Biomechanical comparison of different cerclage types in addition to an angle stable plate osteosynthesis of distal tibial fractures.

BACKGROUND: Different stand-alone cerclage configurations and their optimal twisting techniques have been investigated over the years. This study tests for the stabilizing effect of different supplemental cerclage materials in combination with locked plating of distal tibia fractures. METHODS: Locking plate fixation of a distal tibial spiral fracture was tested as stand-alone and with supplemental cerclage materials (one cable, two cables, wire, fiber tape). Construct stiffness and fracture gap movements were investigated under quasi-static and dynamic loads and compared to the stand-alone lo

Comparison between headless compression screws and tension band wires for the fixation of medial malleolar fractures: a prospective randomized trial.

INTRODUCTION: To date, there has been no prospective randomized trial supporting the rationale of the use of headless compression screw (HCS) compared to conventional fixation methods for medial malleolar fractures. This study aimed to prospectively compare the outcomes of the HCS and tension band wire (TBW) for the fixation of medial malleolar fractures. MATERIAL AND METHODS: Sixty patients were randomized to receive either an HCS or a TBW for the fixation of a medial malleolar fracture. Clinical outcomes were assessed using the Olerud-Molander ankle score (OMAS), EuroQoL five-dimensional in

Pure isolated medial talonavicular joint dislocation following low-energy trauma: a case report.

Midtarsal dislocations are relatively rare injuries secondary to high-energy trauma and are typically accompanied by disruption of ligamentous structures and fractures of the midfoot. We herein present a case of a pure isolated medial swivel dislocation of the talonavicular joint (TNJ) that was sustained following low-energy trauma without an associated fracture. A 78-year-old woman visited our emergency department with severe pain in the midfoot area of the right foot without neurovascular deficits. She had sustained this injury after severe ankle inversion while going downstairs. Plain radio

Multiply Broken Metallic Hardware Removal for United Patella Fracture Applied Twenty Years Before: A Case Report.

Breakage of tension band wires, used to treat the patella fracture, is not uncommon several years after the fracture fixation. Broken wires may migrate to surrounding neurovascular structures, other vital organs like heart and may cause potentially fatal complications. Once the wires have been broken, it is very difficult to remove the broken pieces of metal wires. We report a 50 years old male patient with broken tension band wires at multiple sites for patella fracture. The broken wires were removed 20 years after the initial surgery without any undue complications, however patient sustained

[Metacarpal fractures. Use of an external cemented wire fixation device].

Osteosynthesis for fractures of the metacarpals using a simplified system of Kirschner wires introduced percutaneously and maintained by an acrylic cement bridge has been performed in 40 patients. Consolidation was obtained consistently, within one month on average, with almost no morbidity and virtually without sequelae. This procedure is extremely simple and can be used for all extra-articular fractures of the metacarpals with displacement.

Tension band tendon-approximating cerclage for surgical fixation of patellar fractures: a novel surgical technique.

Patellar fractures are potentially debilitating injuries due to loss of extensor mechanism function, resulting in an inability to extend the leg. Traditionally, these fractures have been surgically managed with open reduction and internal fixation using a tension band construct. This can be performed with K-wires or cannulated screws with suture or cable as the tension band. Plate osteosynthesis is another option that is increasing in popularity. Incorporating the tension band construct converts tensile force into a healing-compatible compressive force. However, these techniques often fail whe

Postoperative sternal dehiscence in obese patients: incidence and prevention.

BACKGROUND: Obesity has been identified as the single most important risk factor for postoperative sternal infection in coronary bypass surgery patients. It is also a major risk factor for sternal dehiscence, with or without infection, for any type of cardiac operation. We assessed whether prophylactic measures could prevent this complication. METHODS: Two studies were conducted. In study A, 3,158 heart surgery patients were analyzed at 3 cardiac units. Obesity was defined as body mass index (BMI) more than 30. Group I (1,253 obese [39.7%]) was compared with group II (1,905 nonobese [60.3%]).

A technique for isolated arthrodesis of the second metatarsocuneiform joint.

Arthrodesis for primary osteoarthrosis of the second metatarsocuneiform joint has received little attention in the literature when compared to salvage of posttraumatic osteoarthrosis after Lisfranc fracture dislocation. The use of screw or plate fixation is commonly advocated for such in situ arthrodesis, and the use of trephine/dowel plugs has also been described. The authors present an alternate approach for addressing primary osteoarthrosis of the second metatarsocuneiform joint with the use of Kirschner wires fashioned into staples with cancellous bone graft interposition.

Combined Percutaneous Iliosacral Screw Fixation With Sacroplasty Using Resorbable Calcium Phosphate Cement for Osteoporotic Pelvic Fractures Requiring Surgery.

Osteoporotic sacral fractures, including acute and chronic insufficiency fractures, are increasing in frequency and present a number of management problem. Many of these patients are treated nonoperatively with relative immobility (eg, bedrest, wheelchair, or weight-bearing restrictions) and analgesics, which likely make the osteoporotic component worse. Surgery in this patient population may be desirable in some cases with the goals of improving mobility, relieving pain, and healing in an aligned position while minimizing deformity progression. However, internal fixation of the osteoporotic p

Single versus dual Kirschner wires for closed reduction and intramedullary nailing of displaced fractures of the fifth metacarpal neck (1-2 KiWi): a randomized controlled trial.

AIMS: The aim of this study was to investigate whether clinical and radiological outcomes after intramedullary nailing of displaced fractures of the fifth metacarpal neck using a single thick Kirschner wire (K-wire) are noninferior to those of technically more demanding fixation with two thinner dual wires. PATIENTS AND METHODS: This was a multicentre, parallel group, randomized controlled noninferiority trial conducted at 12 tertiary trauma centres in Germany. A total of 290 patients with acute displaced fractures of the fifth metacarpal neck were randomized to either intramedullary single-w

Is suture comparable to wire for cerclage fixation? A biomechanical analysis.

BACKGROUND: Cerclage wire is the current standard for circumferential bone fixation. Advances in technology have improved modern sutures, allowing for expanded utility and broader application. The present study compared the strength and durability of cerclage fixation between modern suture materials and monofilament wire. METHODS: The Surgeon's Knot, the Nice Knot and the Modified Nice Knot, were each tied using three separate suture materials: no. 2 FiberWire (Arthrex, Naples, FL, USA), no. 2 Ultrabraid (Smith & Nephew, Andover, MA, USA) and no. 5 Ethibond (Johnson & Johnson, Somerville, NJ,

Closed Reconstruction of Depressed Frontal Sinus Fractures: An Innovative Technique.

This article describes a novel technique for reconstructing a frontal sinus fracture that minimizes the risks associated with conventional approaches. In this study, we report a patient presented with a depressed frontal sinus fracture following a car accident. Through a small (5 mm) sub-brow incision, a freer elevator was inserted beneath the fractured fragments to elevate them. A 16-gauge venous catheter was then introduced horizontally from the medial aspect of the bone, exiting at the lateral side. Following this, a 24-gauge wire was threaded through the catheter, which was subsequently re

Medial Femoral Condyle Flap as Salvage for Osteomyelitic Clavicle Nonunion.

Clavicle nonunion with osteomyelitis is a challenging condition, combining bone infection and structural defects. Traditional 2-stage procedures address these issues but may be unsuitable for patients unable to undergo multiple operations. The medial femoral condyle (MFC) flap has emerged as an effective option, offering both vascularized bone grafting and mechanical support in a single-stage approach, integrating infection control and bone reconstruction. We report a 21-year-old man with right clavicle nonunion and osteomyelitis following internal fixation for a fracture sustained in a motorc

Trends in the management of fractures of the distal radius in Ireland: did the Distal Radius Acute Fracture Fixation Trial (DRAFFT) change practice?

AIMS: The aim of this study was to examine trends in the management of fractures of the distal radius in Ireland over a ten-year period, and to determine if there were any changes in response to the English Distal Radius Acute Fracture Fixation Trial (DRAFFT). PATIENTS AND METHODS: Data was grouped into annual intervals from 2008 to 2017. All adult inpatient episodes that involved emergency surgery for fractures of the distal radius were included. RESULTS: In 2008 Kirschner-wire (K-wire) fixation accounted for 59% of operations for fractures of the distal radius, and plate fixation for 21%.

Radiographic evaluation of the position of implants in the medial malleolus in relation to the ankle joint space: anteroposterior compared with mortise radiographs.

BACKGROUND: Displaced transverse fractures of the medial malleolus are commonly treated with open reduction and internal fixation with two screws or wires. A mortise radiograph is often used to verify the position of the implants relative to the joint space. However, because the medial and lateral talomalleolar spaces are normally not parallel, the mortise projection (which is colinear with the lateral space) does not provide an accurate radiograph of the medial joint space. METHODS: In ten cadaveric ankles, two wires were inserted into the medial malleolus, as is done for fixation of a fract

Surgical treatment of Kirner's deformity.

In 1927 Kirner described a congenital deformity of the fifth finger. Since then only 68 cases have been reported and to date many different views in the pathogenesis and treatment exist. The authors report a case of a 13 years-old girl. By a dorso-lateral approach, the distal phalanx was exposed and bone resection performed; alignment was maintained by Kirschner longitudinal wires, parosteal for the left digit and intramedullary for the right. The cosmetic and functional results were good.

Arthroscopic Reduction and Stabilization of Chronic Perilunate Wrist Dislocations.

An acute perilunate wrist injury that is unreduced for more than 6 weeks results in severe disability, and even open reduction with stabilization through wide dorsal and volar approaches is technically challenging. This report describes an arthroscopic technique for reduction and percutaneous wire stabilization of a chronic perilunate wrist dislocations. The technique involves initial radiocarpal and midcarpal access through the 6R and 3-4 portals, and these portals are used for synovectomy and debridement of capsular flap tears. The midcarpal joint is accessed initially through the radiocarpa

Evaluation of a fluoroscopy-based navigation system enabling a virtual radiation-free preview of X-ray images for placement of cannulated hip screws. A cadaver study.

Accurate placement of cannulated screws is essential to ensure fixation of medial femoral neck fractures. The conventional technique may require multiple guide wire passes, and relies heavily on fluoroscopy. A computer-assisted planning and navigation system based on 2D fluoroscopy for guide wire placement in the femoral neck has been developed to improve screw placement. The planning process was supported by a tool that enables a virtual radiation-free preview of X-ray images. This is called "zero-dose C-arm navigation". For the evaluation of the system, six formalin-fixed cadaveric full-body
