Leveraging Visual Outcome Measures to Advance Therapy Development in Neuroimmunologic Disorders.

The visual system offers unparalleled precision in the assessment of neuroaxonal damage. With the majority of patients with multiple sclerosis (MS) experiencing afferent and efferent visual dysfunction, outcome measures capturing these deficits provide insight into neuroaxonal injury, even in those with minimal disability. Ideal for use in clinical trials, visual measures are generally inexpensive, accessible, and reproducible. Quantification of visual acuity, visual fields, visual quality of life, and electrophysiologic parameters allows assessment of function, whereas optical coherence tomog

Agreement of Fixation Disparity Curve between Two Different Instruments.

SIGNIFICANCE: Fixation disparity tests with various characteristics of fusional stimulus are very important for assessment of decompensated heterophoria. The results suggest that there was no reasonable agreement between the fixation disparity curve's parameters of the modified near Mallett unit and the Sheedy disparometer. PURPOSE: The purpose of this study was to determine the agreement of the fixation disparity curve parameters between the modified near Mallett unit and the Sheedy disparometer in patients with decompensated near heterophoria. METHODS: A total of 147 young adults (mean age

Assessing Suppression in Amblyopic Children With a Dichoptic Eye Chart.

PURPOSE: Suppression has a key role in the etiology of amblyopia, and contrast-balanced binocular treatment can overcome suppression and improve visual acuity. Quantitative assessment of suppression could have a role in managing amblyopia. We describe a novel eye chart to assess suppression in children. METHODS: We enrolled 100 children (7-12 years; 63 amblyopic, 25 nonamblyopic with strabismus or anisometropia, 12 controls) in the primary cohort and 22 children (3-6 years; 13 amblyopic, 9 nonamblyopic) in a secondary cohort. Letters were presented on a dichoptic display (5 letters per line).

Comparison of presbyopic additions determined by the fused cross-cylinder method using alternative target background colours.

PURPOSE: To compare and contrast standard and alternative versions of refractor head (phoropter)-based charts used to determine reading addition. METHODS: Forty one presbyopic subjects aged between 42 and 60 years were tested. Tentative additions were determined using a red-green background letter chart, and 4 cross-grid charts (with white, red, green, or red-green backgrounds) which were used with the fused cross cylinder (FCC) method. The final addition for a 40 cm working distance was determined for each subject by subjectively adjusting the tentative additions. RESULTS: There were signif

Validity and repeatability of a new test for aniseikonia.

PURPOSE: The Aniseikonia Inspector 1.1 (AI) is a new software product to measure aniseikonia using red-green anaglyphs. The purpose of this study was to test whether the AI is a valid and reliable test. METHODS: There were two groups of sample subjects: one at risk of aniseikonia, with anisometropia greater than or equal to 1.00 D (n= 29), and a control group (n= 45). The validity was studied by comparing the measured aniseikonia with the aniseikonia simulated with size lenses. The reliability was estimated by the Bland-Altman statistical method. RESULTS: The results showed that the AI under

Vergence adaptation in children and its clinical significance.

BACKGROUND AND PURPOSE: Vergence adaptation has not been well investigated in children even though it may contribute to binocular dysfunction and near work induced asthenopia. METHODS: We compared vergence adaptation in 18 children and 18 young adult subjects by assessing tonic vergence (TV) before and immediately after a period of sustained near fixation, by measuring heterophoria with a synoptophore through 0.5 mm binocular pinholes. Adaptation was induced by a reading task at 15 cm for a continuous 5 minute period. RESULTS: Mean pre-task TV values of 0.70 MA (Meter Angles) and 0.20 MA wer

Color vision defects in sickle cell anemia.

The Farnsworth-Munsell 100-hue test was used to compare color vision in patients with sickle cell anemia, 20/20 visual acuity, and mild to severe peripheral retinopathy, but without posterior retinal vessel involvement, with matched normal controls. Patients with sickle cell anemia had significantly more blue-yellow and mixed color vision defects and significantly higher Farnsworth-Munsell 100-hue scores than did controls. There were no significant associations between color vision defects and the severity of peripheral retinal findings or the diameter of the foveal avascular zone. These resul

Low vision examination in children with visual impairment.

Low vision aid evaluation should be part of the routine ophthalmic care of all children with subnormal vision. The evaluations establish the child's visual capacities and assess the possible benefits of optical or nonoptical aids to maximize visual function. The concerted efforts of parents, physicians, and teachers are necessary to minimize the child's visual impairment and to avoid subsequent disabilities. Examination technique and equipment is essentially that required for routine ophthalmic care with the addition of a few low vision aid devices. Particular emphasis should be placed on comm

Feasibility, comprehensibility and acceptability of the VISION-Cog, a novel tool to assess cognitive impairment in visually impaired older adults: a cross-sectional pilot study in Singapore.

OBJECTIVES: We pilot-tested the VISually Independent test battery Of NeuroCOGnition (VISION-Cog) to determine its feasibility, comprehensibility and acceptability in evaluating cognitive impairment (CI) in visually impaired older Asian adults. DESIGN: The VISION-Cog was iteratively fine-tuned through pilot studies and expert-panel discussion. In the first pilot study (Stage 1), we recruited 15 visually impaired and cognitively normal participants aged ≥60 years to examine the pilot VISION-Cog's feasibility (length of time to administer), comprehensibility (clarity of instructions) and accepta

Comprehensive Evaluation of Visual Function in Patients With Orbital Fracture by Visual Electrophysiology and Visual Field Examination.

OBJECTIVE: At present, the effect of the visual electrophysiology and vision field examination in patients with orbital blowout fracture is rarely studied. So, the authors investigate the value of visual electrophysiology and vision field examination in the diagnosis of ocular contusion. METHODS: The position and range of fracture of 81 patients were determined by computed tomography (CT) scanning. Visual evoked potential (VEP), electroretinogram (ERG), and mfERG were vision field examination detected in 81 patients and the results were compared with those of contralateral healthy eyes. In ad

Assessment of vision and hearing in children conceived spontaneously and by ICSI: a prospective controlled, single-blinded follow-up study.

Long-term follow-up studies on the health of children born after assisted reproduction technologies are mandatory. Vision and hearing are the most important senses that continue to develop during childhood. There are few reports on vision and hearing in preschool children born after assisted conception. This prospective controlled blinded follow-up study examined 276 term-born singleton intracytoplasmic injection (ICSI) children and 273 spontaneously conceived controls at a mean age of 5.5 years and performed detailed vision and hearing test and clinically examined eyes and ears. There was no

Trichromacy and ultraviolet vision in a nocturnal marsupial.

Color vision among mammals is diverse and complex, with many physiological and genetic factors affecting spectral sensitivity, the ability to perceive different wavelengths of light. In this study, the color vision of the sugar glider (Petaurus breviceps), a nocturnal, gliding mammal, was examined through a series of behavioral tests, genetic analyses, and immunohistochemistry. This is the first study to classify the color vision capabilities of this species. Sugar gliders demonstrated trichromacy and ultraviolet (UV) sensitivity, the latter of which was further supported by genetic analysis.

[The new standard for diagnostic near reading charts-DIN EN ISO 7921:2024].

BACKGROUND: With the publication of the DIN EN ISO 7921:2024 standard, an international standard for medical near reading charts has been introduced for the first time. This standard enables the determination of a valid and internationally comparable threshold for near visual performance, i.e., the reading acuity. MATERIAL AND METHODS: This article reports on the central contents and scientific background of this standard. It describes the definition of print size based on the x‑height, the geometric (logarithmic) progression of print sizes, standard testing distances and requirements regardi

[Fundus perimetry in functional diagnostics of glaucoma. Applicable in the practice?].

Examination of the visual field using static automated perimetry (SAP) is the method of choice for the detection of functional damage secondary to glaucoma. However, with SAP early visual field defects might be missed even if there is already visible damage of the retinal nerve fibre. The microperimetry or beter fundus perimetry provides a detailed examination of the differential luminance threshold within defined retinal areas. However, in contrast to lesions of the retinal receptors, in cases of glaucomatous damage the retinal fibre course has to be considered resulting in a displacement bet

Clinical assessment of thyroid related orbitopathy: a review.

This review follows the process of evaluation of thyroid related orbitopathy (TRO) patients from diagnosis to treatment decision. We will attempt to define the criteria for referring TRO patients to the ophthalmologist and establish a common basis for orbital examination and TRO patient assessment. This should help classify TRO patients and achieve the best treatment regime. Thyroid related orbitopathy (TRO) is an endocrine disorder with orbital manifestations. Though most patients are first seen by an endocrinologist because of thyroid function disturbance symptoms, approximately 10% will fir

A diagnostic ruler for strabismus and amblyopia.

We designed a diagnostic slide ruler for easier detection of strabismus and amblyopia, both of which begin in childhood. This new instrument makes the Parks three-step test easier to use and faster than other methods in diagnosing isolated cyclovertical muscle paralysis. Furthermore, the slide ruler tests visual acuity with both series letters and single letters. Incorporated into the instrument are Laurence's strabismometer, Haab's pupillometer, occluder, pinhole, and millimeter ruler. A laboratory model is now available, and this slide ruler can assist the ophthalmological practitioner in an

Various stages of optic neuritis assessed by subjective brightness of flicker.

In the Aulhorn flicker test, the subjective brightness of a steady field is matched to that of a flickering field at 50 to 0 Hz. The test gives pathologic values only for active optic neuritis (ON) and gives normal results for normal eyes and eyes with subsided ON and other diagnoses, with a specificity of 98% and a sensitivity of 85.5%. Characteristic variations of the curves of subjective brightness during the course of ON allow a subtle assessment of five different disease stages, and there is a clear distinction between various disease patterns, ie, acute, chronic, recurrent, diminishing,
