Use of video laryngoscopy and camera phones to communicate progression of laryngeal edema in assessing for extubation: a case series.

Video laryngoscopy has demonstrated utility in airway management. For the present case series, we report the use of video laryngoscopy to evaluate the airway of critically ill, mechanically ventilated patients, as a means to reduce the risk of immediate postextubation stridor by assessing the degree of laryngeal edema. We also describe the use of cellular phone cameras to document and communicate airway edema in using video laryngoscopy for the patients' medical records. We found video laryngoscopy to be an effective method of assessing airway edema, and cellular phone cameras were useful for …

Laryngeal exposure during laryngoscopy is better in the 25 degrees back-up position than in the supine position.

BACKGROUND: Good visualization of the glottis is important for tracheal intubation. This study evaluated whether the 25 degrees back-up position improves the quality of the laryngeal view during laryngoscopy. METHODS: Laryngoscopy with a curved blade was performed on 40 anaesthetized patients. The patients were randomly assigned to two groups. Laryngeal views were captured with a rigid 0 degrees endoscope. Views were recorded for each patient in Group A (n = 20) during laryngoscopies performed with the patient lying first in the supine position and then in the 25 degrees back-up position. Lar…

[Use of propofol (diprivan) for induction of anesthesia in neurosurgical patients. I. Pressor reaction to laryngoscopy and intubation of the trachea].

Pressor reaction to laryngoscopy and intubation of the trachea was studied in 32 patients with neurosurgical diseases of the brain (supratentorial tumors and arterial aneurysms) under conditions of induction anesthesia with propofol (diprivan) in the mean dose of 2.85 mg/kg. Diprivan alone did not block the development of pronounced pressor reaction to intubation. Combination of diprivan in the same dose with dipidolor (0.4 mg/kg) ensured complete blocking of the pressor reaction in neurosurgical patients. Combination of diprivan and fentanyl (2.85 mg/kg) blocked the development of a pressor r…

Use of sole-agent ketamine sedation to facilitate laryngoscopy and foreign body removal in acute supraglottic airway obstruction.

A 78-year-old man presented with stridor and dysphonia after choking at home. He deteriorated, with respiratory distress and an oxygen saturation of 55% despite oxygen via a non-rebreather mask. Flexible nasendoscopy demonstrated a food bolus between the epiglottis and glottis. The team judged apnoea and assisted positive-pressure ventilation before extraction to be hazardous, while deterioration and limited cooperation made an awake technique impracticable. Sole-agent ketamine sedation facilitated videolaryngoscopy with clinically observed spontaneous breathing; the bolus, a slice of carrot, …

A two-step deep learning framework for predicting difficult video laryngoscopy from ultrasound images: a prospective cohort study.

BACKGROUND: Deep learning integrated with ultrasound systems may assist in predicting difficult airway, a life-threatening complication in anesthesia. The study aimed to assess the feasibility of AI-based ultrasound for predicting difficult video laryngoscopy (DVL); and identify ultrasonographic measurements associated with DVL using AI-generated heatmaps. METHODS: Patients who underwent ultrasonographic airway examinations were enrolled. DVL were classified as Cormack-Lehane grade 3 or 4. The methodology involved two steps: In step 1, ResNet-18 extracted features from ultrasound scans, which…

Effect of bed height on laryngoscopy force and operator ergonomics during simulated endotracheal intubation: A randomized controlled study.

PURPOSE: Excessive force during laryngoscopy for endotracheal intubation can result in injury to airway soft tissues and hemodynamic stress responses. In this randomized controlled trial on simulated intubation, we aimed to evaluate the effect of bed height on laryngoscopy force and operator ergonomics. This study was registered on Clinical Research Information Service (CRIS) registry (KCT0006948). METHODS: Fifty operators with varying levels of experience were enrolled to intubate an airway mannequin at two different bed heights- anterior superior iliac spine (level A) and xyphoid process (l…

Effect of paratracheal pressure on the glottic view during direct laryngoscopy: a randomized, double blind, non-inferiority trial.

BACKGROUND: Cricoid pressure (CP) is commonly used during rapid sequence induction and intubation to prevent regurgitation and aspiration of gastric contents. However, its effectiveness and safety have been questioned. Paratracheal pressure (PP) has emerged as a potential alternative. This study aimed to compare the adverse effects of PP versus CP on the glottic view during direct laryngoscopy. MATERIAL AND METHODS: This randomized, double-blind, non-inferiority trial included 200 adult patients undergoing general anaesthesia. Participants were randomly assigned to receive either PP or CP dur…

McGrath MAC video laryngoscope versus direct laryngoscopy for the placement of double-lumen tubes: A randomised control trial.

BACKGROUND AND AIMS: Role of video laryngoscopes (VLs) in the management of difficult airway with single-lumen tubes (SLTs) is established. VLs provide improved glottis view but are associated with longer time to intubate (TTI). We aimed to compare the TTI for double-lumen tube (DLT) insertion using the McGrath® MAC VL versus direct Macintosh laryngoscope (DL). METHODS: Eleven senior anaesthesiologists experienced in SLT insertion, but not DLT insertion with VL participated. Seventy-four adults belonging to American Society of Anesthesiologists physical status I-II posted for elective surgery

Variables associated with successful intubation attempts using video laryngoscopy: a preliminary report in a helicopter emergency medical service.

BACKGROUND: Multiple studies have demonstrated varying rates of successful endotracheal intubation (ETI). Until the application of video laryngoscopy, little information regarding prehospital intubation could be analyzed objectively by individuals other than the provider performing the ETI. OBJECTIVE: To evaluate the association of variables recorded during video laryngoscopy and successful ETI attempts, defined as placing the endotracheal tube in the trachea. METHODS: We retrospectively reviewed intubations performed by a single helicopter emergency medical service (HEMS) using a video lary…

Videolaryngoscopy versus direct laryngoscopy for double-lumen endotracheal tube intubation in thoracic surgery - a randomised controlled clinical trial.

BACKGROUND: Double-lumen tube (DLT) intubation is necessary for thoracic surgery and other operations with the need for lung separation. However, DLT insertion is complex and might result in airway trauma. A new videolaryngoscopy (GVL) with a thin blade might improve the intubation time and reduce complexity as well as iatrogenic airway complications compared to conventional direct laryngoscopy (DL) for DLT intubation. METHODS: A randomised, controlled trial was conducted in 70 patients undergoing elective thoracic surgery using DLT for lung separation. Primary endpoint was time to successful…

Diagnosis of Extrinsic Upper Esophageal Compression Utilizing Video Laryngoscopy in an Infant Following Failed Transesophageal Echocardiogram Probe Placement.

Anesthesiologists are frequently responsible for placement of transesophageal echocardiography probes prior to cardiac surgery in children. A number of potential complications are possible, including placement failure. This report documents one such failed attempt at probe placement in a 3-month-old patient with a history of ventricular septal defect, and the utilization of video laryngoscopy by the anesthesiologist to diagnose a previously unknown extrinsic esophageal compression likely caused by an aberrant right subclavian artery. This case highlights the multiple vascular anomalies that ma…

Difficult Airway Characteristics Associated with First-Attempt Failure at Intubation Using Video Laryngoscopy in the Intensive Care Unit.

RATIONALE: Video laryngoscopy has overcome the need to align the anatomic axes to obtain a view of the glottic opening to place a tracheal tube. However, despite this advantage, a large number of attempts are unsuccessful. There are no existing data on anatomic characteristics in critically ill patients associated with a failed first attempt at laryngoscopy when using video laryngoscopy. OBJECTIVES: To identify characteristics associated with first-attempt failure at intubation when using video laryngoscopy in the intensive care unit (ICU). METHODS: This is an observational study of 906 cons…

Tracheobronchomalacia in pediatric patients with esophageal atresia: comparison of diagnostic laryngoscopy/bronchoscopy and dynamic airway multidetector computed tomography.

PURPOSE: Diagnostic laryngoscopy and bronchoscopy (DLB) has been the traditional preoperative diagnostic modality for evaluating presence and severity of tracheobronchomalacia (TBM), and requires anesthesia. Alternatively, multidetector computed tomography (MDCT) is potentially a noninvasive modality that provides high-resolution, 3-dimensional (3D) imaging of the thorax providing preoperative guidance for pediatric surgeons. This study compares MDCT with intraoperative DLB in the assessment of TBM in symptomatic pediatric patients with esophageal atresia (EA). METHODS: Following IRB approval…

Possible difficult laryngoscopy caused by masticatory muscle tendon-aponeurosis hyperplasia: we anesthesiologists should be aware of this disease.

Masticatory muscle tendon-aponeurosis hyperplasia (MMTAH) is a new disease entity characterized by limited mouth opening due to contracture of the masticatory muscles resulting from hyperplasia of tendons and aponeuroses. The other clinical feature is that the face of the patient with this disease displays a square mandible configuration. Muscle relaxants provide no relief for the limited mouth-opening ability. Anesthesiologists need to suspect difficult airway when patients have limited mouth opening with square mandible configuration. MMTAH can therefore be a possible cause of difficult intu
