Awareness of Thyroid Diseases and Their Risk Factors Among the Residents of Jeddah, Saudi Arabia.

Background The thyroid gland is responsible for regulating many aspects of body functions. Despite their global prevalence, thyroid disorders often go underdiagnosed, which can lead to serious health complications. In Saudi Arabia, the overall prevalence was 49.76%, among which subclinical hypothyroidism was the most prevalent type. Raising awareness and knowledge about thyroid diseases and their risk factors is essential for the prevention and early treatment of these disorders. Aim and objectives To assess the awareness of thyroid diseases and their risk factors among the residents of Jeddah

Multiple Nutritional Factors and the Risk of Hashimoto's Thyroiditis.

BACKGROUND: Hashimoto's thyroiditis (HT) is considered to be the most common autoimmune disease. It is currently accepted that genetic susce … at selenium/selenoproteins can reduce thyroid peroxidase (TPO)-antibody titers, hypothyroidism, and postpartum thyroiditis. Iron deficiency impairs thyroid metabolism. TPO, the enzyme responsible for the production of thyroid hormones, is a heme (iron-containing) enzyme which becomes active at the apical surface of thyrocytes only after binding heme. HT patients are fre …

The iodide perchlorate discharge test in women with previous post-partum thyroiditis: relationship to sonographic appearance and thyroid function.

BACKGROUND: Post-partum thyroid disease occurs in 50% of anti-thyroid peroxidase (TPO) antibody positive women (detected at 16 weeks' gestation) and is characterized by a transient episode of hyper, hypo or hyper-hypothyroidism. In approximately 20% of these women the hypothyroidism is permanent. However, the extent of long-term thyroid dysfunction, possibly mediated by immune attack, in those anti-TPO Ab + ve women who have had only transient or no thyroid dysfunction during the postpartum period is not clear. OBJECTIVE: We have therefore studied the frequency of iodide organification defect

Antithyroid drugs during breastfeeding.

Antithyroid drugs (ATDs) are widely used for the treatment of Graves' disease (GD) in the general population. Over the past decade, there has been an increasing awareness that several disturbances of thyroid function may occur in mothers after delivery which may be more prevalent than previously appreciated. Exacerbation of immune reactions occurs 3-12 month following delivery. Management of hyperthyroidism during lactation requires special considerations and should be implemented to prevent any adverse outcomes in mother and neonate. Continuation of breastfeeding is safe and should be encoura

Postpartum Hypophysitis: A Case Report and a Literature Review.

Hypophysitis is a rare pituitary gland disease primarily seen in females of reproductive age. Patients can present with various non-specific symptoms, which makes diagnosis challenging. Appropriate endocrine workup supplemented with magnetic resonance imaging (MRI) helps establish a diagnosis. We present a case of a 22-year-old postpartum female who came with nausea, vomiting, and abdominal pain. Global endocrine insufficiency was seen in the laboratory workup, and an MRI confirmed the diagnosis of hypophysitis. She was treated with steroid and thyroid hormone supplementation.

Lower Serum 25-Hydroxyvitamin D Level is Associated With 3 Types of Autoimmune Thyroid Diseases.

Autoimmune thyroid diseases (AITD) are common autoimmune disorders. A few studies have analyzed the association between serum vitamin D levels and AITD, and available data remain inconclusive. The aim of this study was to evaluate the association between serum vitamin D levels and 3 types of AITD, that is Graves' disease (GD), Hashimoto's thyroiditis (HT), and postpartum thyroiditis (PPT). Two independent case-control studies were designed. The first is a cross-sectional case-control study in which we examined the levels of 25(OH)D in patients with newly diagnosed GD or HT and in controls; the…

Thyroid hormone status of newborns in relation to in utero exposure to PCBs and hydroxylated PCB metabolites.

The associations between in utero exposure to polychlorinated biphenyls (PCBs) or hydroxylated PCB metabolites (OH-PCBs), and free thyroxin (fT4) or thyroid-stimulating hormone (TSH) status in the newborn were investigated as a pilot study of a large-scale epidemiologic study on in utero PCB or OH-PCB exposure and thyroid function of the newborns. Umbilical cord tissue was used as the media for the biological monitoring of PCBs/OH-PCBs exposure in utero. For the measurement of fT4 and TSH, a heel-prick blood sample spotted on filter paper, which is called Guthrie card, is collected from each n

Treatment of thyroid disease in pregnancy.

With appropriate therapy, complications related to thyroid disease in pregnancy can be minimized. Although the diagnosis of thyroid endocrinopathy may be difficult in pregnancy, few therapies are contra-indicated. Because medications may cross the placenta, however, clinicians need always to be mindful of potential fetal effects and should work to use the minimal dose necessary to achieve maternal euthyroidism. Thyroid function tests, in particular free T4 and TSH, remain good measures of thyroid function and therapy in pregnancy.

The Dynamic Changes in Maternal Thyroid Parameters Across the Three Trimesters and Their Differential Effects on the Occurrence of Adverse Obstetric Outcomes.

OBJECTIVE: Thyroid parameters undergo significant dynamic changes during pregnancy. This study aimed to comprehensively analyze the impact of abnormal thyroid parameters in each trimester on the incidence of common adverse obstetric outcomes. METHODS: Blood samples drawn for thyroid parameters in each trimester during the antenatal period were determined after the participants gave birth. Serum thyrotropin, free thyroxine, free triiodothyronine, anti-thyroid peroxidase antibody (TPOAb), and anti-thyroglobulin antibody (TgAb) levels were tested using electrochemiluminescence immunoassays. Dietary Iodine Sufficiency and Moderate Insufficiency in the Lactating Mother and Nursing Infant: A Computational Perspective.

The Institute of Medicine recommends that lactating women ingest 290 μg iodide/d and a nursing infant, less than two years of age, 110 μg/d. The World Health Organization, United Nations Children's Fund, and International Council for the Control of Iodine Deficiency Disorders recommend population maternal and infant urinary iodide concentrations ≥ 100 μg/L to ensure iodide sufficiency. For breast milk, researchers have proposed an iodide concentration range of 150-180 μg/L indicates iodide sufficiency for the mother and infant, however no national or international guidelines exist for breast m

Epitope mapping of tsh receptor-blocking antibodies in Graves' disease that appear during pregnancy.

Spontaneous remission of Graves' disease during pregnancy is thought to be due to a reduction of thyroid-stimulating antibody activity. We suspected, however, that a broader change in TSH receptor antibody characteristics might play an important role in modulating disease activity during pregnancy. We measured TSH binding inhibitory Ig, thyroid-stimulating antibody, and thyroid stimulating-blocking antibody activities in 13 pregnant Graves' disease patients at first, second, and third trimesters and 4 months postpartum. To measure and epitope-map thyroid-stimulating antibody and thyroid stimul

A critical period for the role of thyroid hormone in development of renal alpha-adrenergic receptors.

Adrenergic input influences renal cell replication/differentiation and the development of excretory function. Kidney cells make adrenoceptors before the arrival of the majority of nerve terminals, and the current study examines whether thyroid hormone plays a role in receptor development. Propylthiouracil (PTU) was given to pregnant and neonatal rats from gestational d 17 through postnatal d 5, a treatment that obtunds thyroid hormone levels throughout the first 2-3 wk postpartum. The PTU group showed significant deficits in the number of alpha1-receptors, and values resolved to normal in para

Status of anti-thyroid peroxidase during normal pregnancy and in patients with hyperemesis gravidarum.

Autoimmune thyroid diseases (AITD) comprising Hashimoto's thyroiditis, primary myxedema, and Graves' disease are associated with autoantibodies directed against thyroglobulin and thyroid peroxidase (anti-TPO). Anti-TPO occur in 10% of pregnant women, half of whom reportedly develop postpartum thyroid dysfunction. We recently published data on the thyroid function reference ranges in pregnant Chinese but the AITD status of our cohort was unknown. In view of this missing information we have measured anti-TPO in specimens from our cohort stored at -80 degrees C, and compared these to those of pat

Proteomic analysis of milk fat globule membrane proteins in mature human milk of women with and without gestational diabetes mellitus.

Milk fat globule membrane proteins (MFGMP) in human milks have positive effects on infant's health. As gestational diabetes mellitus (GDM) causes variations in MFGMP, it is essential to understand the effects of GDMon MFGMP. This study aims to investigate and compare the MFGMP (>3 months postpartum) of GDM and non-GDM (NGDM) women using four-dimensional-data-independent-acquisition proteomics technology. Principal component analysis shows significant differences in the MFGMP of GDM and NGDM women. A total of 4747 MFGMP were identified in maturehuman milk of GDM and NGDM women. Among these prot

A Narrative Review on the Effect of Maternal Hypothyroidism on Fetal Development.

The thyroid is a butterfly-shaped gland located in the human body's neck region. The thyroid produces three hormones that are essential for regulating body temperature, energy production, weight, hair and nail growth, and menstrual cycle maintenance. The production of these hormones is controlled by a feedback mechanism. Various factors cause changes in the stimulation and inhibition of these hormones, which ultimately causes either excessive release or a decrease in the levels of thyroid hormones. These causes can be physiological or pathological. One of the physiological causes is pregnancy.

Stroke in pregnancy and postpartum: Etiology, incidence, and risk factors in a tertiary care cohort.

BACKGROUND: Stroke during pregnancy and the puerperium is rare but potentially devastating, contributing substantially to maternal morbidity, mortality, and adverse fetal outcomes. Data on incidence, etiology, and modifiable risk factors in low- and middle-income countries remain limited. METHODS: We retrospectively reviewed medical records of women ≥ 18 years admitted with ischemic stroke (arterial or cerebral venous thrombosis) or hemorrhagic stroke during pregnancy or up to 12 weeks postpartum at Harran University Hospital between January 2013 and October 2024. Clinical features, neuroimag

Minimizing neonatal hypothyroidism induced by lithium exposure through breast milk.

BACKGROUND: Lithium-induced hypothyroidism in the neonate is a growing concern for lactating mothers. Maternal hypothyroidism in the postpartum period could lead to hypothyroidism in the infant via maternal compromised thyroid hormones (likely T4) in breast milk, and lithium in breast milk could have a direct effect on the neonatal thyroid axis over lithium carbonate direct administration. METHODS: We have studied the effects of lithium exposure on neonatal pups through two different modes of exposure: direct oral administration of lithium carbonate and indirect exposure of lithium from breas
