Noninvasive ventilation for patients with acute lung injury or acute respiratory distress syndrome.

Few studies have been performed on noninvasive ventilation (NIV) to treat hypoxic acute respiratory failure in patients with acute lung injury (ALI) or acute respiratory distress syndrome (ARDS). The outcomes of these patients, for whom endotracheal intubation is not mandatory, depend on the degree of hypoxia, the presence of comorbidities and complications, and their illness severity. The use of NIV as an alternative to invasive ventilation in severely hypoxemic patients with ARDS (ie, P(aO(2))/F(IO(2)) < 200) is not generally advisable and should be limited to hemodynamically stable patients…

Burden and coping strategies in families of patients under noninvasive home mechanical ventilation.

BACKGROUND: It is well documented that caregivers of patients with chronic respiratory failure under noninvasive mechanical ventilation (NIMV) are at high risk to develop depression, burden, overload and declining health over time. OBJECTIVES: The purpose of the study was to investigate the subjective and objective burden imposed on families of patients under NIMV at home and explore the coping strategies they adopt. METHODS: The study population consisted of 50 informal caregivers of patients with chronic respiratory failure under NIMV at home for at least 6 months. The burden of the famili

[Aerophagia due to noninvasive mechanical ventilation: a first manifestation of silent gastric carcinoma].

Noninvasive mechanical ventilation (NIV) techniques have proven useful in treating patients with respiratory insufficiency of various etiologies. The problems most frequently associated with this ventilatory technique are the appearance of nasal and oropharyngeal dryness, pressure sores where the nasal mask touches the skin, ocular irritation due to air leakage and epistaxis. Aerophagia appears in up to half the patients with NIV and may lead to discontinuing treatment. Drugs that accelerate gastrointestinal transit, changes in the respirator settings or changing the ventilatory modality may h

Influence of noninvasive positive pressure ventilation on inspiratory muscle activity in obese subjects.

Noninvasive positive pressure ventilation (NPPV) can improve ventilation in obese subjects during the postoperative period after abdominal surgery. Compared to nasal continuous positive airway pressure (nCPAP), NPPV was superior in correcting blood gas abnormalities both during the night-time and during the daytime in a subgroup of patients with the obesity hypoventilation syndrome (OHS). However, as it is unknown, if and to what extent NPPV can unload the respiratory muscles in the face of the increased impedance of the respiratory system in obesity, this is what was investigated. Eighteen ob

Noninvasive positive pressure ventilation: what is its role in treating acute respiratory failure?

Noninvasive positive pressure ventilation (NIPPV) is a viable option in treating appropriately selected patients with acute respiratory failure. It is often well tolerated, and it avoids endotracheal intubation with its potential complications. Moreover, gas exchange is reportedly improved. Several issues relating to the use of NIPPV are unresolved, however. The optimal interface, best ventilator mode, and patient selection criteria have not been firmly established. Also, studies are needed to compare the efficacy, safety, and cost-effectiveness of NIPPV and standard endotracheal ventilation.

Noninvasive positive-pressure ventilation in acute respiratory distress without prior chronic respiratory failure.

We evaluated the efficacy of noninvasive mechanical ventilation (NIMV) in alleviating distress and avoiding intubation in patients with de novo acute respiratory failure complicating primary medical disorders. Eleven consecutive patients with severe respiratory distress were entered. In all patients a decision to intubate on an urgent basis had been made, but NIMV could be initiated within minutes. The patients suffered from acute pulmonary edema (five), sepsis/ARDS (two), status asthmaticus (two), and severe pneumonia (two). Dyspnea score (max=10) was (+/- SD) 8.4 +.- 1.6, scale for accessory

Incidence and Risk Factors of Nasal Pressure Injuries in Neonates Receiving Noninvasive Ventilation.

Background/Objective: Nasal pressure injuries following non-invasive ventilation (NIV) have remained a common complication. Available evidence on injury severity characteristics, timing, and predictors of progression to moderate-severe injury, especially in large cohorts, is limited. The objective was to assess the incidence, characteristics and risk factors for nasal pressure injuries among neonates on NIV in a large tertiary neonatal intensive care unit (NICU). Methods: This retrospective observational study recruited all infants who experienced nasal pressure injury while on NIV from March

Sedation and Noninvasive Positive Pressure Ventilation-Where are We now? A Narrative Review.

Noninvasive positive pressure ventilation (NIV) is an important tool for treating the critically ill. Unfortunately, NIV intolerance is a potential reason why patients will fail NIV and have to be intubated and invasively mechanically ventilated. Providing sedation is a possible solution to improve intolerance. While many sedative/analgesic options exist such as dexmedetomidine, ketamine, opioids, benzodiazepines, or propofol, little research is available to guide these choices. Because of the paucity of data, there are no clinical practice guideline recommendations to assist the clinician.

Exploring upper airway and laryngeal resistance to noninvasive ventilation in healthy awake adults.

The interaction between anatomical structures, pressure, and airflow impacts the airway resistance. The airflow during noninvasive ventilation (NIV) relies on the upper airway and laryngeal patency. This study aimed to quantify the airflow resistance at these levels during NIV. In this cross-sectional study examining 10 healthy, awake adults, we established a NIV setup incorporating a continuous video-recorded transnasal laryngoscopy and simultaneous airway pressure measurement using a transducer positioned above and below the vocal folds. Airflow and mask pressure were recorded by a pneumotac…

Association between hemodynamic instability and noninvasive ventilation failure: A large multicenter observational study.

BACKGROUND: Hemodynamic instability is a relative contraindication for noninvasive ventilation (NIV). However, there is limited evidence supporting this contraindication. METHODS: This prospective multicenter observational study across 18 Chinese and Turkish hospitals enrolled acute hypoxemic respiratory failure patients receiving NIV. Hemodynamic instability was defined as requiring vasoactive agents to maintain mean arterial pressure (MAP) >70 mmHg within 24 h of NIV. Reversible instability indicated vasoactive agent discontinuation by 24 h, while irreversible instability required persisten…

A Novel Scale to Assess Humidification during Noninvasive Ventilation: A Prospective Observational Study.

OBJECTIVE: To develop a novel scale to assess humidification during noninvasive ventilation (NIV). METHODS: This study was performed in an ICU of a teaching hospital. Three ICU practitioners with more than 10 years of clinical experience developed an oral humidification scale with a range of 1-4 points. Each studied the current literature on humidification and examined 50 images of mouths of NIV patients with different levels of humidification. Then, through discussion, a consensus scale was developed. Next, 10 practitioners and 33 NIV patients were recruited to validate the scale. Finally, t…

Long-term home noninvasive ventilation (LTHNIV) in restrictive thoracic diseases: the Italian snapshot.

Long-term home noninvasive ventilation (LTHNIV) in restrictive thoracic diseases was explored via the recently published international REINVENT ERS survey. The Italian subset of respondents (ITA-r), the highest above all participating nations, was analyzed and compared to non-Italian respondents (NO-ITA-r). The ITA-r represented 20% of the total answers examined. Ninety-four percent were physicians, whose half worked in a respiratory ICU (RICU). ITA-r mainly worked in community hospitals vs NO-ITA-r who are largely affiliated with university hospitals (p<0.0001). Amyotrophic lateral sclerosis …

[Noninvasive ventilation and positional therapy in COVID-19 : Case report and literature review].

If noninvasive ventilation (NIV or high-flow CPAP) fails in severe cases of COVID-19, escalation of treatment with orotracheal intubation and intermitted prone positioning is provided as standard care. The present case reports show two COVID-19 patients with severe refractory hypoxemia despite NIV treatment during the first wave (first half year 2020) and the resulting influence on the treatment regimen during the second wave (since October 2020) of the pandemic. Both patients (aged 63 years and 77 years) voluntarily positioned themselves on the side or in a prone position without prior sedati…

The intensive care medicine research agenda for airways, invasive and noninvasive mechanical ventilation.

In an important sense, support of the respiratory system has been a defining characteristic of intensive care since its inception. The pace of basic and clinical research in this field has escalated over the past two decades, resulting in palpable improvement at the bedside as measured by both efficacy and outcome. As in all medical research, however, novel ideas built upon observations are continually proposed, tested, and either retained or discarded on the basis of the persuasiveness of the evidence. What follows are concise descriptions of the current standards of management practice in re
