BACKGROUND: Benzalkonium chloride (BAK) and benzethonium chloride (BEC) are well-characterized skin irritants and rare sensitizers, but optimal testing for allergic contact dermatitis (ACD) is not established.
OBJECTIVE: Sensitization prevalence was sought, and several patch testing concentrations and vehicles were compared.
METHODS: One hundred forty-two patients tested to the standard screening series for evaluation of dermatitis consented to additional tests including BAK 0.15% aqueous (aq), BAK 0.15% petrolatum (pet), BEC 0.15% aq, and BEC 0.5% aq. Follow-up to assess clinical relevancy

We enrolled 14 female patients (aged 12-60 years) affected by celiac disease (confirmed by duodenal biopsy), presenting dermatologic eczematous lesions involving their face, neck, and overall arms, after application of gluten-containing emollient cream, bath, or face powder, or after contact with foods containing wheat and durum wheat. Five patients resulted positive to patch-by-patch with wheat and durum wheat (mild to moderate positivity) with erythema and vesicles, in correspondence of their sites of application. Utilizing gluten-free cream and bath, and wearing gloves before hand contact w

BACKGROUND: The validated hypoallergenic (vh) rating system was initiated in 1988 to try to objectively validate the "hypoallergenic" claim in cosmetics.
OBJECTIVES: To show how the system rates cosmetic hypoallergenicity and to compare the prevalence of cosmetic contact dermatitis (CCD) among users of regular cosmetics versus cosmetics with high VH numbers.
METHODS: (1) Made a VH list based on top allergens from patch-test results published by the North American Contact Dermatitis Group (NACDG) and the European Surveillance System on Contact Allergies (ESSCA); (2) reviewed global regulatory

Shellac is a common ingredient in cosmetics such as mascaras, lipsticks, nail polish, and hairsprays. As an emerging contact allergen in patients with eyelid dermatitis and contact cheilitis, it was added to the 2009-2010 North American Contact Dermatitis Group screening panel. Consideration of this allergen should be given with patients who present with allergic contact dermatitis in areas where shellac-containing substances were applied.

Exotic woods from tropical and subtropical regions (eg, from South America, south Asia, and Africa) frequently are used occupationally and recreationally by woodworkers and hobbyists. These exotic woods more commonly provoke irritant contact dermatitis reactions, but they also can provoke allergic contact dermatitis reactions. We report three patients seen at Mayo Clinic (Rochester, MN) with allergic contact dermatitis reactions to exotic woods. Patch testing was performed and included patient-provided wood samples. Avoidance of identified allergens was recommended. For all patients, the derma

BACKGROUND: The Thin-layer Rapid Use Epicutaneous (TRUE) Test has approval for adults.
OBJECTIVE: To evaluate the efficacy and safety of TRUE Test panels 1.1, 2.1, and 3.1 in children and adolescents suspected of having allergic contact dermatitis (ACD).
METHODS: An open-label prospective study was performed, analyzing 102 consecutive patients aged 6 to 18 years referred for suspected ACD, between December 2008 and October 2009. Patch tests were applied for 48 hours, and evaluations of skin reactions were conducted at days 3 and 7, with a follow-up visit 3 weeks after the initial application

Nickel allergy can result in both cutaneous and systemic manifestations, and can range from mild to severe symptoms. A severe form of this allergy is the Systemic nickel allergy syndrome, clinically characterized by cutaneous manifestions (contact dermatitis, pompholyx, hand dermatitis dyshydrosis, urticaria) with chronic course and systemic symptoms (headache, asthenia, itching, and gastrointestinal disorders related to histopathological alterations of gastrointestinal mucosa, borderline with celiac disease). This review aims to briefly update the reader on past and current therapies for nick

BACKGROUND: The standard allergen series used in patch testing contains metals that most commonly cause allergic contact dermatitis, but testing with additional metal allergens is warranted for select patients.
OBJECTIVE: To report our experience with patch testing of metals.
METHODS: We retrospectively analyzed outcomes of 1,112 patients suspected of having metal allergies. Patients were seen from January 1, 2000, through December 31, 2009. Patch testing was performed with 42 metal preparations (6 in the standard series, 36 in the metal series).
RESULTS: Patch testing most commonly was per

Methyldibromoglutaronitrile/phenoxyethanol (Euxyl K 400) is a preservative found in both personal care products and industrial sources. Although Euxyl K 400 initially appeared to have low sensitizing potential, increased prevalence of contact allergy to Euxyl K 400 led to regulatory intervention. This review summarizes the history, epidemiology, and management of contact allergy to Euxyl K 400. Issues related to patch-test preparations are also discussed.

Cutaneous reactions to metal implants, orthopedic or otherwise, are well documented in the literature. The first case of a dermatitis reaction over a stainless steel fracture plate was described in 1966. Most skin reactions are eczematous and allergic in nature, although urticarial, bullous, and vasculitic eruptions may occur. Also, more complex immune reactions may develop around the implants, resulting in pain, inflammation, and loosening. Nickel, cobalt, and chromium are the three most common metals that elicit both cutaneous and extracutaneous allergic reactions from chronic internal expos

Transdermal therapeutic systems (TTSs) are commonly used to treat many diverse medical conditions. There are several advantages to this form of drug delivery; however, adverse cutaneous reactions are common. This review article provides an update on skin reactions due to TTSs containing medications newly available in TTS form, including buprenorphine, diclofenac, lidocaine, oxybutynin, rotigotine, methylphenidate, rivastigmine, teriparatide, and nicardipine.

BACKGROUND: Finding alternative hair dyes for individuals allergic to para-phenylenediamine (PPD) has been difficult. Newer permanent and demipermanent hair dyes that have replaced PPD with para-toluenediamine sulfate (PTDS) are now available.
OBJECTIVE: We examined whether individuals allergic to PPD will tolerate PPD-free hair dyes containing PTDS.
METHODS: A retrospective analysis of patch-test results since October 2006 was done and yielded 28 patients allergic to PPD who were also tested with a hair dye series. From January 2004 through October 2006, seven additional patients allergic t
