, the management of cardiogenic shock consists of vasopressors and inotropes; however, these agents can increase myocardial oxygen consumption, impair tissue perfusion, and are frequently ineffective. An alternative approach is to temporarily augment cardiac output using mechanical devices. The use of these devices-known as temporary circulatory support systems-has increased substantially in recent year

Evaluation of an intraventricular balloon pump for short-term support of patients with heart failure.

The high cost of ventricular assist devices results in poor cost-effectiveness when used as a short-term bridging solution, thus a low-cost alternative is desirable. The present study aimed to develop an intraventricular balloon pump (IVBP) for short-term circulatory support, and to evaluate the effect of balloon actuation timing on the degree of cardiac support provided to a simulated in vitro severe heart failure (SHF) patient. A silicone IVBP was designed to avoid contact with internal left ventricular (LV) features (ie, papillary muscles, chordae, aortic, and mitral valves) based on LV com

Influence of IABP-Induced Abdominal Occlusions on Aortic Hemodynamics: A Patient-Specific Computational Evaluation.

Intraaortic balloon pump (IABP) is used as temporary mechanical assistance in case of cardiovascular diseases, even if different hemodynamic problems and, thus, clinical complications may happen, such as the decrease of visceral perfusion. A computational fluid dynamic (CFD) study was carried out to investigate the effects of different IABP-induced abdominal occlusions on patient-specific aortic flow. Two possible sizes (25 and 34 cm) and two locations (2 and 3 cm) of the balloon were compared, modeling four abdominal occlusions and numerically reproducing IAB inflation/deflation behavior. The

Management of cardiogenic shock attributable to acute myocardial infarction in the reperfusion era.

Cardiogenic shock is the leading cause of death among patients hospitalized with acute myocardial infarction. It is defined as tissue hypoperfusion resulting from ventricular pump failure in the presence of adequate intravascular volume. Rapid assessment and triage of patients presenting in cardiogenic shock followed by appropriate institution of supportive therapies including vasopressor and inotropic agents, mechanical ventilatory support, and intra-aortic balloon pump counterpulsation are critical for effective management of these patients. However, emergency percutaneous coronary intervent
