Late onset dementia with argyrophilic grains and subcortical tangles or atypical progressive supranuclear palsy?

Most clinically demented elderly patients are found at autopsy to have Alzheimer's disease, multi-infarct dementia, Parkinson's disease, Pick's disease, or Creutzfeldt-Jakob disease. We studied 5 patients clinically characterized by late onset dementia whose brains showed no pathological evidence of Alzheimer's disease, or any other specific neuropathological diagnosis. We found argyrophilic grains, coiled bodies, abundant Alz-50-positive and thioflavine S-negative neurofibrillary tangles, and neuropil threads in the hippocampus, entorhinal cortex, locus ceruleus, substantia nigra, subthalamic

No Evidence of a Contribution of the Vestibular System to Frequent Falls in Progressive Supranuclear Palsy.

BACKGROUND AND PURPOSE: Conflicting results about vestibular function in progressive supranuclear palsy (PSP) prompted a systematic examination of the semicircular canal function, otolith function, and postural stability. METHODS: Sixteen patients with probable PSP [9 females, age=72±6 years (mean±SD), mean disease duration=3.6 years, and mean PSP Rating Scale score=31] and 17 age-matched controls were examined using the video head impulse test, caloric testing, ocular and cervical vestibular evoked myogenic potentials (o- and cVEMPs), video-oculography, and posturography. RESULTS: There was

The PSP subcortical atlas - a new neuroimaging atlas for progressive supranuclear palsy Richardson's syndrome.

OBJECTIVES: The severe cerebral atrophy patterns of progressive supranuclear palsy (PSP) create challenges in clinical neuroradiology practice and neuroimaging research as often, subcortical regions are not accurately segmented with existing atlases or require labour intense manual segmentation. We aimed to create a PSP specific neuroimaging atlas. METHODS: Seven subcortical regions were manually segmented on Quantitative Susceptibility Mapping (QSM) images of 33 PSP Richardson's Syndrome (PSP-RS) subjects and used to automatically segment 10 validation subjects using the ANTs Joint Label Fus

Glial fibrillary acidic protein in progressive supranuclear palsy: A systematic review and meta-analysis.

BACKGROUND: Astrocyte pathology is a defining feature of progressive supranuclear palsy (PSP), yet the relevance of fluid glial fibrillary acidic protein (GFAP) remains uncertain. OBJECTIVE: To determine whether mean GFAP concentrations differ between PSP and healthy controls or other parkinsonian disorders, particularly Parkinson's disease (PD) and multiple system atrophy (MSA). METHODS: PubMed and Scopus were searched from inception to 24 January 2026. Original human studies that measured GFAP in plasma, serum, or cerebrospinal fluid (CSF) and included a PSP group were eligible. Standardiz

Annual percentage change of MR Parkinsonism index in progressive supranuclear palsy: a feasibility study.

OBJECTIVES: Considerable evidence suggests that midbrain-based magnetic resonance Parkinsonism index (MRPI) measurements are reliable biomarkers for the diagnosis of progressive supranuclear palsy (PSP). However, the longitudinal atrophy pattern of PSP and potential differences in change rates among PSP phenotypic spectrum remain unclear. This study aims to investigate the longitudinal changes of MRPI measurements and explore their potential role in PSP phenotype progression monitoring. MATERIALS AND METHODS: Thirty-six patients with PSP-Richardson's syndrome (PSP-RS), 21 patients with varian

Longitudinal assessment of white matter alterations in progressive supranuclear palsy variants using diffusion tractography.

INTRODUCTION: White matter (WM) tract degeneration is a characteristic feature of progressive supranuclear palsy (PSP), with longitudinal changes observed in PSP-Richardson's syndrome (PSP-RS). Little, however, is known about the other PSP variants. We assessed cross-sectional and longitudinal WM degeneration across PSP variants using diffusion tractography. METHODS: Forty-eight PSP patients were recruited by the Neurodegenerative Research Group, Mayo Clinic, and underwent two diffusion MRI, 1-year apart. We measured fractional anisotropy and mean diffusivity from eight WM tracts reconstructe

Brain Microstructure Interrogation by Diffusion Tensor and Kurtosis Imaging in Progressive Supranuclear Palsy Subtypes.

BACKGROUND AND PURPOSE: Diffusion tensor imaging (DTI) is commonly used to assess the integrity of gray and white matter (WM) structures in progressive supranuclear palsy (PSP). Beyond DTI, nontraditional diffusion techniques such as diffusion kurtosis imaging (DKI) have been shown to characterize brain tissue further. In this work, we aim to determine the utility of DKI in the differential diagnosis of PSP-Richardson syndrome (PSP-RS) and PSP with predominant parkinsonism (PSP-P) from Parkinson's disease (PD) and controls. METHODS: A multishell diffusion-weighted sequence was acquired at 3 T

Increased 18 F-AV1451 Retention in Soft Tissue of Patients With Progressive Supranuclear Palsy.

We report 18 F-AV1451 PET/MRI findings of a 56-year-old man with progressive supranuclear palsy (PSP). The images revealed multifocal 18 F-AV1451 tracer uptake in both intracranial and extracranial regions, with increased retention in bilateral basal ganglia regions and scalp soft tissue. The findings may reflect systemic tau pathology and provide the clinical experience for the possible assessment of the disease severity.

Progressive Supranuclear Palsy With Post-traumatic Frontal Lobe Damage Mimicking Anti-IgLON5 Antibody Disease.

Phenotypic overlap between autoimmune encephalitis and movement disorders may rarely occur. We present a patient with progressive gait disturbance, vertical supranuclear gaze palsy, cognitive decline, and daytime somnolence, as well as extremity chorea. He was initially suspected of having autoimmune encephalitis. Antibody testing was negative in serum and cerebrospinal fluid. A fall leading to subdural hemorrhage and bilateral post-traumatic frontal lobe changes likely led to his bilateral upper and lower extremity involuntary movements. His mental status improved after lumbar puncture. His c

Levodopa modulates semantic fluency and uniqueness in non-demented patients with progressive supranuclear palsy.

INTRODUCTION: Semantic fluency is the ability to name items from a given category within a limited time, which relies on semantic knowledge, working memory, and executive function. Similar to patients with Parkinson's disease (PD), patients with progressive supranuclear palsy (PSP) scored lower than healthy adults in the well-established semantic fluency test. However, it is unclear how unique are the produced words. This study examined the relationship between semantic fluency and words' uniqueness in patients with PSP. METHODS: Twenty-seven patients with PSP Richardson's syndrome (PSP-RS),

Is there a spinal tap responder in progressive supranuclear palsy? The first prospective study.

OBJECTIVE: Progressive supranuclear palsy (PSP) is a progressive neurodegenerative disease, and sometimes shows idiopathic normal pressure hydrocephalus (iNPH)-like presentations. We aimed to evaluate spinal tap responsiveness in patients with PSP, including the effect of sham spinal tap. METHODS: Eleven patients with PSP, ten with probable/definite iNPH, and eight control patients were prospectively enrolled. All participants underwent sham spinal tap and spinal tap procedures. Gait was evaluated using wearable inertial sensors. We defined "tap responders" as individuals with a 10% or more i

Correlation between the regional brain volume and glymphatic system activity in progressive supranuclear palsy.

INTRODUCTION: Tau protein accumulation in the brain is thought to be one of the causes of progressive supranuclear palsy (PSP). The glymphatic system was discovered a decade ago as a waste drainage system in the brain that promotes the elimination of amyloid-beta and tau protein. We here evaluated the relationships between glymphatic system activity and regional brain volumes in PSP patients. METHOD: Subjects were 24 patients with PSP and 42 healthy participants who underwent diffusion tensor imaging (DTI). We computed the diffusion tensor image analysis along the perivascular space (DTI‑ALPS
