From Unmet Medical Need to Drug Candidate: A Translational Therapeutic Development Roadmap Illustrated by Dual-Payload Antibody-Drug Conjugates.

Transformative therapeutic innovation should not begin with a molecule-or even a molecular target. It should begin with a clearly defined unmet clinical need. Here, we present a seven-step Translational Therapeutic Development Roadmap that systematically connects an unmet medical need to a developable drug candidate through the disciplined sequence of (i) defining the need, (ii) understanding disease and resistance biology, (iii) building a mechanistic hypothesis, (iv) defining a target product profile (TPP), (v) molecular design and experimental validation, (vi) developability and manufactura

Evolving antibody-drug conjugates in breast cancer from precision delivery to tumor microenvironment reprogramming.

Antibody-drug conjugates (ADCs) have emerged as an important therapeutic strategy for breast cancer, particularly in the context of its molecular heterogeneity. Initially developed as targeted cytotoxic delivery systems, ADCs are increasingly being explored as multifunctional platforms that may also influence the tumor microenvironment (TME). The expansion of ADC targets beyond human epidermal growth factor receptor 2 (HER2) to include molecules such as trophoblast cell surface antigen 2 (TROP-2) and human epidermal growth factor receptor 3 (HER3) has broadened the therapeutic landscape, offer

[Prevention of rheumatoid arthritis : Current study results on the therapeutic intervention in patients at risk].

Preventive interventions in patients at increased risk of developing rheumatoid arthritis (RA) have gained in importance in recent years. Current study data show that early, targeted immunomodulatory treatment in carefully stratified high-risk patients can delay the onset of clinically manifest RA and reduce the disease burden in the sense of secondary prevention. This review summarizes the evidence from four key studies and discusses their clinical implications, taking the particular legal and practical challenges associated with off-label use into account.

Modular Assembly of Bioconjugates Enabled by a Pyridine-Based Chemoselective Sequential Conjugation Platform.

Site-selective Cys bioconjugation is central to chemical biology and therapeutic development. Owing to the widespread availability of amine-containing building blocks, the modular assembly of alkylamines and Cys holds significant promise for diverse applications. However, existing strategies often suffer from reversible linkages, stereochemical heterogeneity, or limited modularity. Here we report a programmable pyridine-based chemoselective sequential conjugation platform that integrates a thianthrenium leaving group and a fluorine handle on a single pyridine scaffold, enabling sequential, ort

Antibody-drug conjugates as immuno-oncology agents in colorectal cancer: targets, payloads, and therapeutic synergies.

Colorectal cancer (CRC), particularly the immunologically "cold" microsatellite-stable (MSS) subtype, remains profoundly resistant to immune checkpoint inhibitors. Antibody-drug conjugates (ADCs) are rapidly emerging as a transformative therapeutic modality poised to overcome this challenge. This review reframes ADCs beyond their role as targeted cytotoxics, repositioning them as sophisticated immuno-oncology agents. The central thesis is that by strategically selecting payloads such as topoisomerase inhibitors or auristatins, modern ADCs can induce immunogenic cell death (ICD) or pyroptosis.

The evolving landscape of antibody-based cancer therapies: From monospecific to multi-specific and beyond.

The landscape of antibody-based cancer therapies has evolved dramatically since the first monoclonal antibody (mAb) was approved for cancer treatment three decades ago. Advances in recombinant DNA and molecular engineering have transformed monospecific mAbs into multi-specific and multi-modal constructs, revolutionizing oncology. Unlike traditional mAbs, next-generation antibodies, including bispecific antibodies, antibody-drug conjugates (ADCs), and radionuclide-antibody conjugates (RACs), enable synergistic targeting of tumor-associated antigens (TAAs), immunomodulatory receptors, and immune

STING activation and overcoming the challenges associated with STING agonists using ADC (antibody-drug conjugate) and other delivery systems.

In current immunotherapy cGAS (cyclic GMP-AMP synthase)-STING (stimulator of interferon genes) pathway considered as most focused area after CAR-T cell. Exploitation of host immunity against cancer using STING agonists generates the most interest as a therapeutic target. Classically cGAS activation through cytoplasmic DNA generates 2'3'cGAMP that is naturally identified STING agonist. Activation of STING leads to activation of type-1 interferon response and pro-inflammatory cytokines through TBK/IRF-3, TBK/NF-κB pathways. Pro-inflammatory cytokines attract immune cells to the tumor microenviro

CD30 as a Target Molecule in the Diagnosis and Therapy of Lymphomas.

The tumor necrosis factor (TNF)-receptor superfamily 8 receptor CD30 molecule is expressed in all tumor cells of Hodgkin lymphoma and anaplastic large cell lymphoma but is only weakly expressed in a small subset of large lymphoid cells in normal peripheral lymphoid tissues. This makes this molecule an important target for the diagnosis and treatment of CD30-expressing lymphomas. We describe the road to the discovery of the CD30 molecule and the way CD30 has contributed to more precise diagnosis and classification of lymphomas. Moreover, we address how anti-CD30 immunotherapy was developed and

[Diffuse interstitial lung disease induced by antibody-drug conjugates].

INTRODUCTION: Antibody-drug conjugates (ADCs) represent a promising new therapeutic class in non-small-cell lung cancer (NSCLC) patients. Studies assessing ADC have highlighted a pulmonary toxicity profile in the form of interstitial lung disease (ILD). STATE OF THE ART: Several ADCs for NSCLC are currently being developed. In studies evaluating Trastuzumab-Deruxtecan (Her-2 target), incidence of drug-induced ILD ranged from 10.7 to 26.0%, and from 3.6 to 25.0% in those evaluating Datopotamab-Deruxtecan (TROP-2 target). Incidence of 9.9 and 5% of ILD was observed with Telisotuzumab-Vedotin (c

Intracranial activity of antibody-drug conjugates in advanced non-small cell lung cancer: What have we accomplished so far?

Antibody-drug conjugates (ADCs) represent a rapidly evolving class of therapeutics in non-small cell lung cancer (NSCLC), offering targeted delivery of cytotoxic payloads to tumor cells while minimizing off-target toxicity. Although ADCs have demonstrated promising results in NSCLC, their efficacy in treating central nervous system (CNS) metastases has been uncertain due to concerns over blood-brain barrier (BBB) penetration and the lack of dedicated CNS-specific evaluations in clinical trials. However, emerging evidence challenges this paradigm. While earlier-generation ADCs such as T-DM1 exh

[A Case of Multiple Liver Metastases from Breast Cancer Treated with Sacituzumab Govitecan Using a Scalp Cooling System].

The patient was a 51-year-old woman who underwent surgery for bilateral breast cancer in 2021. In 2023, 2 years after surgery, multiple liver metastases were detected. Although she was treated with paclitaxel(PTX)plus bevacizumab(Bev), followed by pembrolizumab(Pemb)in combination with carboplatin(CBDCA)and gemcitabine(GEM), the liver metastases showed progressive disease. Sacituzumab govitecan was initiated as a third-line therapy in December 2024. At that time, a scalp cooling system was used, resulting in hair loss equivalent to CTCAE Grade 1. The patient did not require a wig, and the scal

Systematic profiling of tumor-associated antigen expression for antibody-drug conjugate in prostate cancer.

BACKGROUND: The aim of this study is to better characterize the expression profiles of well-established tumor-associated antigen (TAA) for antibody-drug conjugates (ADC) in primary lesion (PL) and bone metastatic lesion(BML) of prostate cancer (PCa). METHODS: Mass spectrometry (MS) and immunohistochemistry (IHC) were used to comprehensively compare the expression of HER2, NECTIN4, TROP2, PSMA, TF, STEAP1 and B7H3 in the matched cohort (n = 27 pairs), which included matched PL and BML samples from the same patients. IHC was then used to validate the expression of these TAAs in another independ

Nausea and vomiting in an evolving anticancer treatment landscape: long-delayed and emetogenic antibody-drug conjugates.

Nausea and vomiting are common, distressing side effects associated with chemotherapeutic regimens, resulting in reduced quality of life and treatment adherence. Appropriate antiemetic prophylaxis strategies may reduce/prevent chemotherapy-induced nausea and vomiting (CINV). Historically, investigators assessed antiemetics up to 120 hours after chemotherapy. However, CINV can extend beyond this time. Thus, the effect of antiemetics during the long-delayed period (>120 hours) requires investigation. Emerging treatment options, including certain antibody-drug conjugates (ADCs), are associated wi

Sequencing novel agents in the treatment of classical Hodgkin lymphoma.

INTRODUCTION: Classical Hodgkin lymphoma (cHL) is a curable disease, with durable remission achieved in about 80% of patients following first-line treatment. Three new drugs were introduced to the daily use in cHL: brentuximab vedotin (BV), nivolumab, and pembrolizumab. All three drugs were initially approved for the treatment of relapsed/refractory cHL (RRHL) and with their promising outcomes, they are now incorporated in different stages of the treatment. AREAS COVERED: We performed a literature search using PubMed on all cHL studies investigating BV and CPIs within the past 10 years. We an
