The authors' experience with seven patients with intractable pain that was treated by continuous intraventricular infusion of morphine through an implanted Infusaid pump is reported. The pain was caused by head and neck cancer in six patients and was associated with postpolio syndrome in one. The average follow-up was 7 months. Pain was effectively managed through intraventricular administration of a combination of morphine and mild oral narcotic analgesics. Complications included one case of transient respiratory depression, one pump pocket infection, and one pump failure. The morphine dose r

Neurotransmitter replacement therapy in Alzheimer's Disease is currently being attempted using bethanechol chloride (Urecholine) infused intracerebroventricularly with an Infusaid continuous infusion pump. The rationale of this therapy is based on the severe cortical pre-synaptic cholinergic deficit in the presence of relatively normal post-synaptic muscarinic receptor density. Patients are selected on the basis of strict clinical criteria at a functional stage 4 or 5 of Reisberg. A cortical biopsy at the time of pump and catheter implantation confirms the diagnosis by histological and biochem

Apomorphine pumps are increasingly used for the management of patients with advanced Parkinson's Disease. We report a unique case of hemoperitoneum and penetrating colonic injury resulting from malpositioning of a subcutaneous apomorphine device-a complication not previously described in the literature. Seventy-six-year-old man with advanced Parkinson's Disease presented with acute abdominal pain 3-hours post the re-siting of his apomorphine infusion device. Contrast-enhanced computed tomography (CT) of the abdomen and pelvis revealed high-density fluid (47-67 Hounsfield Units), consistent wit

Extracorporeal membrane oxygenation (ECMO) in patients following intracerebral hemorrhage (ICH) poses a critical dilemma between circuit thrombosis and catastrophic rebleeding. While nafamostat mesylate (NM) has been increasingly explored for ECMO anticoagulation in patients with ICH, it is conventionally administered as a systemic anticoagulant with high activated partial thromboplastin time (aPTT) targets, which still carries significant bleeding risks. Moreover, whether NM can achieve effective regional anticoagulation in venovenous (VV) ECMO remains highly controversial. Here, we present a

BACKGROUND: Although postoperative spinal epidural hematoma is a rare complication, it can cause severe neurologic complications. Studies regarding biportal endoscopic spinal surgery, a type of minimally invasive spinal surgery technique, have been recently reported. The purpose of our study is to report the incidence and risk factors of postoperative hematoma after biportal endoscopic spinal surgery.
METHODS: The subjects included 310 patients that underwent biportal endoscopic spinal surgery from 2015 to 2017. Magnetic resonance imaging (MRI) was performed in all patients before surgery, an

BACKGROUND: Narcotic pain medications are a significant component of most postoperative pain control regimens. Although they are usually effective, they produce several untoward side effects and sometimes provide inadequate analgesia. The continuous infusion of local anesthetic agents (via pain pump) has been used to supplement narcotic analgesics after various surgical procedures. The purpose of this study was to examine the effectiveness of the pain pump after cosmetic breast augmentation.
METHODS: Twenty-five women were enrolled in the study; five were eliminated from analysis because of d

The continuous infusion of Angiotensin II (Ang II) via osmotic pumps is a widely utilized approach for establishing animal models of hypertensive myocardial fibrosis. Nevertheless, this technique exhibits certain limitations in current reports, such as the instability of Ang II, inaccurate pump placement, and the lack of a standardized procedure. Current reports on the methodology for establishment remain relatively limited. This protocol has systematically optimized the establishment of Ang II osmotic pump-induced hypertensive myocardial fibrosis models in mice, including the standardized pre

BACKGROUND: The purpose of this study was to determine effects of amlodipine, a dihydropyridine calcium channel blocker, on development of angiotensin II (AngII)-induced vascular pathologies.
METHODS AND RESULTS: Male LDL receptor -/- mice were infused with vehicle, amlodipine (5 mg/kg/d), AngII (1,000 ng/kg/min), or AngII + amlodipine for 4 weeks through osmotic pumps (n=10/group). Mice were fed a saturated fat-enriched diet for 1 week prior to pump implantation and during 4 weeks of infusion. Infusion of amlodipine resulted in plasma concentrations of 32 ± 2 ng/ml and 27 ± 2 ng/ml for mice

Hypoparathyroidism is the most common endocrinological feature in children with Sanjad-Sakati syndrome. Treatment includes active vitamin D and calcium supplementation. Here, we report a case of a newborn with Sanjad-Sakati syndrome who had severe hypocalcemia since birth who responded to PTH subcutaneous pump infusion. The child was born at 35 weeks with hypocalcemia since the first day of life. The standard medical treatment proved ineffective for the newborn, necessitating the administration of unusually high doses of oral and IV calcium and vitamin D analogue for a 2 months. As a result, i

6 insulin-requiring diabetics were treated at home with continuous, long-term, dual-rate subcutaneous insulin infusion (CSII) by means of a portable syringe pump. The duration of infusion was 48--111 days. Patients were initially stabilised on CSII in hospital and then allowed home, where capillary-blood glucose control was monitored by the patients with glucose-oxidase reagent strips. Patients diluted and changed their own insulin for the pump, adjusting the dose according to the control achieved. Mean (+/- SD) blood-glucose values ranged from 4.8 +/- 1.6 to 7.5 +/- 1.6 mmol/l. In order to suppress the pituitary-ovarian axis for treatment of endometriosis, the luteinizing hormone-releasing hormone (LH-RH) agonist buserelin (Hoechst Canada Inc., Montreal, Quebec, Canada) was administered by continuous subcutaneous (SC) infusion using an osmotic minipump system. The down-regulation of gonadotropin release was confirmed by the absence of LH and follicle-stimulating hormone (FSH) stimulation at pump change every 2 weeks. Between week 3 and week 8 of treatment, a marked decrease in the urinary excretion of buserelin coincided with an increase in serum gonadotropins and es

Literature finds improper intravenous (IV) infusion rates as the most common cause of medication administration errors (MAE). Calculating drip rates and manipulating roller clamps while counting drops within the drip chamber to manage IV infusions - known as the traditional method (TM) - increases the likelihood of IV MAEs compared to electronic infusion pumps. The DripAssist, a novel in-line device, allows users to monitor and adjust infusion rates without calculating rates or counting drops. We conducted a prospective, randomized, crossover study with a convenience sample of U.S. Army medics…

BACKGROUND AND PURPOSE: Ziconotide is a peptide that blocks N-type calcium channels and is antihyperalgesic after intrathecal (IT) delivery. We here characterize the spinal kinetics of IT bolus and infused ziconotide in dog.
EXPERIMENTAL APPROACH: Male beagle dogs (N= 5) were prepared with chronic IT lumbar injection and cerebrospinal fluid (LCSF) sampling catheters connected to vest-mounted pumps. Each dog received the following: 1) IT bolus ziconotide (10 µg + 1 µCi (3) H-inulin); 2) IT infusion for 48 hours of ziconotide (1 µg/100 µL/hour); 3) IT infusion for 48 hours of ziconotide (5 µg/1

Ten per cent of patients with haemophilia A develop intracranial haemorrhage (ICH) with a mortality rate of 30% and an incidence of psycho-neurological sequelae in 50% of survivors. ICH may be spontaneous or in association with trauma and other pathology. The generally recommended management is conservative replacement therapy using bolus injections of factor VIII and no neurosurgical intervention. Adjusted dose continuous infusion therapy provides an alternative method of factor VIII administration that is simple, more cost effective and safer through the maintenance of stable plasma VIII:C l
