Factors Influencing End-of-Life Decisions in Cancer Patients After Traumatic Injuries.

INTRODUCTION: Severe traumatic injuries are frequently associated with poor outcomes, and withdrawal of life-sustaining treatment (WLST) after traumatic brain injury is common. However, the association between WLST and pre-existing cancer in all trauma patients remains underexplored. METHODS: This retrospective cohort study analyzed data from the American College of Surgery Trauma Quality Programs in 2022. Patients aged 18 y or older with information on pre-existing disseminated cancer and chemotherapy status at the time of trauma were included. Descriptive statistics and multiple logistic re

Urgent Chemotherapy for Life-Threatening Complications Related to Solid Neoplasms.

OBJECTIVES: Solid neoplasms can be directly responsible for organ failures at the time of diagnosis or relapse. The management of such specific complications relies on urgent chemotherapy and eventual instrumental or surgical procedures, combined with advanced life support. We conducted a multicenter study to address the prognosis of this condition. DESIGN: A multicenter retrospective (2001-2015) chart review. SETTING: Medical and respiratory ICUs. PATIENTS: Adult patients who received urgent chemotherapy in the ICU for organ failure related to solid neoplasms were included. The modalities

Paths to recovery from substance misuse: change of lifestyle and the role of treatment.

Controlled outcome studies have, on the whole, failed to demonstrate more than rather weak and short-lived effects of substance misuser treatment. There is, at the same time, growing evidence that people often recover from substance misuse problems without the help of formal treatment. This state of affairs has, not least in times of economic recession, been taken as an argument for cutting fundings of treatment facilities and other forms of formal help. This article questions that present knowledge necessarily justifies such conclusions. We do know that even severe misusers are sometimes capa

METHODS: A cross-sectional online survey of bereaved relatives was conducted in Argentin

Current Challenges and Future Innovation in Rural Heart Failure Care: A Qualitative Analysis.

BACKGROUND: Individuals living with heart failure experience challenges managing frequent hospital admissions and life-altering symptoms, including dyspnea, fatigue, and depression. These challenges are exacerbated by the unique obstacles associated with receiving healthcare in a rural community. Their care partners often do not receive sufficient support to care for someone with heart failure and experience fatigue, social isolation, and anxiety. OBJECTIVE: Explore the insights of individuals involved in and/or receiving heart failure care at a rural academic medical center with the aim of i

Promoting the use of advance care planning in people from minority ethnic backgrounds.

Advance care planning supports decision-making autonomy for patients at the end of life. However, research has shown that the uptake of advance care planning is often low among people from minority ethnic backgrounds. This article explores the systemic and intracultural barriers experienced by people from minority ethnic backgrounds when planning care at the end of life. The author also offers a conceptual framework to support greater equity and access to advance care planning in these populations.

Investigation of hopelessness, self-care agency and life satisfaction levels in kidney transplant recipients.

This study investigated relationships between hopelessness, self-care agency, and life satisfaction among kidney transplant recipients. In this descriptive-correlational study, data were collected from 145 recipients (57.9% male; mean age 42.26 ± 13.19 years) using the Beck Hopelessness, Self-Care Agency, and Satisfaction with Life Scales. Participants exhibited mild hopelessness, good self-care agency, and high life satisfaction. Path analysis confirmed that hopelessness its sub-dimensions significantly predicted lower self-care agency and life satisfaction (p < 0.001). Furthermore, specific

[Care for dementia patients by their families and home care].

Present problems concerning home care for the aged including the following three points: 1. The extent of the period during which care is needed has been prolonged as the aged live longer than in previous times. 2. The range of need for home care has increased. 3. As a result of the increase of the individual-dependent "life support infrastructure" due to the decreased role of the family-dependent "life support infrastructure", the aged, who were more self-supporting in past, have become more dependent on care both in quality and quantity. In particular, dementia patients gradually worsen, the

"I kind of gave up on it after a while, became too hard, closed my eyes, didn't want to know about it"-adults with type 1 diabetes mellitus describe defeat in the context of low social support.

BACKGROUND: Type 1 diabetes mellitus (T1DM) is a lifelong condition that requires diligent self-management to avoid complications. Living with T1DM is a considerable challenge and the inability to follow a prescribed regimen is often termed non-compliance. However, this fails to acknowledge that for some people the barriers to glycaemic control may be insurmountable. OBJECTIVE: This qualitative study explores the structural determinants, social context and lived experience of T1DM with 17 adults to understand influences on patterns of self-care, engagement with and trust in health-care servic

Project ENABLE: a palliative care demonstration project for advanced cancer patients in three settings.

At the end of the 1990s, based on data from two major studies of end-of-life (EOL) care, the Study to Understand Prognoses and Preferences for Outcomes and Risks of Treatment (SUPPORT), and the Hospitalized Elderly Longitudinal Project (HELP), a consensus panel report documented the problems and needs of patients with cancer and other life-limiting diagnoses at end-of-life. A national program of The Robert Wood Johnson Foundation (RWJF), Promoting Excellence in End-of-Life Care, attempted to address these needs by funding demonstration projects to test various approaches to improve identified

Family Caregiver Support of Patient Self-Management During Chronic, Life-Limiting Illness: A Qualitative Metasynthesis.

Family caregivers play an integral role in supporting patient self-management, yet how they perform this role is unclear. We conducted a qualitative metasynthesis of family caregivers' processes to support patient self-management of chronic, life-limiting illness and factors affecting their support. Methods included a systematic literature search, quality appraisal of articles, data abstraction, and data synthesis to produce novel themes. Thirty articles met inclusion criteria, representing 935 international family caregivers aged 18 to 89 years caring for patients with various health conditio
