Understanding why cognitive-behavioral therapy is an effective treatment for adolescents with juvenile fibromyalgia.

Recent studies have demonstrated that cognitive-behavioral therapy (CBT) is an effective treatment for children and adolescents with juvenile fibromyalgia. Unfortunately, the specific psychological changes that occur during treatment that explain why CBT works are not well understood. Historically, the increased use of specific coping strategies learned during CBT was thought to be the primary reason for why CBT was effective. However, evidence to support the notion that increases in adaptive coping directly lead to patient improvement is minimal. Instead, a growing number of studies in adults

The role of maladaptive beliefs in cognitive-behavioral therapy: Evidence from social anxiety disorder.

Beliefs that are negatively biased, inaccurate, and rigid are thought to play a key role in the mood and anxiety disorders. Our goal in this study was to examine whether a change in maladaptive beliefs mediated the outcome of individual cognitive-behavioral therapy (CBT) for social anxiety disorder (SAD). In a sample of 47 individuals with SAD receiving CBT, we measured maladaptive interpersonal beliefs as well as emotional and behavioral components of social anxiety, both at baseline and after treatment completion. We found that (a) maladaptive interpersonal beliefs were associated with socia

Therapeutic alliance in telephone-administered cognitive-behavioral therapy for hematopoietic stem cell transplant survivors.

ObjectiveA strong therapeutic alliance has been found to predict psychotherapeutic treatment success across a variety of therapeutic modalities and patient populations. However, only a few studies have examined therapeutic alliance as a predictor of psychotherapy outcome among cancer survivors, and none have examined this relation in telephone-administered cognitive-behavioral therapy (T-CBT). This study evaluated the extent to which therapeutic alliance affected psychotherapy outcomes in survivors of hematopoietic stem cell transplantation (HSCT), a treatment for some cancers.MethodForty-six

Changes in EEG cross-frequency coupling during cognitive behavioral therapy for social anxiety disorder.

Coupling between EEG delta and beta oscillations is enhanced among anxious and healthy individuals during anticipatory anxiety. EEG coupling patterns associated with psychotherapy have not yet been quantified in socially anxious individuals. In this study, we used a double baseline, repeated measures design, in which 25 adults with a principal diagnosis of social anxiety disorder completed 12 weekly sessions of standardized group cognitive behavioral therapy and four EEG assessments: two at pretreatment, one at midtreatment, and one at posttreatment. Treatment was associated with reductions in…

Comparison of group and individual cognitive-behavioral therapy in reducing fear of negative evaluation.

Previous studies have established the efficacy of group and individual formats of cognitive-behavioral therapy in reducing fear of negative evaluation in social anxiety disorder, but the effectiveness of the group and individual therapies has not been widely assessed. This study was conducted to compare the effectiveness of cognitive-behavior group therapy to individual cognitive-behavioral therapy in social anxiety disorder. 28 patients were randomly allocated to two groups of 14. Each group participated in 12 weekly sessions. The final sample was 22 participants after drop-out. Pretest and p

Computer-assisted delivery of cognitive behavioral therapy for anxiety disorders in primary-care settings.

OBJECTIVES: This article describes a computer-assisted cognitive behavioral therapy (CBT) program designed to support the delivery of evidenced-based CBT for the four most commonly occurring anxiety disorders (panic disorder, posttraumatic stress disorder, generalized anxiety disorder, and social anxiety disorder) in primary-care settings. The purpose of the current report is to (1) present the structure and format of the computer-assisted CBT program, and (2) to present evidence for acceptance of the program by clinicians and the effectiveness of the program for patients. METHODS: Thirteen c…

Guided self-help versus cognitive-behavioral group therapy in the treatment of bulimia nervosa.

OBJECTIVE: The aim of the current study was to evaluate whether guided self-help was effective in the short and long term in the treatment of bulimia nervosa. METHOD: Eighty-one patients with bulimia nervosa were randomly assigned to either a self-help manual with a maximum of 18 short weekly visits (guided self-help) or to 18 weekly 1.5-h sessions of cognitive-behavioral group therapy (CBT). The primary outcome variables were monthly frequencies of self-reported binge eating and vomiting episodes. Secondary outcome variables were eating disorder-related psychopathology (assessed with the Eat

Comparing two measures of eating restraint in bulimic women treated with cognitive-behavioral therapy.

OBJECTIVE: To examine changes in dietary restraint patterns revealed by the Eating Disorders Examination Restraint subscale (EDE-R) and the Three-Factor Eating Questionnaire Cognitive Restraint scale (TFEQ-CR) in a large sample of women with bulimia nervosa (BN) who completed 18 weeks of cognitive-behavioral therapy (CBT). METHOD: Data from 134 subjects were obtained from a larger study and analyzed using repeated-measures analysis of variance (ANOVA). RESULTS: The EDE-R showed statistically and clinically significant decreases post-CBT, whereas the TFEQ-CR did not change significantly. DIS

Cognitive-behavioral therapy for panic disorder: a review of treatment elements, strategies, and outcomes.

This article provides an overview of cognitive-behavioral therapy (CBT) for panic disorder. CBT is currently considered a first-line treatment for panic disorder. It offers benefit after short-term intervention, typically consisting of 12-15 sessions conducted in either an individual or a group format. The treatment focuses on the elimination of the patterns that underlie and perpetuate the disorder. Through CBT, patients learn about the nature of the disorder and acquire a set of strategies that counter the fears of panic attacks themselves, and break the recurring cycle of anticipatory anxie

Process and treatment adherence factors in group cognitive-behavioral therapy for partner violent men.

This study used multilevel modeling to examine process and treatment adherence factors as predictors of collateral partner reports of abuse following participation in a cognitive-behavioral grouptreatment program for partner violent men (N = 107). Therapist working alliance ratings predicted lower levels of physical and psychological abuse at the 6-month follow-up and were the strongest predictors of outcome. Homework compliance partially mediated associations between early alliance ratings and psychological abuse at follow-up. Greater group cohesion during treatment, assessed byclient report,

Virtual reality assisted cognitive behavioral therapy for the treatment of Panic Disorders with Agoraphobia.

The chapter describes the characteristics of the Experiential-Cognitive Therapy (ECT) protocol for Panic Disorder and Agoraphobia. The goal of ECT is to decondition fear reactions, to modify misinterpretational cognition related to panic symptoms and to reduce anxiety symptoms. This is possible in an average of eight sessions of treatment plus an assessment phase and booster sessions, through the integration of Virtual Experience and traditional cognitive-behavioral techniques. We decided to employ the techniques included in the cognitive-behavioral approach because they showed high levels of

Evaluation of manual-based cognitive-behavioral therapy for bulimia nervosa in a service setting.

In the present study manual-based cognitive-behavioral therapy for bulimia nervosa was evaluated on an unselected sample of an out-patient service facility. A total of 73 female patients who asked for treatment received the primary diagnosis of bulimia nervosa. Of these, 67 took up treatment. Treatment was completed by 66 patients. Outcome variables were the number of binge episodes along with questionnaire scores for restraint eating, emotional eating, body dissatisfaction and depressiveness. At the end of treatment and 1 year after the end of treatment significant improvements were found in

Cognitive-behavioral therapy, imipramine, or their combination for panic disorder: A randomized controlled trial.

CONTEXT: Panic disorder (PD) may be treated with drugs, psychosocial intervention, or both, but the relative and combined efficacies have not been evaluated in an unbiased fashion. OBJECTIVE: To evaluate whether drug and psychosocial therapies for PD are each more effective than placebo, whether one treatment is more effective than the other, and whether combined therapy is more effective than either therapy alone. DESIGN AND SETTING: Randomized, double-blind, placebo-controlled clinical trial conducted in 4 anxiety research clinics from May 1991 to April 1998. PATIENTS: A total of 312 pati

Cognitive-behavioral therapy versus combined treatment with group psychoeducation and fluoxetine in bulimic outpatients.

A series of 51 female bulimic outpatients, aged 23.4+/-3.9, were assigned either to Cognitive-Behavioral Therapy (CBT) or combined Group Psychoeducation and Fluoxetine (GPF) treatment. The Eating Disorder Examination (EDE) was performed at the beginning of treatment and after 6 months, together with the administration of self reported questionnaires for depression (BDI) and Anxiety (STAI). A significant (p<0.001) reduction of the number of monthly binge episodes (from 25.0+/-12.9 to 6.2+/-3.8 and from 24.8+/-9.1 to 8.0+/-4.3) for CBT and GPF respectively were observed. Similar reductions were

American Academy of Pediatrics. Behavioral and cognitive effects of anticonvulsant therapy. Committee on Drugs.

Behavioral disturbances associated with the use of anticonvulsant therapy may be readily apparent and easily diagnosed. They are most commonly reported with the use of barbiturates and usually include hyperactivity, disturbed sleep, irritability, and emotional lability. But, subtle behavioral disturbances may also adversely affect performance and learning. Behavioral changes resulting from therapy with barbiturates and benzodiazepines tend to be idiosyncratic as opposed to the dose-related affects seen with phenytoin and valproate therapy. Carbamazepine and valproate can affect mood and behavi

Mindfulness-based cognitive therapy (MBCT) in patients with obsessive-compulsive disorder (OCD) and residual symptoms after cognitive behavioral therapy (CBT): a randomized controlled trial.

Up to one-third of individuals with obsessive-compulsive disorder (OCD) do not benefit from evidence-based psychotherapy. We examined the efficacy of mindfulness-based cognitive therapy (MBCT) as a complementary treatment option. In a prospective, bicentric, assessor-blinded, randomized, and actively controlled clinical trial, 125 patients with OCD and residual symptoms after cognitive behavioral therapy (CBT) were randomized to either an MBCT group (n = 61) or to a psychoeducational group (OCD-EP; n = 64) as an active control condition. At post-treatment, there was no significant benefit of M…

Dynamic change patterns in suicidal ideation differ in Brief Cognitive Behavioral Therapy (BCBT) and Present-Centered Therapy (PCT).

OBJECTIVE: Although Brief Cognitive Behavioral Therapy (BCBT) is more effective than Present-Centered Therapy (PCT) for preventing suicide attempts, the two treatments have similar efficacy for reducing suicidal ideation severity. Traditional severity scores may obscure meaningful differences in how specific dimensions of suicidal ideation actually change during treatment. This study examined how quickly and consistently different dimensions of suicidal ideation changed during BCBT and PCT. METHOD: The first five items of the Scale for Suicidal Ideation (wish to live, wish to die, reasons for…

Cognitive behavioral therapy for bipolar disorder: patient-level participation barriers and ketogenic metabolic therapy as a candidate adjunct.

This conceptual analysis synthesizes patient-level participation barriers described in the bipolar disorder CBT literature and considers ketogenic metabolic therapy (KMT) as a candidate adjunct within a delivery-focused framework. CBT is an evidence-based adjunctive psychotherapy for bipolar disorder, but its clinical value depends on whether patients initiate treatment, sustain session continuity, and complete between-session practice that supports skill use in daily life. In routine care, psychotherapy uptake remains limited, discontinuation can occur early, and patient-level barriers can co
