Typology of post-traumatic stress disorder in children and adolescents.

Results obtained from clinical and psychopathological studies of 161 patients with post-traumatic stress disorder (PTSD) aged 3-18 years with symptom durations from one month to four years are presented. Four major clinical variants of the type of PTSD were identified in children and adolescents: insomniac( 34.8%), phobic (23.6%), psychopathic-like (21.7%), and asthenic-depressive (19.9%). These variants, differing in terms of course and prognosis, occurred at different frequencies in different age and ethnic groups; there were no gender-related differences.

Follow-up study of patients with panic disorder and agoraphobia with panic attacks treated with tricyclic antidepressants.

One hundred and seven patients with panic disorder or agoraphobia with panic attacks were studied 1-4 years after treatment with a tricyclic antidepressant. At follow-up more than 80% of the patients remained symptomatic but fewer than half were experiencing panic attacks and only 40% were avoiding phobic situations. Although patients with agoraphobia were more severely ill and had been ill longer than those with panic disorder, their response to tricyclics and eventual outcome was similar to that for patients with panic disorder. Panic and agoraphobic patients who had the most severe symptoms

The structure of posttraumatic psychopathology in veterans attending primary care.

This study attempted to extend research indicating that posttraumatic stress disorder (PTSD) factors of Re-experiencing, Avoidance and Hyperarousal are more related to Fear/phobic disorders, while PTSD Dysphoria is more related to Anxious-Misery disorders. Trauma exposure, PTSD and comorbidity data for 668 veteran patients were analysed using confirmatory factor analyses and relative strengths of the relationships between PTSD factors and the Fear and Anxious-Misery factors were assessed. Combining Simms, Watson, and Doebbeling's (2002) model of PTSD symptoms and Krueger's (1999) Fear/Anxious …

The biological basis of anxiety. An overview.

The DSM-III divides anxiety disorders into two broad categories, Phobic Disorders and Anxiety States. Anxiety states characterised by panic attacks have been separated from generalised anxiety disorders. While this classification may not be generally accepted it is of heuristic value. Delineation of panic disorder as a distinct diagnostic entity has led to renewed efforts to identify a biological cause for the sudden severe somatic and psychological symptoms experienced by these patients. A review of evidence for the involvement of the major neurotransmitter systems is presented. Systematic in…

But what about the Empress of Racnoss? The allocation of attention to spiders and Doctor Who in a visual search task is predicted by fear and expertise.

If there is a spider in the room, then the spider phobic in your group is most likely to point it out to you. This phenomenon is believed to arise because our attentional systems are hardwired to attend to threat in our environment, and, to a spider phobic, spiders are threatening. However, an alternative explanation is simply that attention is quickly drawn to the stimulus of most personal relevance in the environment. Our research examined whether positive stimuli with no biological or evolutionary relevance could be allocated preferential attention. We compared attention to pictures of spid

Screening for social phobia in medical in- and outpatients with the German version of the Social Phobia Inventory (SPIN).

OBJECTIVE: To evaluate the German version of the Social Phobia Inventory (SPIN) as a screening device and to report corresponding cut-off scores for different populations. METHOD: In Study 1, 2043 subjects from a representative sample completed the SPIN. Cut-off values were established on the basis of means and standard deviations. In Study 2, different aspects of validity were examined in a clinical sample comprising 164 subjects, including social phobic individuals, individuals with other anxiety disorders and depression, and non-clinical control subjects. Internal consistency was evaluated

[Social phobia with interpersonal attitude delusions on the background of endogenous diseases].

The paper presents data on 38 patients with slow-progredient schizophrenia and cyclothymia with clinical picture of social phobias combined with sensitive ideas of reference. Two types of such states were singled out: social phobia of self-insufficiency with isolated sensitive ideas of reference (13 patients) and social phobia with the ideas of physical or mental defect and generalized ideas of reference (25 cases). The states of the first type were observed in the structure of circular melancholic depressions and were characterised by the presence of the ideas of low self-esteem. Phobic avoid

Anticipatory anxiety and avoidance in panic disorder with agoraphobia.

The present study utilized the responses of 34 patients with panic disorder with agoraphobia to investigate the occurrence and anticipation of panic attacks in relation to the avoidance of specific situations from the Fear Questionnaire [Marks & Mathews (1979) Behaviour Research and Therapy, 17, 263-267]. Results indicated that self-reports of avoidance of specific situations were often significantly correlated with the anticipation of panic but rarely with the occurrence of panic. The occurrence and anticipation of panic were also frequently associated with social phobic situations in additio

Behavioural and combined therapy in panic states.

This review will focus only on those paradigms for treatment of panic states with agoraphobia. Until recently the behaviour therapy literature has completely ignored panic disorder and has focused exclusively on the agoraphobic aspect of this syndrome. To summarize the behavioural therapy of panic disorder and agoraphobia, it appears that exposure is in the short run the most effective behavioural paradigm in agoraphobia. However, when contrasted with cognitive approaches it does not appear to be as effective in the treatment of panic disorder. In conclusion, it is unclear whether we can speak

Clinical and neurobiological effects of aerobic exercise in dental phobia: A randomized controlled trial.

BACKGROUND: Physical activity has shown to be effective in anxiety disorders. For specific phobia, no studies are available that systematically examined the effects of an aerobic exercise intervention on phobic fear within a randomized-controlled design. Therefore, we investigated the acute effect of a standardized aerobic training on clinical symptoms of dental phobia as well as on stress-related neurobiological markers. METHODS: Within a crossover design, 30 patients with dental phobia (mean age: 34.1 years; mean score of the Dental Anxiety Scale: 18.8) underwent two minor dental interventi

Recognition of irrationality of fear and the diagnosis of social anxiety disorder and specific phobia in adults: implications for criteria revision in DSM-5.

BACKGROUND: In DSM-IV, the diagnosis of social anxiety disorder (SAD) and specific phobia in adults requires that the person recognize that his or her fear of the phobic situation is excessive or unreasonable (criterion C). The DSM-5 Anxiety Disorders Work Group has proposed replacing this criterion because some patients with clinically significant phobic fears do not recognize the irrationality of their fears. In the present report from the Rhode Island Methods to Improve Diagnostic Assessment and Services project we determined the number of individuals who were not diagnosed with SAD and spe

[Phobic orthostatic insecurity].

BACKGROUND AND OBJECTIVES: The authors attempt to expand knowledge about a subjective balance disorder they have called phobic orthostatic insecurity, a condition representing the second cause of medical visits (22.3 %) to their ENT and neuro-otology clinic, and attempt to identify relationships with similar conditions described in psychiatry (agoraphobia, somatoform vertigo, and space-phobia) and in neurology (phobic postural vertigo). They also propose a simple diagnostic method and present their therapies and results. PATIENTS AND METHOD: A total of 151 patients with an indefinite symptoma

[Dizziness and anxiety disorders in the elderly].

Perhaps each dizzy patient feels anxiety. Varying emotional disorders are particularly common in elderly patients with dizziness. The causes of the frequent concurrence of dizziness and mental disorders (anxiety and depression) are diverse. Amongst these there are two chief conditions: 1) vestibular vertigo may cause anxiety to a greater extent than many other symptoms; 2) anxiety and depression are themselves frequently manifested by the sensations resembling vestibular ones that patients are inclined to call dizziness. On the contrary, anxiety may appear in some cases as the sensations resem

The Zurich study. XX. Social phobia and agoraphobia.

The problems in association with agoraphobia and social phobia were examined in an 11-year prospective longitudinal study of a Swiss cohort of young adults. The weighted prevalence rates according to DSM-III were 2.9% for agoraphobia and 3.8% for social phobia. Although the problem of agoraphobia was greater in females, an equal sex ratio was observed for social phobia. There was a significant degree of comorbidity between the two subtypes of phobia, with females exhibiting a significantly greater frequency of co-occurrence of both disorders than males. The course of the two disorders was quit

Social phobia.

Social anxiety disorder (social phobia) is an irrational fear of being observed and judged by other people in various social settings. The individual is afraid that he or she will act in a way that will be humiliating or embarrassing. It is often a chronic, disabling condition that is characterized by a phobic avoidance of most social situations. Social anxiety disorder is the most frequent anxiety disorder (10-15%) that occurs in two subtypes--generalized and specific. It is a disorder that occurs during the adolescence and reflects negatively to the quality of life of an individual. Neurobio

BACKGROUND AND OBJECTIVE: Even in young adult age, psychological disorders are highly prevalent. Family doctors and gynaecologists are the p … iagnosed somatoform disorders in 28 % of the young women (F-DIPS: 3, 8 %). The F- DIPS found mostly phobic disorders (29 %) (claimcards: 6,1 %). A disorder-specific therapy was only rarely initiated. The treatment (psychotherapy and/or drug therapy) of women with psychiatric disorders appeared to be insufficient. CONCLUSION: The study indicates that primary-care …

A comparison of publication trends on avoidant personality disorder and social phobia.

The objective of this study was to ascertain the number of articles published per annum on the topics of avoidant personality disorder (APD) and social phobia (SP) in the period from 1973 to 2001. We hypothesized that while annual publication rates on SP would exhibit a sound growth, the number of scientific articles on APD published per annum would present a stagnant or declining trend. We performed a comprehensive literature review on APD and SP using the three largest existing databases for medical and psychological journals: MEDLINE, PsycINFO, and Web of Science. The references were gather
