Burden of gallbladder and biliary tract cancer (GBTC) in the middle east and North Africa (MENA) from 1990 to 2021: from the global burden of disease (GBD).

INTRODUCTION: Gallbladder and biliary tract cancer (GBTC) is a rare but aggressive malignancy with poor survival outcomes, mainly due to late-stage diagnosis and limited treatment options. Despite prominent global disparities in incidence and outcomes, comprehensive data on the GBTC burden in the Middle East and North Africa (MENA) region remain scarce. MATERIALS AND METHODS: We used Global Burden of Disease (GBD) 2021 data. We retrieved incidence, deaths, and DALYs, including crude and age-standardized rates per 100 000, with 95% uncertainty intervals for 21 MENA countries, the MENA region,

Raised carbohydrate antigen 19-9 levels detect recurrences and impacts overall and disease-free survival in radically resected gallbladder cancer: A simple surveillance marker?

BACKGROUND: There are no established markers which can be used for surveillance after curative resection in gallbladder carcinoma (GBC). Though carbohydrate antigen 19-9 (CA 19-9) has low specificity as a diagnostic marker, its role as a surveillance marker has not been explored. The aim of this study is to evaluate the predictive ability of CA 19-9 as a surveillance marker to detect recurrences on follow-up. METHODS: A retrospective analysis of a prospectively maintained database of radically resected GBC who were either on observation or completed adjuvant therapy (chemotherapy or chemoradi

Risk factors for postoperative respiratory complications and their predictive value. A study in 40-75 year-old men undergoing elective surgery for peptic ulcer or gallbladder disease.

Age, body weight, smoking, chronic bronchitis and duration of anaesthesia were assessed as risk factors for respiratory complications following surgery for peptic ulcer or gallbladder disease. The studies were made on 53 men aged 40-75 years. All variables were associated with increased postoperative risk of chest X-ray abnormality or arterial hypoxaemia or clinically overt respiratory complications. In the statistical analysis, however, none of the factors proved to be useful as a predictor of postoperative respiratory complications. The observed high frequencies of X-ray abnormalities (54%)

Guts, Glucose, and Gallbladders: The Protective Role of GLP-1/GIP Receptor Agonists Against Biliary Complications in Patients with Type 2 Diabetes and Inflammatory Bowel Disease.

Background: Patients with type 2 diabetes mellitus (T2DM) and inflammatory bowel disease (IBD) face elevated risk of hepatobiliary complications. The biliary safety of GLP-1 and dual GLP-1/GIP receptor agonists in this population is uncertain. Methods: We conducted a retrospective cohort study using the TrinetX LIVE global health research network. Adults (≥18 years) with coexisting T2DM and IBD were assigned to exposure (semaglutide or tirzepatide) or comparator (no GLP-1/GIP therapy) cohorts. The index was first prescription (or matched date). Primary outcomes-cholelithiasis, cholecystitis, c

Single center experiences of needle-scopic grasper assisted single incision laparoscopic cholecystectomy for gallbladder benign disease: comparison with conventional 3-port laparoscopic cholecystectomy.

PURPOSE: Single incision laparoscopic cholecystectomy (SILC) has some technical problems. Our group has performed needlescopic grasper assisted SILC (nSILC) to overcome these problems. In this study, we introduce our technique and evaluate the safety and feasibility of this technique compared with the conventional laparoscopic cholecystectomy (CLC). METHODS: The medical records of 485 patients who received nSILC and CLC were reviewed retrospectively. Surgical outcomes including operative time, hospital stay, postoperative pain and perioperative complication were compared between the 2 techniq

In vivo 1 H MRS of human gallbladder bile in understanding the pathophysiology of primary sclerosing cholangitis (PSC): Immune-mediated disease versus bile acid-induced injury.

Primary sclerosing cholangitis (PSC) has been considered to be either an "autoimmune disease" or a "bile acid-induced injury." In vitro MRS studies on PSC patients have limitations due to the contamination of bile with contrast agent (commonly administered during endoscopic retrograde cholangiopancreatography) and/or the use of patient cohorts with other diseases as controls. The objective of this study was to quantify biliary metabolites using in vivo 1 H MRS and gain insight into the pathogenesis of PSC. Biliary metabolites in 10 PSC patients and 14 healthy controls were quantified in vivo u

Pain persists in many patients five years after removal of the gallbladder: observations from two randomized controlled trials of symptomatic, noncomplicated gallstone disease and acute cholecystitis.

After removal of the gallbladder, pain may persist in some patients. To study this condition, 124 patients from two randomized trials, including those with symptomatic noncomplicated gallbladder stones (n = 90) and acute cholecystitis (n = 34), were interviewed, while 139 patients (90%) excluded from both trials responded to a questionnaire 5 years after the operation. Thirty-four patients (27%) of those randomized had pain; 23 (18%) had diffuse, steady pain; and 11 (9%) had pain attacks resembling their preoperative symptoms. A significant dominance of diffuse pain occurred in women (P = 0.02

Surgical Techniques: Robotic versus conventional Laparoscopic cholecystectomy IN benign Gallbladder disease: a randomized controlled, open, parallel, non-inferiority, single-center trial (STaRLING trial).

BACKGROUND: Robotic surgery is increasingly being adopted for gallbladder removal, despite the absence of randomized trials comparing its safety and patient outcomes to the current gold standard, laparoscopic cholecystectomy (LC). Existing literature reviews have found several case series showing no significant differences in postoperative complications or recovery between robotic cholecystectomy (RC) and LC. This trial aims to evaluate whether RC is non-inferior to conventional LC. METHODS: This is a prospective single-center, randomized controlled, parallel, non-inferiority trial. All adult

Global, regional, and national burden of gallbladder and biliary tract cancer from 1990 to 2021 and projections to 2050: a systematic analysis integrating the Global Burden of Disease 2021 and Mendelian randomization.

BACKGROUND: Gallbladder and biliary tract cancer (GBTC) imposes a growing global public health burden, with significant socioeconomic disparities. We assessed trends from 1990 to 2021, quantified inequalities, projected future burden, and evaluated the causal role of high body mass index (BMI) using Mendelian randomization (MR). METHODS: Using Global Burden of Disease (GBD) 2021 data, we analyzed global/regional/national incidence, mortality, and disability-adjusted life years (DALYs) for GBTC. We decomposed drivers of burden changes, quantified cross-country inequalities with slope index of

Is routine single-incision laparoscopic cholecystectomy feasible? A retrospective observational study.

BACKGROUND: Single-incision laparoscopic cholecystectomy (SILC) has been shown to be safe for uncomplicated gallbladder diseases. Routinely applying SILC is debatable. METHODS: Two hundred SILCs were performed with single-incision multiple-port longitudinal-array and self-camera techniques. RESULTS: Eighty-eight (44%) procedures were scheduled for complicated diseases. The routine group had a higher comorbidity rate, a lower preoperative endoscopic retrograde cholangiopancreatography rate, a higher intraoperative cholangiography rate, a higher proportion of complicated gallbladder diseases, …

The Application of Probe-Based Confocal Laser Endomicroscopy as a Diagnostic Tool in Choledochoscopic Gallbladder-Preserving Surgery.

BACKGROUND AND AIMS: Choledochoscopic gallbladder-preserving surgery (CGPS) has the advantage of treating benign gallbladder diseases on the premise of gallbladder preservation. However, it has no reliable preoperative diagnosis if the gallbladder is benign. Probe-based confocal laser endomicroscopy (pCLE) can obtain real-time and clear endoscopic images at the cell level in vivo. It is widely used in the diagnosis of digestive system diseases, but not in gallbladder diseases yet. We applied these two technologies in a complementary way into the diagnosis of gallbladder diseases and thereby li…

Incidental findings during routine pathological evaluation of gallbladder specimens: review of 1,747 elective laparoscopic cholecystectomy cases.

Introduction Cholecystectomy for benign gallbladder diseases can lead to previously undiagnosed gallbladder cancer during histopathological evaluation. Despite some controversy over its usefulness, histopathological evaluation of all gallbladder specimens is common in most hospitals. We evaluated the results of routine pathology of the gallbladder after cholecystectomy for benign gallbladder diseases with regard to unexpected primary gallbladder cancer (UPGC). Methods Patients undergoing cholecystectomy because of benign gallbladder diseases between 2009 and 2013 were enrolled in this study. A…
