Residual Surgical Left Atrial Appendage Leaks: Thromboembolic Risk and Anatomy-Tailored Percutaneous Closure Strategies.

BACKGROUND: Atrial fibrillation is a leading cause of cardioembolic stroke, and the left atrial appendage (LAA) is the principal source of thrombus formation in non-valvular disease. Surgical LAA exclusion is commonly performed during cardiac surgery, but incomplete closure resulting in a residual communication is not uncommon and may carry ongoing thromboembolic risk. Unlike peri-device leaks after transcatheter closure, residual surgical leaks are anatomically and mechanistically distinct, typically arising through a central, fibrotic ostium with a variable residual pouch, and no validated m

Percutaneous closure of paravalvular leak after Cone repair for Ebstein's anomaly.

We report the use of an Amplatzer Vascular plug for percutaneous closure of a paravalvular leak after Cone repair in a 5-year-old boy with Ebstein's anomaly. A paravalvular leak of the tricuspid valve developed gradually after Cone repair-surgery. The combination of fluoroscopy, transesophageal and transthoracic imaging during general anesthesia was necessary for correct and safe positioning of the device by percutaneous approach. The results were promising.

Low-profile KA microplug set for transarterial occlusion of patent ductus arteriosus in a small dog: First experience in interventional cardiology.

BACKGROUND: Cardiologists occlud most patent ductus arteriosus (PDA) defects in dogs using the Amplatz® canine duct occluder via a transarterial approach. However, this approach can be problematic in small dogs due to their small femoral artery diameters. In such cases, cardiologists have opted for lower profile devices or use coils or vascular plugs deployed using a more challenging transvenous approach. CASE DESCRIPTION: The present report describes a 1-year-old, 6 kg male Cavalier King Charles Spaniel with a small PDA, which was successfully occluded using the Micro Plug Set (KA Medical, M

Percutaneous paravalvular leak closure: clinical outcomes and practical insights from a single-center experience in Japan.

While transcatheter paravalvular leak (PVL) closure offers a viable alternative to reoperation in high-risk patients, data from Japan remain limited. To address this, we retrospectively analyzed 28 consecutive patients who underwent percutaneous PVL closure at a single center in Japan between 2014 and 2022 (mitral, n = 23, aortic, n = 5; median follow-up, 2.0 years). We herein discuss real-time transesophageal echocardiography (TEE) with three-dimensional imaging, guided defect characterization, device sizing, and intra-procedural assessment. The study’s primary clinical endpoint was all-cause

The First Reported Transcatheter Closure of Patent Ductus Venosus Using the LifeTech™ Konar-MFO VSD Occluder in an Infant with Hyperammonemia.

INTRODUCTION: Congenital portosystemic shunts (CPSS) are rare vascular anomalies that can lead to hyperammonemia and liver dysfunction. CASE PRESENTATION: A 1-month-old infant was diagnosed with incidental patent ductus venosus (PDV) and a focal nodular hyperplasia-like liver nodule. Laboratory tests revealed hyperammonemia (192 µmol/L) and cholestasis. Imaging confirmed a 7.8-mm shunt connecting the left portal vein to the inferior vena cava. Urgent transcatheter closure was performed via the right jugular vein using a 12/10-mm LifeTech™ Konar-MFO occluder. Post-procedural imaging showed com

Simplified Trans-Axillary Aortic Valve Replacement Under Local Anesthesia - A Single-Center Early Experience.

BACKGROUND: The axillary artery is an alternative route for patients with comorbidities and unfavorable femoral arteries who need transcatheter aortic valve replacement (TAVR). Simplified trans-axillary transcatheter aortic valve replacement (TAx-TAVR) implies a completely percutaneous approach under local anesthesia and arteriotomy closure with vascular closure techniques. Herein, we report on early experience with simplified TAx-TAVR under local anesthesia. METHODS: We enrolled all consecutive patients who underwent simplified TAx-TAVR in our center. Main study parameter was the incidence o

Saving Subclavian Arteries with Percutaneous Approach: a Solution for Catheter Misplacement.

INTRODUCTION: Inadvertent placement of a central venous catheter (CVC) into a major artery can lead to potentially devastating complications. In critically ill patients, the use of vascular closure devices is an attractive therapeutic option. We report two cases of catheter misplacement into cervical arteries managed with the Perclose ProStyle® system. METHODS: A retrospective analysis was conducted on adult patients admitted to the Intensive Care Unit (ICU) between July and December 2023 who underwent removal of a misplaced catheter using an endovascular approach. RESULTS: Two patients were…

Successful first-in-man percutaneous transapical-transseptal Melody mitral valve-in-ring implantation after complicated closure of a para-annular ring leak.

AIMS: Transcatheter techniques can theoretically be applied to the treatment of para-annular ring (PAR) leaks. Little is known about their potential application and resultant complications in such cases. We describe the first-in-man percutaneous transapical-transseptal Melody valve-in-ring (ViR) implantation after a complication from percutaneous PAR leak closure. METHODS AND RESULTS: A 49-year-old woman, at high operative risk, presented with congestive heart failure secondary to severe para-ring/extravalvular regurgitation two months after bypass surgery and mitral ring annuloplasty. Balloon-assisted ExoSeal closure for inadvertent subclavian artery cannulation following central venous catheterization: A case report.

Accidental arterial puncture is a rare but significant complication associated with central venous catheterization. This report describes a case of inadvertent subclavian artery catheterization that was successfully managed with the ExoSeal vascular closure device. A female patient in her early 30s was hospitalized for severe acute pancreatitis that necessitated cannulation of the right subclavian vein. Postprocedure, infusion difficulties suggested a need for arterial catheterization, which was confirmed by imaging. The intervention resulted in prompt hemostasis and prevented further vascular

Ventricular Septal Defect.

Ventricular septal defects (VSDs) are the most common congenital heart malformations seen in children. Because spontaneous closure occurs frequently, patients with small VSDs should be followed clinically with no limitations except endocarditis prophylaxis. Surgical closure is recommended for only small defects with significant associated lesions such as aortic regurgitation, aortic valve prolapse, right or left ventricular outflow obstruction, tricuspid regurgitation, left ventricle to right atrial shunt, or recurrent endocarditis. Enlarging left ventricular size or deteriorating left ventric

Percutaneous approach to aortic disease: EVAR, TEVAR and newer techniques for more proximal disease.

Advances in device design, imaging and vascular-closure technology have driven a progressive shift from surgical femoral exposure to totally percutaneous strategies for endovascular aortic repair. This narrative review summarises contemporary evidence and practical considerations for percutaneous access across the spectrum of aortic disease, from infrarenal endovascular aneurysm repair (EVAR) to thoracic endovascular aortic repair (TEVAR), fenestrated/branched repair and endovascular treatment of the aortic arch. Percutaneous femoral access is now supported by randomized evidence, large regist

Redirection of anomalous venous pulmonary flow to left atrium using a vascular plug II.

An 8-year-old male patient was sent to the catheterization laboratory for assessment of a partial anomalous pulmonary venous connection found by echocardiography. During the intervention, cineangiogram revealed left pulmonary veins draining to the right atrium through left innominate and vertical veins. Despite this anatomical feature, these pulmonary veins were also connected to the left atrium. In a second intervention, we successfully occluded the vertical vein using a vascular closure device (Amplatzer Vascular Plug II, AGA Medical), redirecting pulmonary venous flow towards the left atriu

Transcatheter closure of Abernethy malformation associated with interrupted inferior caval vein and other systemic venous anomalies.

Abernethy malformation is a rare entity. We report a 5-year-old boy presenting with severe pulmonary hypertension in whom Abernethy malformation and inferior caval vein interruption were diagnosed by CT angiography. In addition, the iliac veins were thrombosed with multiple venous collateral drainage. This abnormal venous anatomy caused difficulty in device closure of the Abernethy malformation, which was successfully closed using a vascular plug.

Failure of percutaneous closure of prosthetic, aortic paravalvular leak.

We report the progression of aortic insufficiency after percutaneous closure of an aortic prosthesis paravalvular leak with the Amplatzer vascular plug (AGA Inc, Golden Valley, MN). Removal of the device and replacement of the aortic prosthesis was successfully performed. Based on operative findings, we hypothesize that shape mismatch between the occluder system and the leak might promote tearing at the end of slanted defects further enhancing the regurgitant area.
