Promising Clinical Outcomes Using Rotational Achilles Autograft for Knee Arthroplasty Terrible Triad.

BACKGROUND: The knee arthroplasty terrible triad (TT) is a combination of periprosthetic joint infection, extensor mechanism insufficiency, and soft tissue defect. The TT is a limb-threatening condition that has historically poor functional results and high probability of amputation or fusion. The rotational gastrocnemius-Achilles rotational flap (GARF) holds promise as native soft tissue to provide coverage and muscular power to assist with knee extension. We hypothesized that application of the GARF in conjunction with two-stage revision improves functional and limb-salvage outcomes compared

Low prognostic nutritional index is highly associated with periprosthetic joint infection in revision arthroplasty: a 14-year retrospective cohort study.

BACKGROUND: The relationship between preoperative nutritional status and periprosthetic joint infection (PJI) remains unclear in patients undergoing revision arthroplasty. This study aimed to investigate (1) whether preoperative nutritional status influences the risk of PJI in these patients, and (2) to determine the optimal threshold for risk stratification. METHODS: A retrospective cohort study involved 1,117 patients who underwent hip or knee revision between January 2011 and September 2024. Prognostic nutritional index (PNI) was calculated based on serum albumin and lymphocyte counts. Mul

Outcomes of device extraction in patients with chronic kidney disease on renal replacement therapy and cardiac implantable electronic device infections.

BACKGROUND: Patients with chronic kidney disease (CKD) on renal replacement therapy (RRT) are at high risk for cardiovascular implantable electronic device (CIED) infections. Although device extraction is standard management, it is often avoided in these patients because of high procedural risks. OBJECTIVES: Evaluate the outcomes of CIED extraction in CKD on RRT patients with device infection. METHODS: This study used data from the TriNetX research network. The International Classification of Diseases-10th Revision-Clinical Modification coding system was used to identify patients. Adults wit

Sacral neuromodulation device biofilm differs in the absence and presence of infection, harbors antibiotic resistance genes, and is reproducible in vitro.

INTRODUCTION/PURPOSE: Sacral neuromodulation (SNM) is effective therapy for overactive bladder refractory to oral therapies, and non-obstructive urinary retention. A subset of SNM devices is associated with infection requiring surgical removal. We sought to compare microbial compositions of explanted devices in the presence and absence of infection, by testing phase, and other clinical factors, and to investigate antibiotic resistance genes present in the biofilms. We analyzed resistance genes to antibiotics used in commercially-available anti-infective device coating/pouch formulations. We fu

Relation of Left Ventricular Assist Device Infections With Cardiac Transplant Outcomes.

Left ventricular assist device (LVAD)-specific infections (LSIs) are common in patients on LVAD support awaiting heart transplant (HT), yet their impact on post-HT outcomes is not completely understood. We hypothesized that LSIs would result in vasoplegia and negatively affect post-HT 30-day and 1-year outcomes. LSI was defined as driveline, pump, or pocket infection. The short-term outcome was a composite of acute renal failure, allograft rejection, and mortality at 30 days after HT. The long-term outcome was a composite of allograft rejection and death within 1 year after HT. We performed a

Prior hip or knee prosthetic joint infection in another joint increases risk three-fold of prosthetic joint infection after primary total knee arthroplasty: a matched control study.

AIMS: There is little information regarding the risk of a patient developing prosthetic joint infection (PJI) after primary total knee arthroplasty (TKA) when the patient has previously experienced PJI of a TKA or total hip arthroplasty (THA) in another joint. The goal of this study was to compare the risk of PJI of primary TKA in this patient population against matched controls. PATIENTS AND METHODS: We retrospectively reviewed 95 patients (102 primary TKAs) treated between 2000 and 2014 with a history of PJI in another TKA or THA. A total of 50 patients (53%) were female. Mean age was 69 ye

Higher risk of revision for infection using systemic clindamycin prophylaxis than with cloxacillin.

Background and purpose - Clindamycin has not been compared with other antibiotics for prophylaxis in arthroplasty. Since 2009, the Swedish Knee Arthroplasty Register (SKAR) has been collecting information on the prophylactic antibiotic regime used at every individual operation. In Sweden, when there is allergy to penicillin, clindamycin has been the recommended alternative. We examined whether there were differences in the rate of revision due to infection depending on which antibiotic was used as systemic prophylaxis. Patients and methods - Patients who had a total knee arthroplasty (TKA) per

[Diagnosis in patients with a painful arthroplasty].

The differential diagnosis between asepting loosening or prosthetic joint infection is not always easy. Tc-99m-labelled leucocyte scans, frozen section and histology can help recognise doubtful cases. We report the experience of the Unit for Infectious Diseases and Septic Orthopaedics of the ASL-2 Liguria, Italy, with a Tc-99m-labelled leucocyte scan and intraoperative frozen section to choose the best therapeutic approach: one-stage or two-stage exchange or arthrodesis-arthroplastica. All cases underwent histology and intraoperative cultures to confirm the diagnosis, and the effectiveness of

Role of hybrid monolateral fixators in managing humeral length and deformity correction.

BACKGROUND AND PURPOSE: Humeral lengthening and deformity correction are now being done increasingly for various etiologies. Monolateral external fixators have advantages over traditional Ilizarov circular fixators; they are easy to apply, they are less bulky, and they are therefore more convenient for the patient. We assessed the effectiveness of hybrid monolateral lateral fixators in humeral lengthening and deformity correction. METHODS: We retrospectively reviewed 23 patients (40 humeri) with various pathologies who underwent lengthening-with or without deformity correction using monolater
