KTP laser stapedotomy with a self-crimping, thermal shape memory Nitinol piston: follow-up study reporting intermediate-term hearing.

The aim of this study was an evaluation of the mid-term hearing results after the implantation of a self-crimping heat memory Nitinol piston in stapes surgery. The 12-month postoperative results were compared with those at a minimum of 3 years (maximum 6.7, average 4.4 years). The medical records of all 44 patients who underwent surgery with a Nitinol piston for stapes fixation between November 2005 and January 2007 were evaluated retrospectively. The prostheses used in all cases measured either 4.5 or 4.75 × 0.6 mm. We hypothesized that the 12-month postoperative hearing results would be perm

Acoustically functional 3D‑printed middle ear model for prosthesis development and otosurgical training.

BACKGROUND: Cadaver temporal bones are traditionally used in otosurgical training and research, but they have many limitations. The development of novel individualized middle ear prostheses requires easy and durable artificial middle ear models that are as realistic as possible anatomically, functionally, and acoustically. The aim of this study was to create an acoustically and mechanically functional, three-dimensionally (3D) printed middle ear model that overcomes the challenges of cadaver temporal bones while promoting prosthesis development. METHODS: Structures from microcomputed tomograp

Simultaneous true stapes fixation and bilateral bony dehiscence between the internal carotid artery and the apex of the cochlea: the ultimate pitfall.

BACKGROUND: A dehiscence of a semicircular canal can mimic the audiologic characteristics of otosclerosis. OBJECTIVE: To present a unique case report of a patient presenting with stapes fixation due to otosclerosis but persisting conductive hearing loss after uneventful laser-assisted stapedotomy with interposition of an àWengen titanium clip stapes prosthesis. Eventually, a bilateral bony dehiscence between the apical turn of the cochlea and the internal carotid artery was eventually identified, which explained the persisting postoperative conductive hearing loss. A dehiscence of the semicir

Audiological outcome after stapes surgery in relation to prosthesis type.

PURPOSE: Different techniques are used to fix crimp and CliP® Piston stapes prostheses to the long process of the incus (LPI). The CliP® Piston provides a stiff connection in contrast to the static bended loop of the crimp prosthesis, which imitates the physiological incudostapedial joint (ISJ) and thereby potentially leads to different hearing outcome. METHODS: In a retrospective single-center study of German-speaking one hundred and ninety patients who underwent stapes surgery CliP® Piston or crimp prostheses between the years of 2016 and 2019 by the same surgeon and in the same setting. Ossiculoplasty with presculpted banked cartilage.

The use of self-stabilizing, presculpted cartilage homografts for middle ear reconstruction has an established track record with long-term results equivalent to reconstruction methods using alloplastic material. This method affords several advantages over other techniques, which will be discussed in detail.

Wideband acoustic immittance measurements in assessing crimping status following stapedotomy: A temporal bone study.

OBJECTIVE: To ascertain if wideband acoustic immitance (WAI) measurements are useful in assessing crimping status following stapedotomy. DESIGN: WAI measurements were obtained using the Mimosa Acoustics HearID system. Wideband chirp sound stimuli and a set of tone stimuli for nine frequencies between 0.2 and 6 kHz were used at 60 dB SPL. Five sets of measurements were performed on each temporal bone: mobile stapes, stapes fixation and stapedotomy followed by insertion of a tightly crimped, a loosely crimped and an uncrimped prosthesis. STUDY SAMPLE: Eight fresh-frozen temporal bones were har

[Replacement of ear ossicles with titanium prostheses].

BACKGROUND: Titanium has been a well established implant material for many years. New material processing techniques now permit the manufacture of small implants for ossicular chain reconstruction. METHODS: Between November 1994 and September 1995, 100 titanium middle ear implants (55 PORP, 45 TORP) were used for reconstruction of the ossicular chain. A range of five different sizes for partial and total protheses suits all implantation needs. The shape of the implants can be altered by bending. Time consuming intraoperative shaping and trimming is avoided. RESULTS: At a follow-up time of th

Innovative Categorization and Operative Management of Auditory Ossicle Disruption Following Trauma: Therapeutic Efficacy and Interventional Paradigms.

Currently, there is no consensus on the treatment protocol for ossicular chain trauma. This study aims to investigate the classification and treatment strategies for traumatic ossicular chain dislocation. We retrospectively analyzed 15 patients. Traumatic ossicular chain dislocations were categorized based on the location of trauma identified during surgery: Type I-ossicular trauma without stapediovestibular dislocation; Type II-stapediovestibular dislocation (with or without associated incus dislocation). Of the 10 patients with Type I trauma, 9 experienced head trauma, and 1 had a penetratin

Visualisation of passive middle ear implants by cone beam and multi-detector computed tomography: a comparative in vitro study.

OBJECTIVES: Modern passive middle ear titanium prostheses are filigree structures, resulting in poorer depiction on CT compared to prostheses used in the past. We compared the visibility of newer prostheses on cone beam CT (CBCT) to multi-detector CT (MDCT) with standard and lower dose in vitro, and analysed image noise and metal artefacts. METHODS: Six different titanium middle ear prostheses (three partial and one total ossicular replacement prostheses, two stapes prostheses) were implanted twice in formalin-fixed head specimens-first correctly and then with displacement. Imaging was perfor

Preliminary Model for the Design of a Custom Middle Ear Prosthesis.

HYPOTHESIS: Custom prostheses could be used to recreate the ossicular chain and improve hearing. BACKGROUND: Ossicular discontinuity or fixation occurs in 55% of cases of conductive hearing loss, with most cases involving the incus. Reconstruction has been achieved by a variety of methods; however, there has been little improvement in hearing outcomes in decades. METHODS: Precise measurements of anatomic dimensions, weight, and center of gravity were taken from 19 cadaveric incudes. These measurements were combined with measurements from the medical literature and micro-computed tomography (
