BACKGROUND: Keratoectasia is a rare but well-known complication after laser-assisted in situ keratomileusis (LASIK). Patients with this condition can have high and irregular astigmatism. When the treatment of the high astigmatic correction cannot be accomplished surgically or when the keratoectasia patient rejects surgical enhancement, optical correction with devices such as soft or rigid gas-permeable contact lenses may be pursued. In fact, toric soft contact lenses are a good first option for fitting postoperative keratoectasia patients.

Generally in keratoconus the corneal apex displacement is inferior in position and bilateral in expression. Two young male keratoconic subjects enrolled in the Dundee University Scottish Keratoconus Study (DUSKS) demonstrated with corneal topography the presence of unilateral superior keratoconus. Successful visual rehabilitation of a superior cone with contact lens fitting represents a particular challenge to the contact lens practitioner. Retinal prostheses have shown some clinical success in restoring vision in patients with retinitis pigmentosa. However, the post-implantation visual acuity does not exceed that of legal blindness. Keratoprosthesis (implantation of artificial, plastic cornea) is indicated in severe cases with corneal leucoma (non-transparent, cicatrized cornea) in which keratoplasty (corneal transplantation) is not possible or has repeatedly failed.
