Trends in Palliative Care Consultation for Extremely Premature Neonate Nonsurvivors in the Intensive Care Unit.

BACKGROUND: Extremely premature infants (<28 weeks gestational age) have a greater risk of mortality and, when they survive, higher rates of significant neurological disability and need for medical care. As a result, extremely premature infants and their families often benefit from early involvement of palliative care. OBJECTIVES: To identify possible associations between palliative care consultation and length of life, code status, and manner of death of extremely premature versus nonextremely premature neonates. METHODS: A retrospective, cohort study of premature infants with a terminal ad

Challenge of antimicrobial-resistant organisms in neonatal intensive care units: a comprehensive review.

The escalation of antibiotic-resistant organisms (AROs) represents a formidable challenge to clinical practice and public health globally. Within the landscape of Gram-positive pathogens, methicillin-resistant Staphylococcus aureus and vancomycin-resistant enterococci remain the most prevalent threats. Concurrently, the rise of Gram-negative AROs, including multidrug-resistant Escherichia coli, Klebsiella pneumoniae and carbapenem-resistant Enterobacterales, has severely curtailed available therapeutic options. These pathogens are associated with significant morbidity and mortality in neonatal

Quality of neonatal intensive care in Africa: a systematic review and meta-analysis.

BACKGROUND: High-quality neonatal intensive care can prevent a significant proportion of neonatal deaths. A neonatal intensive care unit (NICU) is essential for managing life-threatening complications among small and sick newborns. However, little is known about the quality of neonatal intensive care units in Africa. Therefore, this study aimed to estimate the pooled proportion of neonates who received essential quality of care in NICUs, assess care outcomes, and investigate variability across care elements. METHODS: A systematic review and meta-analysis were conducted using studies reporting

Breathe Easier: Optimizing Continuous Positive Airway Pressure in the Neonatal Intensive Care Unit.

BACKGROUND: Rates of bronchopulmonary dysplasia (BPD) continue to increase despite advances in healthcare. Various methods of delivering continuous positive airway pressure (CPAP) can be contributing factors that increase the risk of developing BPD. A recent increase in BPD rates at a local neonatal intensive care unit (NICU) prompted further investigation into potential preventative practices. PURPOSE: The project's goal was to reduce rates of BPD in the NICU by 20% after transitioning to FlexiTrunk CPAP, as well as through education to increase nursing confidence, which will improve complia

Antibiotic stewardship in the neonatal intensive care unit and prevention of antimicrobial resistance.

Antimicrobial resistance is an evolving threat to infants admitted to the neonatal intensive care unit. Antibiotic-resistant organisms may colonize infants, cause infections, or contribute to nosocomial outbreaks, and are associated with infant morbidity and mortality. As microorganisms continue to acquire resistance to available antimicrobials, infants become at risk of therapeutic failure. One key strategy to prevent the development of antimicrobial resistance is through antimicrobial stewardship to optimize antimicrobial use. This review starts with an overview of neonatal sepsis and driver

Case Report: An Unexpected Herbaspirillum Huttiense Bacteremia in the Neonatal Intensive Care Unit.

Herbaspirillum huttiense is a gram-negative bacterium that is rarely implicated in human infections. This case underscores the risk of opportunistic infections in immunocompromised neonates receiving high-dose corticosteroid therapy. It contributes to the scientific literature by expanding the current understanding of neonatal Herbaspirillum infections. A preterm infant was admitted to the neonatal intensive care unit due to thrombocytopenia secondary to maternal chronic idiopathic thrombocytopenic purpura. During high-dose methylprednisolone therapy, Herbaspirillum huttiense was isolated from

Staphylococcus caprae Infections in Neonatal Intensive Care Units: A Report of Two Cases.

Staphylococcus caprae (S. caprae) is a coagulase-negative staphylococci. This group of bacteria is typically part of the skin flora but can become pathogenic in susceptible hosts, such as preterm infants with prolonged stays in neonatal intensive care units. Two preterm newborns with late-onset sepsis caused by S. caprae, one of whom also developed meningitis. Antibiotics were initiated, and both infants had favorable outcomes. While S. caprae is a well-documented agent of infection in adults, there are only rare reports of infection in neonates, particularly in preterms. Cerebral near-infrared spectroscopy guided neonatal intensive care management for the preterm infant.

Infants requiring admission to the neonatal intensive care unit (NICU) are particularly vulnerable to developing brain injury. The severity of the underlying clinical conditions and the complexity of care call for continuous, cot-side, non-invasive monitoring tools. Near-infrared spectroscopy (NIRS) measures the regional tissue oxygen saturation of hemoglobin (rStO2) and provides continuous information on the net-result of several factors. Cerebral rStO2 correlates with echocardiography-derived measures of blood flow. Cerebral fractional tissue oxygen extraction provides information on the bal

Acute Kidney Injury in Neonatal Intensive Care Unit: Epidemiology, Diagnosis and Risk Factors.

Acute kidney injury (AKI) is associated with long-term consequences and poor outcomes in the neonatal intensive care unit. Its precocious diagnosis represents one of the hardest challenges in clinical practice due to the lack of sensitive and specific biomarkers. Currently, neonatal AKI is defined with urinary markers and serum creatinine (sCr), with limitations in early detection and individual treatment. Biomarkers and risk factor scores were studied to predict neonatal AKI, to early identify the stage of injury and not the damage and to anticipate late increases in sCr levels, which occurre

Mortality in the neonatal intensive care setting: Do benchmarks tell the whole story?

AIM: Australian neonatal mortality data are collected and shared within collaborative networks. Individual unit outcomes are benchmarked between units and presented in quarterly or yearly reports. Low mortality is commonly interpreted as optimal performance. However, current collected data do not differentiate between death due to severe illness and death following treatment limitation. This study aims to explore the physiological condition immediately before death, and the proportion of deaths attributed to treatment limitation. METHODS: This retrospective single centre study of 100 consecut
